How to Freeze and Thaw Blood Samples for Reliable Laboratory Testing

Blood samples do not share one freeze-thaw protocol. The right specimen, temperature, storage time, and thaw procedure depend on the assay and laboratory SOP.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not freeze a blood sample until you have checked the exact test, specimen fraction, collection tube, and current laboratory SOP or test instructions. Whole blood, serum, and plasma are not interchangeable, and there is no universal blood-sample freeze-thaw protocol. The assay determines whether the sample should be separated, how it should be stored, how many freeze-thaw cycles are acceptable, and how it must be thawed and handled afterward.

Start with the test and specimen—not the freezer

“Blood sample” can mean whole blood, serum, or plasma. These are different specimen types, and a test validated for one may not accept another. Freezing also does not make a specimen universally stable: temperature, storage duration, permissible freeze-thaw count, and post-thaw handling depend on the analyte and method.

Before processing or freezing, confirm the test’s current instructions and the laboratory’s validated SOP. Check the required specimen fraction, collection tube and additives, whether and when centrifugation is required, storage temperature and maximum duration, permitted freeze-thaw cycles, and any post-thaw acceptance criteria. CLSI PRE04, first edition published August 3, 2023, covers general handling, transport, processing, and storage principles for whole blood, serum, and plasma. It does not replace test-specific requirements.

  • Whole blood is blood that has not been separated into cellular and liquid fractions. Do not assume it should be frozen: freezing may be unsuitable for a test that requires a different specimen type or intact cells.
  • Serum is the liquid separated after blood has clotted. Some workflows call for separating it from the clot before refrigeration or freezing.
  • Plasma is the liquid fraction obtained from anticoagulated blood. Its handling requirements can differ from serum, including for coagulation testing.

Whether and when to centrifuge depends on the assay and collection system. A centrifugation setting from one test’s instructions is not a general blood-processing setting.

These distinctions matter before the sample reaches storage. A tube may contain an additive intended for a particular specimen type, and the assay may require a specific fraction and processing sequence. Do not treat serum and plasma as interchangeable simply because both are liquid fractions, or assume that a tube collected for one test can be repurposed for another. The receiving laboratory’s requirements take precedence over a general description of specimen handling.

CLSI PRE04 describes general principles for routine examinations using whole blood, serum, or plasma and addresses preexamination variables such as processing, storage equipment, and temperature monitoring. Its role is to help frame careful specimen handling—not to supply one set of freezing instructions for every measurand. For exact limits, the current assay instructions and validated local procedure remain essential.

How to prepare a sample for freezing

Use the test-specific instructions for every processing step. The sequence below is a planning checklist, not a substitute SOP: it identifies what to verify rather than prescribing a universal laboratory method.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Identify the test and verify its current requirements. Confirm the accepted specimen type, tube, processing sequence, storage limits, and shipping conditions with the method instructions or receiving laboratory.
  2. Process only as directed. If the test requires serum or plasma, follow its stated clotting, centrifugation, and separation instructions. Do not infer the correct timing or force from another assay.
  3. Transfer only when the method calls for it. Follow the specified container and transfer procedure. For example, CDC’s instructions for test submission CDC-10244 call for sterile transfer of separated serum to a threaded tube with an O-ring. That container instruction belongs to that workflow, not to every blood test.
  4. Plan aliquots if the protocol permits. Dividing a specimen into appropriately sized portions can reduce the need to thaw and refreeze the same container. CDC’s 2024 syphilis recommendations advise aliquoting plasma to avoid repeated freeze-thaw cycles. Use containers compatible with the specimen, storage temperature, and institutional validation; do not assume that a vial is suitable for clinical diagnostic use just because it is marketed as cryogenic.
  5. Label and store under the validated conditions. Follow the laboratory’s identification, documentation, temperature-monitoring, and storage-equipment procedures. Record handling as required by the quality system and the test instructions.

Freezing is not a substitute for prompt processing. For one specific workflow, CDC’s CDC-10244 instructions say to refrigerate separated serum within 8 hours and to freeze it when storage will exceed 48 hours. Those timings are not general limits for all serum or blood samples.

