To label a revision in Google Docs, open version history, choose the revision, and select More > Name this version. To rename the document itself, click its title at the top and enter a new one. A named version marks a point in the document’s history; renaming changes the file’s title.
Choose whether to name a revision or rename the document
Google Docs treats these as separate actions. A named version is a label for a revision in version history. A document name is the title of the file. Changing one does not do the other.

Open the document on a computer.

Open version history. Use the Last edit or Version history control, or select File > Version history > See version history.

Find the revision you want to label. Open its More menu and select Name this version.

Enter a short, descriptive label and save it. To name the active revision, Google also documents the route File > Version history > Name current version.
Google documents a limit of 40 named versions per document. This is the limit for Docs; Google lists a separate 15-name limit for spreadsheets. Google Docs Editors Help: Create, view, or download a file.

Click the document’s title at the top of the file.

Type the replacement name and press Enter.
This changes the file’s title, not the labels on revisions in its history. Google’s instructions for renaming a file are on its version history help page.

Version history also lets users with edit permission work from an earlier revision. These choices have different results:

Restore: makes the selected revision the active version of the document.

Make a copy: creates a separate file based on that earlier revision, which you can edit separately.
Be careful when deleting version history
Google’s help page describes deleting historical versions that have not been explicitly named. Deleting version history is a separate action from naming a version, and Google warns that it is permanent and cannot be undone. Do not use history deletion as part of routine naming.
