How to Handle Workers’ Comp Forms When a State Updates Its Requirements

When a state updates a workers’ comp form, verify the version, transition dates, recipients, and filing channel on the responsible agency’s current page.

When a state changes a workers’ compensation form or filing process, first identify the state and claim involved, then check that state agency’s current form, instructions, effective and required-use dates, and submission channel. Replace saved templates only after confirming which version applies and whether the agency still accepts the old one. Requirements differ by state; do not assume one state’s transition rules apply elsewhere.

Start with the state and form that govern the claim

Workers’ compensation forms are maintained by state agencies, not through one universal national form list. Confirm the jurisdiction for the particular claim and who is responsible for filing or receiving the document. An employer’s headquarters alone does not establish which state’s claim process applies.

Open the responsible agency’s official forms page and search by form number and title. State catalogs may separate forms by participant or purpose: Texas, for example, organizes forms into categories that include employer, carrier, employee, and medical forms. New York’s listings identify form owners, filing destinations, and current-version details. Use the agency’s own page rather than an unofficial copy.

Check the version and every date that affects the transition

Read the form’s revision date and instructions, along with any linked notice or bulletin. Record the effective date, the date the new version becomes mandatory, any transition period, and the last date the former version will be accepted. These dates can differ.

Maine illustrates the distinction: its forms index says revised forms were effective June 1, 2026, and required as of September 1, 2026. That schedule applies to Maine, not to other states. New York says the Workers’ Compensation Board may announce that older versions of updated forms will no longer be accepted; Georgia’s forms page said outdated forms would be returned beginning October 1, 2025. Check the policy for the particular form instead of assuming an old copy remains valid.

Confirm how the form must be filed

A form update can change the filing channel as well as the document itself. Verify the required portal or mailing address, who must receive copies, and whether a separate electronic transaction is needed. Uploading a document does not necessarily complete every reporting step.

New York: RFA-2 eForm

In an April 9, 2026 payer update, New York said use of the RFA-2 eForm became mandatory effective April 8, 2026. Paper forms postmarked after April 7 would not be accepted or acted on. For this form, follow the newer April update rather than relying on an earlier notice describing a planned mandate.

Pennsylvania: LIBC-764

The LIBC-764 instructions describe mailing the notice to the employee and uploading it to WCAIS or mailing it to the Bureau. They also state that an EDI transaction is needed to update claim status in WCAIS. The PDF carries an April 2023 revision date, so verify the current form and instructions before using it.

Use official agency materials, and distinguish guidance from requirements

Connecticut’s Workers’ Compensation Commission advises parties in its system to use the official forms so documents are up to date, accurate, and meet statutory requirements. The forms are PDFs and are mostly fillable online. Idaho, by contrast, cautions that its policy statements and guidance do not have the force and effect of law. Read the applicable law, regulation, form instructions, and agency notices together; do not treat every explanatory page as a binding rule.

Update your filing process without losing the audit trail

  1. Locate the controlling agency page. Confirm the state, claim, participant, and form number, then open the relevant official form and instructions.
  2. Write down the transition details. Capture the revision date, effective date, required-use date, any old-form cutoff, and the permitted filing methods.
  3. Check all recipients and related transactions. Confirm copies, portal steps, mailing requirements, and whether a companion EDI transaction or other status report is necessary.
  4. Replace or retire internal copies. Update templates, checklists, and filing instructions; clearly remove or mark superseded forms so staff or vendors do not select them by mistake.
  5. Retain the official notice. Keep the agency’s form page, bulletin, or update with the process record. If the instructions remain unclear, contact the agency or the carrier or claims administrator before filing.

When comparing an old and new process, check the valid version, effective and mandatory dates, old-form acceptance, paper versus electronic options, recipients, and any separate status transaction. Also confirm that the form is intended for the right participant, such as an employer, carrier, employee, or self-insured entity.