A useful pre-freezing check is to resolve the questions in order, rather than starting with a convenient freezer setting: What exact assay will be run? Which specimen fraction does it accept? What collection tube and processing sequence are required? How soon must the sample be separated or refrigerated? What storage temperature and duration are allowed? Does the receiving laboratory specify a container or shipping condition? Finally, what limits apply after thawing? If any answer is unclear, pause and contact the laboratory responsible for the assay before changing the specimen’s state.

For example, CDC’s CDC-10244 workflow provides a complete but test-specific set of instructions: separate serum from the clot, refrigerate it within 8 hours, and use the stated storage conditions if shipment or testing will be delayed. It also calls for sterile transfer to a threaded tube with an O-ring. The details are useful as an example of the decisions a written procedure can cover, but they should not be copied into another assay’s SOP without validation.

Aliquot planning should likewise be deliberate, not automatic. Aliquots can make it possible to thaw only the amount needed and avoid cycling a larger container repeatedly. But the source guidance does not establish a universal aliquot size, a universal number of portions, or an extended shelf life because aliquoting was performed. Follow the method’s volume needs, container requirements, and validated handling steps.

Examples of test-specific storage limits

The values below illustrate why one freezer temperature or storage duration should not be applied to every sample. Each row belongs to the named test or guidance workflow.

Source and scope Storage instruction stated by the source Important qualification
CDC test submission CDC-10244 Keep serum at 2–8°C; if storage will exceed 48 hours, freeze at −20°C or lower, for up to 8 weeks before shipping. These are instructions for CDC-10244’s serology submission workflow, not a universal serum storage limit. The workflow also specifies serum separation and transfer instructions. CDC-10244 instructions
CDC, Laboratory Recommendations for Syphilis Testing, United States, 2024 For the described serum collection workflow, allow approximately 15–30 minutes for clot formation at room temperature. The recommendations state 2–8°C and testing within 5 days, or freezing at ≤−20°C for longer storage. The report notes that plasma storage times vary by test. It also says not to store serum in frost-free freezers because their freeze-thaw cycles are detrimental to frozen serum stability. CDC 2024 recommendations
WHO laboratory guidance, cited protocol −65° to −80°C for up to 6 months; no more than two plasma freeze-thaws. These values apply to the protocol in the cited WHO guidance, not to plasma generally. WHO laboratory guidance PDF

These examples cannot be combined into a single recommendation. A storage limit from one serology workflow does not establish the limit for another serology assay, a chemistry test, a molecular assay, or a coagulation test.

The numbers also answer different operational questions. In CDC-10244, the 2–8°C instruction applies to serum held for that workflow, freezing is specified when storage will exceed 48 hours, and the stated frozen limit is up to 8 weeks before shipping. In the CDC 2024 syphilis recommendations, the described serum workflow gives a 15–30 minute clotting interval and a 5-day testing window at 2–8°C, with freezing at ≤−20°C for longer storage. The WHO protocol’s −65° to −80°C range, up-to-6-month duration, and maximum of two plasma freeze-thaws belong together as that protocol’s parameters. None supplies a general rule for all blood-derived specimens.

Do not select a temperature by choosing the coldest value in the table. A colder condition or longer duration is not automatically validated for a different test, and a freezer set point alone does not demonstrate that the specimen was maintained within the required range. Follow the named method’s storage instruction, equipment requirements, and local temperature-monitoring procedures.

Why repeated freeze-thaw cycles matter

Repeated freezing and thawing can affect some analytes. CDC warns that repeated cycles may reduce antibody activity and lead to erroneous serology results. The practical response is to avoid unnecessary cycles and, when the test protocol allows it, prepare aliquots so only the amount needed for a run is thawed. Aliquoting does not itself establish a longer storage life or make a specimen suitable for a different assay.

Do not assume a universal maximum number of cycles. The WHO guidance cited above sets a limit of two plasma freeze-thaws for its particular protocol, while other sources may give different instructions or not establish a cycle limit in the public guidance. Use the current assay instructions and laboratory validation for the sample in hand.

A frost-free freezer is not automatically suitable for long-term serum storage. CDC’s 2024 syphilis recommendations specifically caution that temperature cycling in frost-free freezers can be detrimental to frozen serum stability. Follow the test’s specified equipment and temperature-monitoring requirements rather than judging suitability by the freezer’s nominal set point alone.

