Fix phpMyAdmin access by tracing the request through four layers in order: the web server, PHP or PHP-FPM, phpMyAdmin’s configuration, and MySQL or MariaDB. Start with the exact HTTP status and timestamp, then check the active virtual host, the directory containing index.php, the PHP handler or FPM socket, and the database connection settings. This order separates a bad URL from a PHP crash, a permissions problem, and a database authentication failure.

Identify the failure before changing anything
Open the phpMyAdmin URL in a private browser window and record the complete message, HTTP status, URL, and time. Also note whether the page is blank, shows a PHP error, displays a login form, or loads and then redirects. A browser message alone is not enough: the same visible symptom can be produced by different layers.

Keep a copy of the configuration before editing it. Make one change at a time, test, and record the result so that a working setting can be restored if a later change introduces a new error.

Use this diagnostic order
- Reproduce and timestamp the failure. Request the page again and write down the exact status, URL, and browser text. The timestamp lets you correlate entries across several logs.
- Confirm the active web-server site. Determine which Apache virtual host or Nginx server block answers the public hostname. A different enabled site can return a valid-looking 404 even when phpMyAdmin is installed correctly elsewhere.
- Confirm the phpMyAdmin path. The URL’s alias or location must point to the top-level phpMyAdmin directory, the one that contains
index.php. Check spelling, case, trailing path prefixes, and whether the site configuration was actually enabled. - Confirm PHP execution. Apache must have its configured PHP handler, while Nginx must pass PHP requests to the correct PHP-FPM service and socket. A static directory listing or downloaded PHP file means PHP is not being handled safely.
- Read all relevant logs at the same time. Check the web-server error log, PHP-FPM log, and system journal. Authentication failures may be written to syslog rather than the Apache access or error log.
- Validate phpMyAdmin and database settings. Inspect
config.inc.php, authentication mode, database host, username, password, and socket. Use the MySQL client’sstatuscommand or the database configuration to identify the real socket. - Account for a proxy or path prefix. If another proxy terminates TLS or publishes phpMyAdmin below a prefix, make the externally reachable URL and forwarded host and scheme consistent with phpMyAdmin’s settings.
Correct the Apache or Nginx mapping
Find the directory that should be served
Locate the phpMyAdmin installation and verify that its top-level directory contains index.php and the expected application subdirectories. The config.inc.php file belongs in this same top-level directory. If the alias points one directory too high or too low, the server can return a 404 or expose the wrong content.
Check the active configuration rather than an unused file. For Apache, inspect the enabled virtual host and its Alias, Directory, and rewrite rules. For Nginx, inspect the enabled server block and its root or alias, the location that handles the phpMyAdmin path, and the regular-expression location that handles PHP files. Confirm that the hostname and port in the request select this block.
Test configuration before reloading
Run the appropriate syntax test before applying a change. Typical commands are sudo apachectl configtest for Apache and sudo nginx -t for Nginx. Only after a successful test should you reload the relevant service. A reload applies the corrected configuration without unnecessarily stopping the server; if the syntax test fails, use the reported file and line number instead of reloading a broken configuration.
Resolve a 404 without masking it
A 404 normally means the request reached a web server but its alias or location did not resolve to phpMyAdmin. Verify the public URL, enabled site, path prefix, and the presence of index.php. Then read the server error log to distinguish a wrong filesystem path from an application redirect that happens to end at a missing URL. Do not create a second copy of the application merely to make one URL work; correct the selected virtual host.
Resolve a 403 safely
Inspect explicit deny rules, directory access controls, and filesystem ownership. The web-server account needs to traverse parent directories and read the application files, but the installation does not need to be world-writable. Check for an access rule inherited from a parent directory or from a security include. Correct ownership and the narrowest required permissions, then retest. Making the entire phpMyAdmin tree writable by every user can turn a routing problem into a code-injection risk.

