How to Evaluate AI Agent Actions Before They Reach a Human Approval Gate

A practical architecture for evaluating AI agent actions at the tool boundary, with proposal schemas, hard-deny rules, reviewer requirements, fail-closed workflows, audit logging, and test cases.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

The safest approval gate evaluates the exact tool call immediately before it can change anything—not a vague plan and not only the model’s final message. Normalize the proposed action, enforce hard scope rules, assess harm and uncertainty, show the reviewer the target and evidence, then allow, deny, or pause. If review is unavailable, fail closed, and record the proposal, decision, and execution result.

What an approval gate must decide

An AI agent can produce several model messages before it invokes a tool. Those messages may describe an intention, but the tool call is where a side effect becomes possible: a record is deleted, a payment is submitted, a credential is changed, a production command runs, or data leaves a controlled system. The gate therefore needs to answer a concrete question at that boundary: Should this specific operation, with these arguments, identity, target, and evidence, run now?

This is a system property, not a prompt-only safeguard. The model, orchestration harness, custom tools, identity system, network, data stores, and runtime permissions all influence whether an action is safe. A well-worded instruction cannot compensate for a tool that bypasses review or an environment that grants broader access than the policy allows.

  • Deterministic policy: blocks actions that are outside scope or explicitly prohibited.
  • Risk evaluation: considers impact, reversibility, uncertainty, authorization, and untrusted input.
  • Human judgment: handles ambiguous intent and high-consequence decisions.
  • Operational controls: stop execution when review is unavailable and preserve an audit trail.

Place enforcement at the side-effect boundary

Put the check beside each custom tool or execution path that can create a side effect. Agent-level input and output filters can miss calls made after a handoff, through a nested agent, or by a tool that the outer conversation never exposes. A gate attached to the actual execution interface sees every invocation that reaches that interface.

For a read-only operation, the same boundary can apply a lighter policy. For a write, deletion, external message, privileged command, or data transfer, the boundary should be mandatory. Do not rely on the model to remember to ask for approval; make the tool unable to execute until the gate returns an explicit result.

Coverage checks

  • Enumerate every tool that can write, delete, transmit, publish, purchase, authenticate, or change permissions.
  • Trace calls made by handoffs and nested agents, not only calls initiated by the top-level agent.
  • Check alternate execution paths such as batch jobs, retries, scheduled runs, direct SDK calls, and administrative consoles.
  • Keep read-only inspection separate from mutation where possible, so policy can distinguish observation from change.

OpenAI’s guidance treats side-effect checks as distinct from broad input and output checks, while Anthropic describes safety as an interaction among the model, harness, tools, and environment. Together, those principles imply that coverage must be tested at the operation that can actually change state.

Normalize every proposal into a reviewable record

Before policy or a person evaluates an action, convert it into a stable representation. The record should be understandable without reconstructing the agent’s entire conversation.

The target must identify the actual resource, not merely a category such as “the customer database.” Arguments should be the values that will be sent to the tool after templating and variable substitution. Identity should include the effective principal and relevant roles, not just the agent name. For authorized cybersecurity work, OpenAI’s guidance specifically calls out target, action, arguments, calling identity, and the engagement window; other domains need equivalent fields.

Prevent proposal drift

Bind the approval decision to a digest of the normalized proposal. If the target, arguments, identity, or environment changes, require a new evaluation. A plan approved for one operation must not silently authorize a materially different downstream call. Include the policy version and tool version so a later reviewer can reproduce what the gate saw.

Apply hard policy before asking for judgment

Run deterministic rules first. Deny an action that is plainly outside the authorized scope or prohibited by policy; do not send it to a reviewer as if approval could make it acceptable.

Typical hard-deny conditions

  • The target is outside the declared engagement, tenant, account, environment, or ticket scope.
  • The caller lacks the required role, delegated authority, or time-bounded authorization.
  • The operation attempts credential theft, persistence, unauthorized data exfiltration, destructive change, or a bypass of the control itself.
  • The request violates a data-residency, privacy, retention, or separation-of-duties rule.
  • Required fields are missing, malformed, or inconsistent with the authenticated context.
  • The tool is not on the approved capability list or its version is untrusted.