When several tests or repeat runs may be needed, think about the planned use of the specimen before freezing. The cited CDC syphilis recommendations specifically describe aliquoting plasma to avoid repeated freeze-thaw. This supports a practical principle: reduce avoidable cycles when the method permits, and keep track of the specimen’s handling history so the laboratory can assess it against the applicable limit. It does not mean every sample should be divided, nor that aliquots may be stored indefinitely.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Repeated cycles are a quality concern because they can compromise the result, not simply because they are inconvenient. CDC’s warning is specifically about loss of antibody activity and erroneous serology results. Other analytes may have different stability behavior, so do not project that particular effect—or a particular cycle limit—onto another test without supporting instructions.

Thaw the specimen according to its assay instructions

There is no single thaw temperature, duration, mixing step, or post-thaw hold time established for all blood-derived specimens. The correct approach is to use the current method insert, receiving laboratory instructions, and validated SOP. Do not invent a water-bath temperature, a fixed thaw time, or a mixing procedure when the method does not specify one.

Before thawing, determine whether the method requires complete thawing, controlled mixing, a particular post-thaw interval, or immediate testing. If the instructions do not resolve a handling question, consult the laboratory responsible for the assay rather than applying a rule from a different specimen type.

After thawing, apply the test’s own acceptance and rejection criteria. There is no general visual rule in the cited sources that makes a sample acceptable or unacceptable based only on its appearance. Whether to remix, refreeze, or reject a specimen must come from assay-specific instructions or laboratory validation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

It is useful to separate thaw instructions into two checks. First, establish the method’s physical handling requirements: how thawing should be performed, whether mixing is specified, and when the specimen must be tested or otherwise processed. Second, establish the method’s acceptance rules: which handling histories are allowed and what to do if the sample was stored or thawed outside the stated limits. The sources here do not set universal answers to either check.

If instructions are missing or conflict, do not resolve the conflict by borrowing a temperature or duration from another specimen workflow. Contact the receiving laboratory or the person responsible for the validated procedure. Record the question and the disposition according to local quality procedures, particularly when an excursion or an undocumented freeze-thaw may affect test acceptance.

Choose storage containers and equipment through validation

When a protocol calls for transferring separated serum or plasma, use the specified container. CDC’s cited guidance includes clean polypropylene tubes for serum or plasma handling and, for CDC-10244, a sterile threaded tube with an O-ring. These examples support checking material and closure compatibility; they do not approve any particular consumer product for clinical testing.

  • Confirm the required sterile status and whether the tube material is compatible with the specimen and assay.
  • Check that the cap and seal are appropriate for the storage temperature and transport conditions.
  • Use only containers and equipment accepted by the laboratory’s validation and quality procedures.
  • Follow laboratory procedures for freezer monitoring and specimen identification; CLSI PRE04 includes consideration of storage equipment and temperature monitoring within preexamination handling.

For coagulation testing specifically, CLSI H21, sixth edition, published April 22, 2024, is a focused reference for plasma-based coagulation specimens, including fresh and frozen plasma stability discussion and specimen rejection criteria. General serum or serology guidance should not be substituted for coagulation requirements.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Polypropylene cryogenic vials or aliquot tubes may be a relevant supply category when a validated method calls for storing separated serum or plasma in that type of container. A product description alone does not establish suitability for a clinical diagnostic workflow. Before selecting a container, confirm its material, sterile status if required, cap and seal compatibility, temperature rating, and compatibility with the institution’s validated procedure. Do not treat a consumer listing or brand claim as laboratory approval.

Freezer and temperature-monitoring equipment are part of specimen-quality procedures, not just storage conveniences. The applicable guidance and SOP may specify equipment and monitoring practices; a stated freezer temperature is meaningful only when the laboratory can maintain and document the required conditions. Follow local procedures for monitoring, identification, and any response to a temperature excursion.

Common mistakes and how to avoid them

  • Freezing before confirming the required specimen. Verify whether the assay accepts whole blood, serum, or plasma before processing or storage.
  • Using a published example as a universal limit. Keep each temperature and duration tied to the test or protocol that states it.
  • Repeatedly thawing one large container. Where the method permits, plan aliquots to limit repeated cycles.
  • Applying centrifugation, clotting, or transfer directions from another test. Follow the specified collection system and assay instructions.
  • Using an unsuitable freezer for frozen serum. For the CDC syphilis workflow, avoid frost-free freezers because their cycles can harm frozen-serum stability.
  • Guessing at post-thaw handling. Confirm the method’s thaw, mixing, hold-time, and acceptance rules; these are not universal.