Make sure PHP and PHP-FPM are actually working
Apache deployments
Verify that the active Apache site uses the intended PHP handler for .php files. A site can serve HTML successfully while PHP requests fail because the module or proxy handler is missing, disabled, or associated with a different PHP installation. Check the loaded Apache configuration and the PHP handler selected by the virtual host. The web-server error log will usually show a handler or interpreter error when PHP cannot start.
Nginx deployments
Nginx does not execute PHP itself. Its PHP location must pass requests to the PHP-FPM service, set the script filename correctly, and use the socket or TCP endpoint on which FPM is listening. A representative location has the following shape, but the include names and socket path must match the installed distribution:

}Use the socket reported by the PHP-FPM pool configuration, not a path copied from another server. If FPM listens on TCP instead, use that host and port. A stale socket, a stopped service, or permissions that prevent Nginx from opening the socket commonly produces a 502.
Check PHP version and extensions
Confirm the PHP version used by the web server, not only the version returned by an interactive shell. Compare it with the requirements of the installed phpMyAdmin release and verify that its required extensions are enabled. A PHP syntax or compatibility error, missing extension, or changed default can produce a 500 while the service itself appears to be running.
Check the PHP-FPM service status and journal, using the service name supplied by your operating system. For example, inspect the status of the installed FPM unit and review entries around the failure with journalctl. On Nginx, read both the Nginx error log and the FPM log; on Apache, read the Apache error log and the PHP handler’s log. A 502 with no corresponding FPM activity usually indicates that Nginx is targeting the wrong endpoint.

Handle the HTTP/3 host-header case
If Nginx HTTP/3 is enabled, the phpMyAdmin FAQ documents a host-header issue that can break two-factor authentication and related URL behavior. Add fastcgi_param HTTP_HOST $host; in the PHP-FPM server block where the other FastCGI parameters are defined, test the Nginx configuration, and reload it. Place the parameter in the PHP-FPM configuration block, not in an unrelated static-file location.
Read the right logs together
Use the same timestamp in each source. The access log confirms that a request arrived; the error log explains routing and permission failures; PHP-FPM records interpreter and pool problems; and the system journal records service crashes, restarts, and socket failures. Authentication failures can also appear in syslog, which is separate from Apache’s access and error logs.
On a system using systemd, commands such as journalctl -u <php-fpm-service> and journalctl --since can narrow the time window. Distribution log paths differ, so use the paths configured by the active service rather than assuming a particular filename. Search for the request time, the PHP file path, the FPM pool name, and messages containing permission, socket, include-path, or authentication errors.
Do not enable verbose PHP error display on a public production site merely to reveal a 500. Read the protected logs instead. Error output can disclose database credentials, filesystem paths, and other information useful to attackers.

Check config.inc.php and PHP path restrictions
Confirm location and syntax
phpMyAdmin loads config.inc.php from its top-level directory after loading its defaults. If the file is in the wrong directory, has a syntax error, contains accidental output before the opening PHP tag, or is unreadable by the web-server account, the application may fail before displaying its normal welcome or login screen.

Check the file for balanced quotes and semicolons, remove unintended characters before <?php, and verify ownership. A syntax check with the PHP binary can catch parse errors, but make sure the command uses the same PHP version as the web server. Keep secrets out of shell history and do not paste passwords into a public bug report.
Review include_path and open_basedir
The phpMyAdmin FAQ notes that include_path must contain . for normal operation. If open_basedir is enabled, it must permit . and ./lang; an overly restrictive value can cause missing-file or language-loading errors that appear as a 500. Adjust the restriction to include only the required installation and temporary paths, then retest rather than disabling the restriction broadly.

Review the configured authentication mode and the values for database host, username, password, and socket. A cookie-based login still requires a valid database account; a configuration-based login uses the credentials stored in the configuration and must be protected accordingly. If an auxiliary control account is configured, invalid controluser or controlpass values can cause access-denied messages even when the user’s own account is correct.
Check for a second authentication layer. HTTP Basic Authentication, a directory-level .htaccess rule, or a proxy login can conflict with phpMyAdmin’s own login flow. Remove or reconcile overlapping rules and test in a private window so that stale browser credentials do not obscure the result.
Fix database connection and socket errors
When MySQL or MariaDB is on the same VPS
A Unix socket is appropriate when the database server runs on the same machine. Find the active socket in the MySQL or MariaDB configuration, or connect with the command-line client and run status. Set phpMyAdmin’s socket value to that exact path. Common-looking paths are not interchangeable: a package upgrade or a separate database instance can place the socket somewhere else.
Also verify that the database service is running and that the web-server process can reach the socket. A correct path with insufficient filesystem permission still fails. Check the database service journal and the PHP-FPM log at the same timestamp to distinguish a stopped database from a PHP connection error.