OpenAI’s examples for authorized cybersecurity workflows include out-of-scope hosts, credential theft, persistence, data exfiltration, destructive changes, production access, and bypass attempts. Those examples are specific to that context; they are not a universal prohibited-action list. A financial, healthcare, or infrastructure system needs its own domain rules.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Keep policy outcomes explicit

Outcome Meaning Required behavior
Allow The action satisfies scope and risk rules without additional judgment. Issue a short-lived authorization tied to the proposal digest, then execute.
Deny A rule or authorization check makes execution impermissible. Do not call the tool; return a reason safe to expose to the agent and log the full reason.
Review The action is ambiguous, high impact, or insufficiently supported. Pause the run and present the exact proposal and context to an authorized human.

Do not collapse “deny” and “review.” A reviewer should not be asked to override a non-negotiable prohibition, and an ambiguous but potentially legitimate operation should not be rejected merely because a rule cannot determine intent.

Assess harm, reversibility, authorization, and uncertainty

After hard policy, estimate whether the consequences warrant a person’s decision. There is no universal numerical threshold in the cited guidance; classifications must fit the domain and its tolerance for error.

Questions for a risk assessment

  • Potential harm: Could this expose personal data, interrupt a service, create a financial obligation, damage a reputation, or weaken security?
  • Reversibility: Can the exact state be restored, and how quickly? A tested rollback is different from an unverified backup.
  • Blast radius: Is the operation limited to one record or capable of affecting many accounts, hosts, or tenants?
  • Authorization scope: Does the authority cover this target, action, time window, and environment?
  • Uncertainty: Are the target, arguments, intent, or supporting facts ambiguous?
  • Untrusted influence: Did a document, web page, email, user-generated record, or tool response supply instructions that could be prompt injection?
  • Privilege: Would the call gain access, alter permissions, create credentials, or bypass an existing control?

Escalate when plausible harm is high, reversal is difficult, authorization is unclear, or untrusted content materially shaped the proposal. A low-impact, repeatable operation may be allowed automatically after policy checks; a production deletion, external disclosure, or permission change generally deserves explicit approval even when the agent is confident.

Verify evidence and provenance

If the rationale depends on factual claims, attach the material the claim came from and make the relationship inspectable. A reviewer should be able to see not only what the agent says, but why it says it.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Three evidence tests

  • Faithfulness: Does the cited material actually support the proposal’s rationale?
  • Completeness: Has the agent omitted contradictory or limiting information?
  • Sufficiency: Is the evidence adequate for the consequence of the action, rather than merely plausible?

For example, a support ticket may authorize a configuration change but say nothing about a second production system. A web page may contain an instruction aimed at the agent rather than evidence about the task. Treat such content as untrusted input, preserve its provenance, and do not let it expand authority.

Store source identifiers, retrieval times, relevant excerpts or hashes, and the transformation that produced the rationale. The reviewer can then distinguish a well-supported proposal from one that merely repeats an assertion.

Design the human review surface around the actual call

Approval is useful only when the person can make an informed decision quickly. Show the operation that will run, not an abstract plan.

Minimum reviewer view

  • Tool name and operation.
  • Exact target or resource identifier.
  • Fully resolved arguments, with secrets masked but their presence and destination indicated.
  • Effective caller identity, roles, delegated authority, and environment.
  • Requested scope, ticket or job reference, and engagement window where relevant.
  • Expected side effect, estimated blast radius, and reversibility or rollback information.
  • Policy checks that passed, checks that triggered escalation, and the policy version.
  • Evidence supporting the rationale, including provenance and relevant contradictory material.
  • Warnings about prompt injection, malformed data, unusual destinations, or changed state.

Use separate controls for approve once, deny, and, where your governance model permits it, approve a narrowly bounded set. A bounded approval should specify the exact tool, target pattern, argument constraints, identity, and expiration; it must not become an unlimited delegation.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Make changes visible

If a proposal was edited, retried, or regenerated while waiting, show a diff and invalidate the old decision when a material field changes. Never let a reviewer approve one set of arguments while the tool executes another.

