An FDA-approved generic is expected to provide the same clinical benefit and risks as its brand-name reference product, but a similar name or a lower price alone does not establish that a product is interchangeable.
How to check a generic option safely
- Gather the exact prescription details. Use the medicine name, strength, dosage form (such as a tablet or liquid), and route (such as oral or topical) shown on the prescription or label. Ask the pharmacist or prescriber to verify the proposed product against those details.
- Ask whether an FDA-approved generic exists for that exact product. Not every prescription drug has an available generic. Ask whether the option is appropriate for your prescription, rather than assuming that a product with a similar name or purpose is a match. See the FDA’s Generic Drugs: Questions & Answers.
- Use the Orange Book as a reference, not a personal treatment decision. The FDA’s Orange Book lets you look up approved drug products and therapeutic-equivalence evaluations. Match the active ingredient, strength, dosage form, and route, then review the listing with your pharmacist or prescriber.
- Share relevant sensitivities or problems after a switch. Tell the pharmacist or prescriber about allergies, ingredient sensitivities, or a concern that began after changing products. Inactive ingredients can differ between products.
- Wait for professional confirmation before changing how you take the medicine. A listing does not establish that you can change the dose, split or crush a tablet, or use a different dosage form. Do not stop or switch a prescribed medicine on your own.
What makes a product a generic equivalent?
FDA says an approved generic has the same active ingredient as its reference drug, as well as the same strength, dosage form, and route. It must meet bioequivalence requirements and applicable quality and manufacturing standards. FDA describes approved generics as providing the same clinical benefit and risks as their brand-name counterparts. Read FDA’s Generic Drug Facts for its consumer explanation.

Some inactive ingredients may differ. FDA says these differences usually do not matter, but an ingredient can affect tolerability or absorption for a minority of patients. That is why a known allergy or a new concern after a switch is worth discussing with a pharmacist or prescriber.

Why a related medicine or formulation may not be interchangeable
A medicine can be related to the prescribed drug without being its therapeutic equivalent. In the Orange Book Preface, FDA describes therapeutic equivalents as pharmaceutical equivalents for which bioequivalence has been demonstrated; under labeled conditions, they can be expected to have the same clinical effect and safety profile.

A pharmaceutical alternative may contain the same therapeutic moiety but differ in amount, dosage form, salt, or ester. A liquid, extended-release tablet, different strength, or related medicine is therefore not automatically a substitutable generic. Have the pharmacist or prescriber assess the exact product rather than relying on a shared ingredient name, appearance, or intended use.
How substitution differs for biologic medicines
Biologic medicines do not have conventional generics in the same sense as many small-molecule drugs. FDA says a biosimilar is highly similar to its reference biologic; only a biosimilar designated interchangeable may be substituted at the pharmacy under applicable state law. Not every biosimilar is interchangeable, and state pharmacy rules affect substitution. Ask your pharmacist or prescriber about the specific biologic and local requirements. See FDA’s overview of biosimilar and interchangeable biologics.

Who can confirm what your pharmacy may dispense?
Whether a pharmacy can substitute a particular product depends on the medicine and applicable jurisdictional rules. FDA specifically notes that pharmacy-level substitution of interchangeable biosimilars is subject to state pharmacy laws. Because the prescription and location determine the practical answer, ask the pharmacist handling the prescription to confirm what may be dispensed and whether the proposed product matches it.
