Fix architecture drift by checking each affected architecture decision record (ADR) against today’s requirements and the system as it is built. Reaffirm decisions that still fit; document changed decisions in new ADRs that supersede—not erase—the old ones; then track any code or artifact changes needed to bring implementation into line. Finally, assign ownership and review records when relevant context changes.
What an outdated ADR does—and does not—tell you
A stale ADR is a reason to investigate, not proof that the original decision was wrong. Requirements, constraints, available options, and the consequences of a choice can change while the choice itself remains sound. Check the decision’s original context and the current system before deciding what to do.

Architecture drift can also run in the opposite direction: the record may still describe an accepted decision, but the code or configuration may no longer follow it. Treat the decision history and implementation as related but separate things to verify.
How to repair outdated decision records
1. Find decisions that may be stale or missing
Collect the ADRs and identify those connected to changed requirements, platforms or dependencies, interfaces, quality attributes, team ownership, known architecture violations, or implementation exceptions. Also look for consequential choices that teams rely on but cannot find in an accessible record. Google’s ADR guidance describes ADRs as useful when teams choose among engineering options or need to document a solution accessibly.
2. Compare each record with current reality
For each candidate, compare its stated context and requirements with current ones. Inspect relevant code, configuration, diagrams, interfaces, and supporting documents; ask people responsible for the affected system whether assumptions or consequences have changed. Classify the outcome plainly:
- Still valid: the decision continues to fit the current context.
- Changed: the team has made or needs to make a different choice.
- Unclear: there is not enough evidence to reconstruct the history or confirm the decision.
Do not turn an uncertain reconstruction into a claim about what the team decided. The UK government’s Architectural Decision Record Framework calls for records to capture context, decision, consequences, stakeholders, and links to supporting documents, and to be reviewed when context or consequences change. Its mandate is UK public-sector-specific; those record-keeping practices can still inform other teams.

3. Reaffirm valid decisions or create a superseding ADR
If the decision remains valid, retain its record and note the review outcome in the team’s chosen change-history mechanism. If the decision changes, create a new ADR stating the current context, options considered, accepted choice, rationale, and consequences. Link it to the earlier ADR, mark the earlier one as superseded, and keep it in the decision log. This preserves why the change happened and how the architecture evolved; Google puts the principle simply: “If you make adjustments, include the previous decision and why a change is made.”

AWS’s ADR best practices and ADR process describe treating a different choice as a new proposal that supersedes the earlier record when accepted.

4. Reconcile the implementation separately
Compare the accepted decision with the system as built, including the relevant code, configuration, diagrams, interfaces, and other artifacts. If legacy code does not comply, decide how to handle that work: AWS recommends changing code or artifacts gradually as related changes are made, or recording explicit technical-debt work for a refactor. Track remediation separately from the historical decision record so readers can distinguish what the team decided from what remains to be implemented.
5. Make the repaired records findable and maintainable
Store ADRs where affected developers and stakeholders can access them, link the collection from the project’s main documentation, and connect records to relevant code and supporting materials. Google recognizes both version-controlled Markdown near the code and shared documents or an internal wiki; AWS likewise describes a Git repository and a wiki as common options. Choose the location that makes ownership, history, review, approval, and supersession clear. No single platform is established as best for every team.

Assign responsibility for maintaining the collection and review records regularly. There is no universal interval established by the cited guidance. As a practical policy, choose a cadence that fits the system’s rate of change, and trigger a review when requirements, constraints, available solutions, context, or consequences change.

Yes, when the historical information is available. Microsoft’s guidance on maintaining an ADR notes that teams can reconstruct decisions retroactively for a brownfield workload. Record what can be supported by evidence, identify what remains uncertain, and avoid presenting an inferred rationale as documented history.

Optional tooling for Markdown ADRs
ADR Tools is an open-source command-line tool for creating and maintaining Markdown ADRs, including records that supersede earlier ones. It can help manage the record sequence; it does not determine whether a decision is substantively out of date. Teams still need to validate context, implementation, and consequences.