Another frequent mistake is to focus only on the storage temperature and overlook the steps before and after freezing. A specimen can be stored at a stated temperature yet still fail the workflow’s requirements if it was not separated on time, placed in the specified container, held too long before processing, or thawed in an unvalidated way. Check the entire sequence, from collection and separation through transport and post-thaw testing.

Likewise, avoid interpreting a lack of published detail as permission to improvise. If a public instruction does not state a thaw interval or cycle count, that does not establish that every handling choice is acceptable. The method insert, receiving laboratory, and validated SOP may contain requirements not present in a general overview.

Which guidance should govern the procedure?

Use the sources in the order that matches the specimen and test: first the current assay instructions and receiving laboratory requirements, then the validated local SOP, followed by relevant professional guidance. CLSI PRE04 provides general preexamination principles across whole blood, serum, and plasma, while noting that detailed requirements remain specific to measurands and specimen types. For plasma-based coagulation assays, consult the focused CLSI H21 guidance as applicable.

If a specimen has already been frozen or thawed outside the documented limits, do not decide that it is acceptable solely from its appearance or from a rule intended for another test. Refer the handling history to the laboratory responsible for the assay and apply its validated acceptance criteria.

CLSI PRE04, first edition, was published August 3, 2023. It provides general recommendations for appropriate handling, transport, processing, and storage of specimens used as whole blood, serum, or plasma for routine laboratory examinations. CLSI H21, sixth edition, was published April 22, 2024 and is directed to plasma-based coagulation assays. These references address different scopes; neither turns a test-specific specimen requirement into a universal rule.

In practice, the governing instruction is the one that applies to the specific assay, specimen, and laboratory. If a CDC submission workflow, professional guidance document, and local procedure appear to differ, check their scope and use the current requirements for the test being performed. When uncertainty remains, ask the receiving laboratory before freezing, thawing, or shipping the sample.

Frequently Asked Questions

Can you freeze blood samples?

Sometimes, but “blood sample” is not specific enough to determine whether freezing is appropriate. First check whether the test requires whole blood, serum, or plasma and follow its current instructions. Some cited CDC serology workflows process blood to separate serum before freezing; that is not a universal requirement for every assay.

Can you freeze serum or plasma?

Some test workflows specify frozen storage for separated serum or plasma, but temperature and permitted duration vary. For example, CDC’s CDC-10244 instructions specify serum at 2–8°C and freezing at −20°C or lower when storage will exceed 48 hours, for up to 8 weeks before shipping. That limit applies to CDC-10244, not to all serum or plasma tests.

How many times can you freeze and thaw a blood sample?

There is no universal cycle limit. CDC warns that repeated cycles may reduce antibody activity and produce erroneous serology results. The WHO protocol cited here allows no more than two freeze-thaws for its particular plasma workflow. Use the current assay instructions and laboratory validation for the specimen in question.

How long can serum be frozen?

There is no single storage duration for all serum tests. CDC-10244 specifies up to 8 weeks at −20°C or lower before shipping for that workflow. CDC’s 2024 syphilis recommendations describe testing within 5 days at 2–8°C or freezing at ≤−20°C for longer storage in the described workflow. Check the exact test instructions rather than applying either limit generally.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can I thaw a sample in a water bath or refreeze it afterward?

The sources cited here do not establish a universal thaw temperature, thaw duration, mixing procedure, or rule for refreezing. Follow the assay instructions and validated laboratory SOP. If the handling history is uncertain or outside the stated limits, ask the laboratory responsible for the test to apply its acceptance criteria.

Are frost-free freezers suitable for frozen serum?

CDC’s 2024 syphilis recommendations caution against storing serum in frost-free freezers because their temperature cycles can be detrimental to frozen-serum stability. This is a specific recommendation in that guidance; follow the equipment requirements for the assay and laboratory procedure.

The Bottom Line

Reliable freeze-thaw handling starts with the exact test, not a general freezer rule. Confirm whether the assay needs whole blood, serum, or plasma; follow its validated processing, container, temperature, duration, and thaw instructions; and avoid unnecessary freeze-thaw cycles. Treat published limits as examples tied to their named workflows, and ask the receiving laboratory when the applicable procedure is unclear.
