Those reports answer different questions. Then check what the test examined, what the result classification means, and whether any heteroplasmy percentage applies only to the sample tested. A positive result is not automatically a diagnosis or a prediction; a negative result is limited by the test’s scope; and a variant of uncertain significance does not confirm or rule out disease. For a health-related or unclear finding, review the complete report with the ordering clinician or a genetics professional.
First identify what kind of test produced the result
The purpose of the test sets the boundaries for interpreting its result. A clinical diagnostic assay investigates a health question; a direct-to-consumer (DTC) health report offers information under standards different from provider-driven clinical testing; and an ancestry test traces a maternal lineage. These are not interchangeable. MedlinePlus explains the distinction between DTC and clinical testing in its guide to DTC genetic test results.

Look for the test name, why it was ordered, and the sections of the mitochondrial genome examined. Check whether the report assessed mitochondrial DNA (mtDNA) alone or also included nuclear genes. A result from selected sites, for example, does not answer the same question as a broader sequencing test. The report or its laboratory notes should describe the test’s scope and method.
Crashes, No Sound, or Screen Glitches?
Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minutePC Slower Than It Used to Be?

Record the variant exactly as the report gives it, including its position or reference sequence if listed. Then find the laboratory’s classification and the evidence or explanation that accompanies it. A variant name by itself is not a diagnosis; an online database entry or third-party interpretation of raw data should not replace the laboratory’s assessment.

| Report classification | How to read it |
|---|---|
| Pathogenic or likely pathogenic | The laboratory considers the finding disease-causing or likely disease-causing. Its significance for a particular person still depends on the test, the clinical picture and other relevant evidence. |
| Variant of uncertain significance (VUS) | Available evidence is insufficient, conflicting or incomplete to classify the variant confidently. A VUS does not establish a diagnosis and cannot by itself rule one out. |
| Benign or likely benign | The laboratory considers the change not disease-causing or likely not disease-causing. Read this in the context of the test’s scope and the question being investigated. |
| Negative or no relevant finding detected | The test did not identify a reportable finding within its scope and limits. It does not mean every possible mtDNA or nuclear-genetic cause was assessed or excluded. |
Laboratories may use different methods and criteria, and mtDNA interpretation has specific challenges. A 2020 ClinGen expert-panel publication noted that standardized criteria for mtDNA variant assessment were insufficient at the time, contributing to inconsistencies in pathogenicity reporting. See the ClinGen mtDNA interpretation specifications.
Interpret heteroplasmy as a sample-specific measurement
Heteroplasmy means that a person’s cells contain a mixture of mtDNA copies: some carry a particular change and some do not. A reported percentage describes the fraction measured in the tested sample using that assay. It is not automatically a percentage for every tissue in the body or a precise forecast of disease severity.

- ORIGINS AND INHERITANCE: AncestryDNA is the only DNA test that can show your origins results, DNA matches, and traits by each side of the family, without your parents taking a DNA test.
Clinical effect can depend on the specific variant, its proportion and which tissues carry it. Consequently, two results with similar percentages need not mean the same thing. GeneReviews discusses the role of tissue distribution and heteroplasmy in mitochondrial DNA-associated Leigh syndrome spectrum.
Detection limits are also method-specific. A 2016 Mayo Clinic Laboratories sample report for mitochondrial full-genome analysis by next-generation sequencing gives limits of less than 10% heteroplasmy for point mutations and less than 20% for large deletions. Those figures describe that historical sample report only; they are not universal thresholds or current specifications for all laboratories. Ask what the assay used for your result could detect.

Separate ancestry meaning from health meaning
An mtDNA haplogroup identifies a maternal-line lineage. Because mtDNA is passed through the egg, it traces one ancestral line rather than a person’s complete ancestry. A haplogroup label alone is not a diagnosis of mitochondrial disease. MedlinePlus explains the limits of this kind of result in its overview of genetic ancestry testing.
Put a health result in clinical context
For a suspected mitochondrial condition, a clinician considers the finding alongside symptoms, medical and family history, examination and other diagnostic evidence. A consumer risk result is not a yes-or-no answer about whether someone will develop disease. MedlinePlus advises interpreting genetic test results in relation to the test’s purpose and a person’s history; see what genetic test results mean.

- TRACE ANCIENT MIGRATIONS: Maternal and Paternal Haplogroup reports follow your ancestors' migration paths across continents over thousands of years (paternal haplogroup requires a Y chromosome). Find out how much of your ancestry can be traced back to the Neanderthals.
- PRIVATE BY DESIGN: Your DNA data is encrypted, protected, and always under your control. Enhanced security measures are implemented to keep your information safe. Subject to 23andMe’s Terms of Service and Privacy Statement at 23andMe online.
A negative result can be reassuring only within defined limits: it means the test did not detect a relevant finding it was designed and able to detect. Ask whether the assay covered the relevant mtDNA regions and variant types, what its detection limits were, and whether nuclear genes or other causes may need consideration.

Questions to take to the clinician or genetics professional
- What did this test examine, and what important changes could it miss?
- How was the finding classified, and what evidence supports that classification?
- Does a heteroplasmy percentage apply only to this sample? Could testing a different tissue or using another method change the interpretation?
- Does the result fit the symptoms and family history?
- Would clinical confirmation, broader testing or evaluation of relatives be appropriate?
- What does the finding mean for health care now, and what does it not allow us to predict?
Bring the full report, not just a summary screen, along with relevant symptoms and family history. This helps the professional assess the result against the assay’s purpose and limitations.
