How to Manage Urgency or Diarrhea While Running

Runner’s diarrhea has no single established cause. Learn how to handle an urgent episode, test possible triggers carefully, and recognize warning signs.

If you suddenly need a toilet while running, slow down or stop and use one if you can; don’t push through severe pain, dizziness, weakness, or significant ongoing diarrhea. Runner’s diarrhea means frequent loose bowel movements during or soon after a run. Urgency by itself is not necessarily diarrhea, and running may not be the cause. A few careful changes to food, timing, and training can help you spot personal patterns, but persistent symptoms or warning signs call for medical advice.

Why running can trigger urgency or diarrhea

“The cause of runner’s diarrhea isn’t clear,” Mayo Clinic Staff wrote in a Mayo Clinic News Network article published July 8, 2021. The article describes several possible contributors rather than one established cause: the jostling of organs during exercise, reduced blood flow to the intestines, changes in intestinal hormone secretion, food volume or unfamiliar foods, and pre-run stress or anxiety. It also notes that food moves more quickly through the bowels of athletes in training. Mayo Clinic News Network

Timing alone does not prove that exercise caused a bout of diarrhea. Infection, medication effects, food intolerance, and digestive disorders can also cause it. Mayo Clinic’s overview of diarrhea causes

What to do when the urge hits mid-run

  1. Slow down or stop. Use a toilet if one is available. Do not try to run through severe pain, dizziness, weakness, or significant ongoing diarrhea.
  2. Pay attention to hydration. Diarrhea can contribute to dehydration. Drink appropriately before, during, and after activity, but do not assume that drinking more will stop urgency; the sources do not establish a symptom-preventing amount or beverage.
  3. Scale back if symptoms continue. Consider reducing your distance or intensity until you feel better, then build back gradually as symptoms allow. Mayo Clinic advises consulting a healthcare provider if its suggested changes do not help. Mayo Clinic’s runner-specific guidance

How to look for your own triggers

Keep a brief log of when symptoms occur and what you ate or drank, including race fuel, and note the run’s duration or intensity. Change one likely trigger at a time. That makes it easier to tell whether a pattern repeats; it does not guarantee that a particular change will work.

Possible trigger Adjustment to consider Timing or qualification
High-fiber or gas-producing foods Try limiting them before a run, rather than removing them from your diet altogether. Mayo Clinic suggests limiting them the day before a run. Runners who run daily should experiment to find a tolerable fiber level.
Sugar alcohols, such as sorbitol Avoid them before running. Mayo Clinic specifies avoiding them for at least one day beforehand.
Caffeine and high-fat foods Try limiting them before a run. Mayo Clinic suggests limiting them in the three to six hours before running.
Food close to the start Try leaving time between eating and running. Mayo Clinic suggests avoiding food for at least two hours before running. This is general guidance, not an individualized nutrition plan.
Energy gels or bars Notice whether a product coincides with symptoms; don’t introduce a new one on race day. These products may contribute for some people. The source does not recommend a particular product.
Dairy, if you are lactose intolerant Consider lactose-reduced or lactose-free dairy products. Mayo Clinic’s suggestion applies to people with lactose intolerance.
NSAIDs, such as ibuprofen or naproxen Ask a clinician about medication concerns; do not change prescribed medicine on your own. Mayo Clinic advises avoiding these because they have been shown to increase gastrointestinal complaints.
Tight waist clothing or hard training Try comfortable, loose-fitting clothes and temporarily reduce intensity or distance if needed. Tight clothing at the waist may aggravate symptoms; return to training gradually as symptoms improve.

These are general suggestions from Mayo Clinic, not a ranking of treatments or a guarantee of results. The source does not establish which adjustment works best; look for a repeatable change in your own symptoms. Mayo Clinic News Network

When diarrhea needs medical attention
