What each choice changes

How to choose during Novigrad Dreaming
Novigrad Dreaming is a main quest in The Witcher 3: Wild Hunt. Geralt investigates a nightmare-plagued house and encounters Corrine Tilly, an oneiromancer, and Sarah, a godling who has been living there. The consequential decision is whether Sarah may remain.
- Want Haunted House? Allow Sarah to stay. This preserves the later quest involving Corrine and Sarah.
- Prefer Sarah’s later meeting with Johnny? Expel her. Doing so closes off the Haunted House prerequisite.
Choose based on the character outcome you want; you do not need to pick one option to receive the main-story lead.

The main-story dream is separate from Sarah’s fate
Corrine’s dream sequence points Geralt toward Dandelion and his connection to Ciri in Novigrad. The walkthroughs describe this lead separately from Sarah’s later circumstances, so the Sarah choice is about optional content rather than securing that main-quest information. (IGN; Witcher Wiki)
