Start by identifying what has failed: a black or disconnected display, a headset Windows or SteamVR cannot detect, lost tracking, a SteamVR crash, or a wireless stream that will not connect. Those symptoms point to different causes. The steps below are specific where noted: Valve Index and HTC Vive instructions apply to those wired/SteamVR setups, while the network checks apply to Quest 3 Mixed Reality Link on Windows 11.
First, identify the symptom and your setup
Before changing cables, drivers, or network settings, note what the headset actually does and how it connects. A headset that shows the desktop but is missing from SteamVR has a different problem from one that is detected but has no video.

- Display: Is the view black, does it say “no video input” or “display disconnected,” or does it show the desktop?
- Detection: Does the headset disappear from SteamVR or fail to appear at all?
- Tracking: Does the view or a controller turn grey, drift, or stop responding?
- Streaming: Does the headset connect to the PC but fail to establish a wireless stream?
- Setup details: Record the headset model, wired or wireless connection, GPU, Windows version, exact error text, and headset LED state.
Use the troubleshooting instructions for your headset and runtime. A Vive base-station fix, for example, is not a general fix for a camera-based inside-out tracking system.
For a wired headset, check power and every connection
A loose connection can look seated. Inspect the full path from the PC to the headset, including any power supply, breakaway connector, tether, or Link Box in the setup.

- Check that the headset and any required Link Box or power supply are powered.
- Reseat the headset tether and any breakaway connection. On Valve Index, Steam specifically notes that the tether may appear connected even when it is not fully seated.
- Check the USB and video connections at both ends. For a Vive with a Link Box, check the headset-to-Link-Box and Link-Box-to-PC connections as well.
- Confirm the PC’s video connection is in a supported port on the graphics card, not an unrelated motherboard port. For the cited Vive Link Box setup, Steam says the monitor and Link Box should connect to the same GPU.
- Restart the headset and SteamVR after reseating connections. If the problem remains, restart the PC before moving on to driver or runtime troubleshooting.
For Index, Steam also recommends checking the graphics driver, including its high-definition audio component. If an error persists, use the official troubleshooting page for the exact headset rather than applying another model’s cable or port instructions.

Check video routing and ports on Vive
For the cited Vive setup, verify that the monitor and Link Box are connected to the same GPU and that both video ports work. If the PC has another port of the same type, test it. On a laptop, do not assume every video port is routed through the same GPU; the relevant check is which GPU actually drives the port used by the Link Box.

Check Direct Mode for SteamVR Error 208
Steam’s Error 208 guidance concerns Direct Mode. The headset should be handled by SteamVR as an HMD, not as a regular monitor. If the desktop appears inside the headset after SteamVR closes, Steam says Direct Mode is not enabled. If Direct Mode cannot be enabled, update the GPU driver, then restart SteamVR and the PC. These steps describe SteamVR’s cited Vive error path; other headsets may use different diagnostics.

For Valve Index, use the LED state as a clue
Steam says a steady red Index LED means the display failed to initialize. Its guidance is to check connections and drivers, power-cycle at the breakaway connector, and restart SteamVR. Persistent blinking red may indicate a firmware problem; contact Steam Support if it continues.

If tracking is lost on a Vive Lighthouse system
These checks apply to Vive systems that use Lighthouse base stations. They do not apply to headsets that track with cameras built into the headset.
- Confirm the base stations are powered and synced, and that the headset and tracked devices remain visible to a base station.
- Follow Steam’s published placement guidance: mount stations above head height, at least 6.5 feet high, angle them down 30–45 degrees, and keep them no more than 5 metres (16 feet) apart. These are setup measurements, not a guarantee that every room will track reliably.
- Check that station cables and other connections are secure.
- If tracking drops intermittently or controllers float, temporarily cover reflective surfaces in the room and see whether tracking changes.
If these checks do not match your tracking system, use the manufacturer’s instructions for that headset’s tracking method instead of changing Lighthouse placement.

If SteamVR crashes or another VR component may be interfering
After checking the physical connection, try SteamVR Safe Mode. Steam says this mode can prevent some third-party drivers and components from loading. If the problem disappears in Safe Mode, an installed component may be interfering; Steam identifies VR utility applications as examples. If Safe Mode does not resolve the failure, use Steam Support or the headset maker’s support channel with the error and setup details you recorded.

- Keep the Windows 11 PC awake and unlocked while reconnecting.
- If the stream still will not establish, check whether firewall or proxy settings are blocking the network communication Mixed Reality Link needs. Microsoft identifies these settings as common reasons for a failed streaming connection.
When to replace a cable or seek support
Do not buy a cable as a first response to a black screen or tracking fault. Consider a replacement only when you have a matching wired headset setup and inspection or port testing points to cable damage or failure. Verify the exact headset, connector, and PC compatibility against the manufacturer’s documentation before buying. A USB-C Link cable is relevant only to a compatible wired Quest setup; a DisplayPort cable is relevant only to a headset and connection that support it.
If connection checks and the model-specific software steps do not resolve the problem, contact the headset maker or runtime support with the exact error, LED state, PC and GPU details, and the checks already performed. Persistent Index blinking-red LEDs in particular warrant Steam Support rather than trial-and-error part purchases.