Choose action review, plan review, or both

Per-action review gives a fresh check at each side effect. Plan review can reduce repeated interruptions when a sequence is predictable. They are not interchangeable.

Pattern Strength Failure mode Useful control
Per-action review Inspects the exact call at the moment of execution. Many prompts can create friction and approval fatigue. Auto-allow low-risk, repeatable calls and group only tightly identical operations.
Whole-plan review Lets a person understand a multi-step objective once. A later step may change target, arguments, or risk. Re-check every materially changed or high-impact action.
Hybrid Combines context of a plan with boundary protection. Teams may assume plan approval covers everything. Bind plan approval to constraints and require fresh approval when they are exceeded.

Repeated prompts can be tuned out, so reduce low-value interruptions rather than weakening the boundary. A plan approval should be treated as a constrained authorization, not a blanket permission for unforeseen calls.

Pause, resume, and fail closed

  1. Freeze the proposal: Persist the normalized record and its digest before asking for approval.
  2. Pause the run: Keep the agent from retrying, switching tools, or routing around the gate while waiting.
  3. Notify the right reviewer: Route by resource owner, risk domain, separation of duties, and on-call coverage.
  4. Record the decision: Capture approver identity, timestamp, decision, conditions, and optional reason.
  5. Revalidate on resume: Check that authorization, target state, policy version, and proposal digest still match.
  6. Execute once: Use an idempotency key where the tool supports it and record the result.

If the reviewer is unavailable, the approval service times out, the identity provider cannot confirm authority, or the audit store is unavailable, fail closed for consequential operations. Do not interpret a missing response as approval. A carefully designed low-risk read operation may have a separate availability policy, but that exception must be explicit and documented.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

OpenAI’s Agents SDK describes an interruption flow in which an approval is raised, the run is approved or rejected, and the same paused run is resumed; nested-agent approvals can also surface to the controlling workflow. Whatever framework you use, preserve the original proposal rather than asking the model to reconstruct it from memory.

Log a complete, tamper-evident record

An approval flag alone is not an audit trail. Record enough information to explain what was proposed, what evidence was considered, why policy produced its outcome, and what actually happened.

Record component Examples
Proposal Tool, operation, target, resolved arguments, identity, run and parent-run identifiers.
Context Environment, authority, ticket, engagement window, untrusted inputs, model and tool versions.
Evaluation Policy version, rules evaluated, evidence identifiers, faithfulness/completeness/sufficiency findings, risk factors.
Decision Allow, deny, or review; approver identity; timestamp; conditions; reason.
Execution Start and end time, idempotency key, tool response, changed-resource identifiers, error or rollback result.

Protect logs from alteration, restrict access to sensitive arguments, and retain enough detail for incident response and policy improvement. NIST describes evaluation results accumulating into a machine-readable audit trail so users can assess agent actions and outputs; the same principle applies to production gate records.

Compare implementation approaches before choosing one

Approach Coverage Decision surface Enforcement Traceability and testing
Prompt-only instruction Unreliable; may miss tool calls, handoffs, and retries. Usually a plan or free-form explanation rather than resolved arguments. Cannot guarantee denial or fail-closed behavior. Conversation logs rarely capture policy rationale or execution proof.
Model-based reviewer Depends on where it is inserted and whether every call passes through it. Can summarize context, but needs the structured proposal and evidence. Should not be the sole control for explicit scope prohibitions. Requires calibrated test cases and human sampling; confidence is not proof.
Deterministic policy gate Strong when attached to every execution path. Excellent for identity, target, scope, and argument constraints. Can deny and fail closed; cannot reliably infer ambiguous intent. Rules and outcomes are reproducible and easy to regression-test.
Human-in-the-loop gate Strong if all consequential calls pause at the boundary. Can inspect exact call, evidence, and uncertainty. Can allow or deny; must define timeout and unavailable-review behavior. Decision quality depends on the interface, reviewer training, and records.
Hybrid gate Combines deterministic checks, risk assessment, and targeted review. Supports automatic low-risk handling and rich escalation context. Best control range when deny, review, and fail-closed paths are explicit. Supports reproducible policy tests plus review-quality evaluation.

