Check freshness before retrieved text reaches the model: compare each chunk with a trusted source timestamp, version, and validity state; then exclude or quarantine material that is expired, superseded, too old for its domain, or impossible to verify. Relevance alone is not enough—an outdated passage can closely match a question and still lead to a wrong answer.
Why freshness needs its own check
Retrieval-augmented generation (RAG) can find text that is semantically relevant without establishing that it is still true. A policy, price, procedure, or software instruction may match a query precisely even after the source has changed. Treating relevance as proof of currency lets superseded evidence enter the prompt as if it were current.

A 2026 arXiv preprint by Ahmed, Poech, and Röttger describes this risk as stale-document poisoning. Its benchmark covers 317 verified knowledge reversals across medicine, law, software, and platform policy. In the study’s experiment, stale evidence flipped 30% of Llama answers and 37% of Qwen answers when models were not instructed to trust the retrieved document; the reported rates were higher when they were told to follow it. These are experimental findings, not general production error rates. The authors also report that a fixed recency-aware hybrid reranker reduced errors by 4.6–10.0 percentage points when date metadata was accurate, underscoring that temporal controls depend on reliable metadata. What freshness data to carry with each chunk
Keep freshness metadata attached from ingestion through retrieval and generation. A practical record can include:

- Source identity: the authoritative system or document from which the chunk came.
- Last-observed or ingestion time: when the pipeline last confirmed the content and loaded it into the index. An ingestion time alone does not prove the source itself was recently updated.
- Source revision time or version: use the source’s own revision identifier or timestamp when available, rather than relying only on when your pipeline processed it.
- Lifecycle state: whether the item is active, expired, withdrawn, or superseded, where the source has meaningful validity rules.
- Validity window: start and end dates when content applies only during a defined period.
This is an engineering design pattern, not a universal metadata standard. AWS recommends tracking ingestion timestamps and flagging content that exceeds staleness thresholds before serving it. AWS Agentic AI Lens also emphasizes synchronization and live retrieval for information that must be current.
Set thresholds according to risk and change rate
There is no single refresh interval that makes every RAG corpus safe. A stable reference document and a fast-changing inventory feed have different freshness requirements. Choose a threshold for each source or content class by considering how quickly facts change and the consequences of serving an old fact. Prices, inventory, and system status are examples AWS identifies as needing live authoritative retrieval rather than reliance on cached content.

- Define the maximum acceptable age for each domain or source class.
- Use stricter limits when a stale answer could cause significant harm or operational loss.
- Define what to do when the timestamp, version, or source state is missing or cannot be trusted; do not silently treat unknown freshness as current.
- Revisit thresholds when the source’s update pattern or the impact of stale information changes.
Put a freshness gate before generation
Run the gate after retrieval and before assembling the model’s context. Evaluate every candidate chunk against its applicable threshold and authoritative source state. AWS’s operational guidance is explicit: “Track ingestion timestamps and flag content that exceeds staleness thresholds before it is served.”
- Retrieve candidates. Preserve each chunk’s source and freshness metadata in the retrieval result.
- Check the source state. Reject or quarantine chunks marked expired, withdrawn, or superseded by an authoritative revision.
- Check age. Compare the relevant timestamp with the policy for that source or domain.
- Handle unknowns conservatively. If freshness cannot be established, exclude the chunk or mark it as unverified rather than presenting it as current.
- Build context from eligible material. Keep any freshness qualification visible to the generation step and, when relevant, to the user.
Quarantine can be useful when a chunk needs review or source reconciliation; exclusion is simpler when there is no safe basis for serving it. If the gate removes all useful evidence, the system should say that it could not verify current information rather than silently falling back to stale context.

Keep the index aligned with its sources
A freshness gate cannot reliably detect a newer source version if the pipeline never learns that the source changed. Synchronize from authoritative sources, and verify both update and deletion behavior so obsolete chunks do not remain eligible after a document is replaced or removed. Re-indexing may also be necessary after changes to chunking or embedding methods, because the index then represents content differently. Treat source synchronization, deletion handling, and re-indexing as part of freshness control—not just maintenance tasks.

Use live retrieval for facts that must be current
For rapidly changing or transactional facts, query the authoritative system at answer time instead of assuming an indexed copy is fresh enough. Examples include current prices, inventory, and system status. If the live lookup fails, make the limitation explicit: the system could not verify current information, or it is answering from general knowledge rather than a live source. Do not imply that an unverified cached result is current.

Test the full path before relying on the gate
Test whether stale content can still reach the model, not just whether the index contains timestamps. A practical test is to change or supersede a source and trace a query that would have retrieved its old version.

- Update or supersede a source document and record its authoritative version or status.
- Run the normal synchronization and indexing pipeline.
- Inspect the indexed state to confirm the old chunks were updated, removed, or marked ineligible.
- Submit a query likely to retrieve the former version.
- Inspect the retrieval results and request trace before generation to confirm the old content did not enter the context.
- Repeat with a deletion and with missing or unreliable freshness metadata to check the failure path.

Choose controls by what they can prove
When assessing a freshness approach, compare detection latency, the trustworthiness of source timestamps and versions, update and deletion correctness, how unknown freshness is handled, retrieval relevance, operational cost, and serving latency. A highly restrictive age filter may reduce exposure to old material but also discard useful evidence; a recency reranker can prioritize newer items but cannot compensate for inaccurate dates or a missing authoritative update. No universal refresh interval or cross-vendor winner is established by the available guidance, so validate the policy against the behavior and risk of your own sources.
