To lower your risk of a first stroke, build habits that support healthy blood pressure and overall cardiovascular health: eat a Mediterranean-style or DASH-style diet, move regularly, avoid tobacco, and prioritize sleep and a healthy weight. Arrange regular preventive care to check blood pressure, cholesterol, and blood sugar; some people also need clinician-directed medication. These steps can reduce risk, but they cannot guarantee that a stroke will not happen.
This U.S.-focused guide is for adults who have not had a stroke. If you have had a stroke or transient ischemic attack (TIA), your prevention plan is different and should be individualized with a clinician.

Why prevention starts with finding risk factors
More than 600,000 people in the United States have a first stroke each year, according to the American Stroke Association in 2024. The association says up to 80% of strokes may be preventable. “Up to” is a population-level estimate, not a forecast for any one person—and it does not mean lifestyle changes alone prevent that share of strokes.

1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsHigh blood pressure, elevated cholesterol, elevated blood sugar, and obesity are among the risk factors preventive care can identify. In 2025, the American Heart Association reported that 46.7% of U.S. adults had high blood pressure, defined in that release as at least 130/80 mm Hg. Regular primary care can help detect problems before symptoms appear and determine whether lifestyle changes, medication, or both are appropriate. Access to care, affordability, neighborhood conditions, and other social factors can also affect prevention.

Make blood pressure a priority
High blood pressure is a leading modifiable risk factor for cardiovascular disease, including stroke. The 2025 AHA/ACC blood-pressure guideline uses these categories for adults:
| Category | Blood-pressure reading |
|---|---|
| Normal | Below 120/80 mm Hg |
| Elevated | Systolic 120–129 mm Hg and diastolic below 80 mm Hg |
| Stage 1 hypertension | Systolic 130–139 mm Hg or diastolic 80–89 mm Hg |
| Stage 2 hypertension | Systolic at least 140 mm Hg or diastolic at least 90 mm Hg |
These are classification categories, not personal treatment instructions. A clinician interprets readings in context and can confirm whether high blood pressure is persistent. Do not start, stop, or change blood-pressure medicine on your own.
Use home readings as part of care
The 2025 guideline recommends home blood-pressure monitoring for patients to help confirm an office diagnosis and track progress as part of an integrated care plan. If your clinician asks you to monitor at home, ask how often to take readings, how to record or share them, and what results should prompt a call. A monitor does not diagnose or treat high blood pressure by itself.

Choose an eating pattern you can sustain
Both Mediterranean-style and DASH-style eating patterns are supported in the cited U.S. guidelines, but in different contexts. The 2024 AHA/ASA stroke-prevention guideline recommends a Mediterranean pattern for adults without prior cardiovascular disease and for those at increased risk; its summary especially notes benefit when the pattern is supplemented with nuts and olive oil. The 2025 blood-pressure guideline describes DASH as a heart-healthy pattern. They are options to discuss and adapt, not competing prescriptions for every person.
| Pattern | What the cited guidance says | Practical focus |
|---|---|---|
| Mediterranean-style | Recommended in the 2024 AHA/ASA stroke-prevention guideline for adults without prior cardiovascular disease and those at increased risk; the summary especially notes nuts and olive oil. | Build meals around the pattern you can maintain; include nuts and olive oil if suitable for you. |
| DASH-style | Described in the 2025 AHA/ACC blood-pressure guideline as a heart-healthy pattern. | Emphasize vegetables, fruits, whole grains, legumes, nuts and seeds, and low-fat or nonfat dairy, with lean proteins and non-tropical oils. |
Pay attention to sodium
In its 2025 adult blood-pressure guidance, the AHA/ACC advises limiting sodium to less than 2,300 mg per day and moving toward 1,500 mg per day. Checking food labels can help you see how packaged foods contribute to your daily total.

Move regularly and break up long periods of sitting
The 2024 stroke guideline summary recommends at least 150 minutes per week of moderate-intensity aerobic activity, 75 minutes of vigorous activity, or a combination, preferably spread through the week. It also identifies prolonged sedentary time while awake as associated with increased stroke risk and recommends counseling to avoid being sedentary.
Start at a level you can manage and build gradually; shorter bouts spread across the week can help you work toward a regular routine. If you have a chronic medical condition or are unsure what activity is safe, check with a health professional before starting an exercise program. In its separate blood-pressure prevention and management context, the 2025 guideline describes 75–150 minutes per week of aerobic and/or resistance exercise.
Avoid tobacco and protect your sleep
Do not smoke or vape
Public American Stroke Association guidance advises against smoking or vaping and recommends avoiding secondhand smoke. If quitting is difficult, support may include a health professional, a quit line, a cessation program, or medication when appropriate.

Make sleep part of prevention
Sleep is one of the American Heart Association’s Life’s Essential 8 health metrics. Its public stroke guidance gives 7–9 hours per night as general adult guidance; individual sleep needs vary, and this is not a stroke-specific dose-response target.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Discuss weight and alcohol in the right context
The 2025 AHA/ACC blood-pressure guideline describes a goal of at least 5% body-weight reduction for adults with overweight or obesity as part of blood-pressure guidance. Weight change is not, by itself, a guarantee of stroke prevention; a clinician can help set an appropriate plan and address other risk factors.

Ask your clinician what to check
A preventive visit can include a physical examination and blood tests for risk factors. Ask how often you should have these checks and what your results mean for your overall risk. Depending on your situation, a clinician may recommend medication such as an antihypertensive or a statin. GLP-1 receptor agonists are discussed for specific patients with diabetes and elevated cardiovascular risk or established disease; they are not general-purpose stroke-prevention drugs.
Make sure your risk assessment reflects your circumstances. The 2024 guideline identifies oral contraceptive use, pregnancy complications, early-onset menopause, and estrogen use for gender affirmation among factors that may matter. They do not affect every person in the same way, so discuss what is relevant to you rather than changing prescribed treatment without medical advice.
- Ask whether your blood pressure, cholesterol, and blood sugar have been checked and whether the results need follow-up.
- Ask whether home blood-pressure monitoring belongs in your care plan and how to take and share readings.
- Discuss a realistic eating and activity plan, including any health conditions or barriers that affect what is practical.
- Review tobacco use, sleep, weight, medications, and personal risk factors with your clinician.