Test the gate as an operational control

Build a contextual test corpus rather than testing isolated prompts. Include proposals that are allowed, prohibited, ambiguous, malformed, stale, adversarial, and changed after approval.

Essential test cases

  • An in-scope read and an in-scope write with complete arguments.
  • A target just outside the permitted scope.
  • A production request made with a development identity.
  • A destructive operation with and without a tested rollback.
  • A proposal whose rationale cites evidence that is incomplete or contradictory.
  • Prompt-injection text embedded in a document, ticket, web response, or tool output.
  • A nested agent or retry that attempts to bypass the original gate.
  • A proposal changed while waiting for approval.
  • Reviewer timeout, identity-provider failure, audit-store failure, and duplicate delivery.
  • Malformed arguments, unexpected destinations, and oversized blast radius.

Measure more than whether the model sounds cautious. Check policy compliance, coverage of every relevant call, faithfulness to evidence, completeness of the reviewer context, sufficiency of evidence, false approvals, unnecessary escalations, timeout behavior, and execution/decision consistency. NIST recommends contextual measurement, independent evaluation or red-teaming, and regular improvement of risk measurement. Re-run the corpus whenever tools, policies, models, environments, or threat assumptions change.

Common design failures and fixes

Approving a summary instead of the call

Failure: The reviewer sees “clean up old records” and approves without the target set or deletion arguments.
Fix: Display the fully resolved operation and bind the decision to its digest.

Relying on an outer agent check

Failure: A nested agent or direct tool path executes without passing through the check.
Fix: Enforce at the tool or execution boundary and test every handoff and retry path.

Using a human to override explicit policy

Failure: Review becomes a way to approve an out-of-scope or prohibited operation.
Fix: Deny hard-policy violations before escalation; reserve human review for ambiguity and impact.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Treating approval as proof of correctness

Failure: Teams retain only an “approved” flag and cannot reconstruct the evidence or actual result.
Fix: Preserve proposal, evidence, policy outcome, approver, and execution records.

Failing open on timeout

Failure: A service outage turns missing review into automatic execution.
Fix: Fail closed for consequential operations and alert operators to the blocked run.

Allowing scope creep after approval

Failure: The agent changes the target or arguments during a retry.
Fix: Revalidate the digest and authorization immediately before execution; require a new decision for material changes.

A practical rollout sequence

  1. Inventory side effects: List tools and alternate paths that can mutate state, transmit data, spend money, or change privilege.
  2. Define the proposal schema: Require target, operation, arguments, identity, authority, environment, rationale, and evidence.
  3. Attach the boundary gate: Make execution impossible without an allow result or a recorded approval.
  4. Implement hard denies: Encode scope, identity, prohibited targets, and malformed-input rules first.
  5. Design the review screen: Show exact values, impact, reversibility, evidence, warnings, and policy version.
  6. Add pause/resume safety: Freeze the proposal, handle nested approvals, revalidate on resume, and fail closed on outages.
  7. Instrument the audit trail: Capture decision and execution records with sensitive-data controls.
  8. Run adversarial and contextual tests: Include injection, drift, retries, stale authority, and unavailable reviewers.
  9. Tune friction deliberately: Auto-allow only well-defined low-risk cases; monitor approval fatigue and false escalations.
  10. Review continuously: Update policy and test cases as the agent, tools, environment, and threat model evolve.

Bottom line for governance teams

Evaluate the concrete action where it can cause harm. A dependable gate combines deterministic scope enforcement, consequence and uncertainty assessment, evidence provenance, a reviewer view of the exact call, fail-closed interruption handling, and an auditable record through execution. Plan approval can reduce friction, but it must never silently authorize a changed or newly risky operation. Human approval improves control only when the person sees enough accurate context to make a bounded decision.