When the database is remote
Use the database host and port instead of a local Unix socket. Confirm that the database account is permitted to connect from the VPS address, that the server listens on the intended interface, and that network filtering allows the port. Do not expose the database port broadly just to make phpMyAdmin work; restrict it to the required source addresses and use encrypted transport when the deployment requires it.
Interpret access-denied messages
When the login page appears but authentication fails, verify the exact MySQL or MariaDB account, password, host component, and authentication method. The same username can have different privileges for different hosts. Reset a password only after confirming that the account and host are the intended ones. If HTTP authentication is also enabled, test each layer separately so that a web-server denial is not mistaken for a database denial.
Repair reverse-proxy and URL-prefix problems
Behind a reverse proxy, phpMyAdmin must know the URL that users actually reach. Set $cfg['PmaAbsoluteUri'] to the externally reachable scheme, hostname, port, and path prefix when automatic detection is wrong. Preserve the original host and scheme headers as traffic passes from the proxy to Apache, Nginx, or PHP-FPM. A mismatch can produce repeated logins, links that drop the prefix, redirects to an internal hostname, or 404 responses only after an action.
Test both the initial page and a state-changing navigation such as opening a database or returning from login. Inspect the browser’s address bar and the proxy’s upstream request to ensure that HTTPS remains HTTPS from phpMyAdmin’s perspective. If only HTTP/3 requests fail, apply the explicit HTTP_HOST FastCGI parameter described in the PHP-FPM section and retest with another protocol to isolate the cause.

Recovery is not complete until the management interface is protected. The phpMyAdmin project’s security guidance is explicit: Serve phpMyAdmin on HTTPS only.
Use a valid certificate, redirect or disable plaintext access, and ensure that cookies are protected by the HTTPS deployment.
- Follow phpMyAdmin security announcements and upgrade to supported releases after checking compatibility with the installed PHP version.
- Disable PHP error display in production; keep diagnostic detail in protected logs.
- Remove unused
setupand test directories. - Deny direct access to auxiliary files, temporary files, and configuration backups.
- Choose an authentication mode appropriate for the deployment and protect any stored credentials.
- Consider an authentication proxy, IP restrictions, two-factor authentication, or fail2ban-compatible logging.
- Review syslog as well as web-server logs because failed login attempts may be recorded there.
Do not leave a temporary broad allow rule, world-writable directory, debug setting, or copied configuration file in place after testing. Recheck the public URL from an untrusted network and confirm that only the intended hostnames and paths are reachable.
A final verification checklist
- The request reaches the intended Apache virtual host or Nginx server block.
- The phpMyAdmin alias or location points to the directory containing
index.php. - The web server passes PHP files to the intended handler or PHP-FPM endpoint.
- The PHP version and required extensions match the installed phpMyAdmin release.
- PHP-FPM is running, and its socket path and permissions match the web-server configuration.
config.inc.phpis in the top-level directory, parses cleanly, and is readable by the service account without being writable by everyone.include_pathcontains., and anyopen_basedirpolicy permits.and./lang.- The database host, port or socket, username, password, and authentication method are correct.
- No HTTP Basic Authentication or directory rule conflicts with phpMyAdmin’s login.
- A reverse proxy preserves host and scheme information, and
$cfg['PmaAbsoluteUri']matches the public URL when a prefix or proxy is used. - HTTPS is enforced, unused setup and test content is removed, and auxiliary files are protected.
If one item still fails, return to the timestamped logs and isolate that layer rather than changing settings in another one. A 404 requires routing evidence, a 403 requires access-control evidence, a 500 requires PHP or configuration evidence, a 502 requires FPM evidence, and a login denial requires database or overlapping-authentication evidence.
