How do I evaluate AI accuracy before using it?
Start with the decision the system will inform—not with a model’s general benchmark. AI accuracy depends on the intended use and the conditions under which it is tested; a score from a different task, dataset or operating environment does not establish that the system will perform safely at your workplace.

NIST AI RMF 1.0 reproduces a definition from ISO/IEC TS 5723:2022: accuracy is the “closeness of results of observations, computations, or estimates to the true values or the values accepted as being true.” In practice, that means defining what counts as a correct answer for this HSE task and how you will establish the reference answer. A model’s confidence or fluency is not itself evidence that its answer is correct.

Document the task, who will use the output, what action may follow, the people and work activities affected, and the operating conditions in which the AI is intended to be used. Include the consequences of an incorrect answer and of no answer—for example, a missed hazard, an unnecessary alert, or a delay while a worker waits for guidance. Decide whether AI is appropriate for the task at all.

Set acceptance criteria and escalation rules before looking at test results. The criteria should reflect the severity of potential harm, the existing controls and the role of human review; there is no universal accuracy threshold that makes an HSE system safe. For risks that could lead to serious injury or death, NIST calls for the most urgent prioritisation and thorough risk management.
2. Build a realistic, held-out test
Use cases the system did not learn from, drawn or constructed to represent the actual workplace, equipment, data quality and operating conditions. Keep the test separate from development and tuning so it can provide an independent check. Record how cases were selected, how reference answers were established, what was excluded and how the test was run.
Include ordinary cases as well as rare but serious hazards, incomplete or ambiguous inputs, poor-quality data and foreseeable conditions outside the intended operating range. Check likely distribution shifts—for example, changes in site, shift, equipment or input quality. A polished demonstration or performance on development data does not establish field validity. NIST’s AI Risk Management Framework (AI RMF 1.0) emphasises evaluation in the context of intended use, with limitations documented.

3. Measure the errors that matter
Do not reduce results to one aggregate accuracy percentage. Report false negatives and false positives separately when their consequences differ. In hazard detection, a missed hazard may be more serious than an unnecessary alert, but the acceptable trade-off depends on the task and the controls around it.
- For detection or classification: measure missed cases and false alarms; use precision and recall where they fit the task.
- For risk scores or probabilities: assess calibration if people will use the stated probability to choose an action.
- Across the workplace: examine results by relevant sites, shifts, equipment, populations and input-quality conditions. An overall result can conceal a weak result in a consequential segment.
- For the complete process: assess whether the human-AI team reaches the right decision, not only whether the model’s output matches a reference answer.
Choose measures and acceptance criteria that answer the defined decision question. If an output is advisory, test how it affects the user’s decision; if it triggers or controls action, evaluate the resulting process and safeguards as well as the output.
How can I tell if AI is safe for health and safety decisions?
Accuracy is only one part of safety. Evaluate how the system behaves when conditions are poor, change unexpectedly or fall outside its intended scope. Define what a safe failure looks like: whether the system should refuse, flag uncertainty, request better input or hand the case to a competent person rather than return a confident but unsupported answer.

Test resilience and set a fallback
Challenge the system with foreseeable variations in inputs and operating conditions, including missing, ambiguous or degraded information. Establish who can override its output, when work must stop, and what manual or other fallback applies if the system is unavailable, uncertain or behaving abnormally. Make sure these steps are usable in the actual workplace, not just recorded in a procedure.

Include cybersecurity threats in the assessment. Consider whether interference with the system or its data could affect a safety decision, and identify how suspected incidents will be reported, contained and reviewed. NIST’s AI RMF Core provides a risk-management structure for documenting intended use, limitations, robustness and ongoing evaluation; it is voluntary guidance, not a safety certificate.
Evaluate the people-and-process system
Observe or simulate the full workflow with the people expected to use it. Check whether they can recognise errors, understand the limits of an output and respond correctly to uncertainty. Look for over-reliance on confident answers, missed warnings, unclear responsibility and loss of critical skills through automation. Involve relevant HSE and domain experts, along with human-factors expertise where appropriate.

Assign named roles for approving use, monitoring performance and reviewing incidents. Decide who has authority to pause or withdraw the system and how changes to the model, prompt, data, equipment, process or working conditions will be assessed before continued use.
What should an AI risk assessment include?
For workplaces in Great Britain where the Health and Safety Executive (HSE) is the enforcing authority, HSE’s policy statement published June 12, 2026 says that workplace AI affecting health and safety requires a risk assessment and appropriate controls to reduce risk so far as is reasonably practicable; it also says to include cybersecurity threats. The policy concerns that jurisdiction and enforcement context, not every country or every regulator. Check the requirements that apply to your workplace and sector.

HSE states: “Health and safety legislation requires a risk assessment to be undertaken for uses of AI which impact on workplace health and safety and appropriate controls to be put in place to reduce risk so far as is reasonably practicable.” Read the HSE policy on its regulatory approach to AI. The NIST AI RMF can help structure the assessment, but it is voluntary, use-case agnostic and not a substitute for legal compliance or proof that a system is safe.

A practical assessment record should bring together the decision boundary, evidence, controls and accountability in one place:
- Use and boundaries: intended task, users, affected people, permitted inputs, operating conditions, prohibited uses and the consequences of wrong, late or unavailable output.
- Evidence and acceptance: test-set design, reference-answer method, test conditions, performance measures, results by relevant segments, acceptance criteria and known limitations.
- Risk controls: human review, override authority, uncertainty handling, safe failure, stop-work triggers, fallback arrangements and cybersecurity risks and controls.
- People and accountability: user training, responsibility for approval and monitoring, incident reporting and review, and any human-factors findings.
- Change and withdrawal: monitoring measures and thresholds, review cadence, reassessment triggers and the conditions for pausing or withdrawing the system.
How should I compare two AI systems for the same HSE task?
Compare systems only on the same task, held-out dataset and acceptance criteria. Ask each vendor for its test protocol, limitations and change-control commitments—not just a headline score. Your own workplace evaluation should establish whether the evidence applies to your conditions.
| Comparison area | What to compare | Why it matters |
|---|---|---|
| Errors and workload | Missed-hazard rate, false-alarm burden and fit-for-purpose measures such as precision and recall | A system with fewer misses may still create an alert burden that users cannot manage; the acceptable balance depends on the hazard and workflow. |
| Coverage | Performance across relevant sites, populations, shifts, equipment and input-quality conditions | Aggregate results can mask weaker performance in a group or condition that matters. |
| Robustness and uncertainty | Behaviour under changed conditions, poor or ambiguous input, and uncertainty; calibration if probabilities guide action | Real operating conditions may differ from development or test conditions. |
| Human-AI workflow | Whether users detect errors, understand limitations and reach safe decisions; effects on skill and reliance | Workplace safety depends on the combined process, not just the model output. |
| Safeguards and operations | Override and fallback capability, cybersecurity and data handling, monitoring and incident response | These determine how failures are contained, detected and addressed during use. |
| Vendor evidence and change control | Test protocol, documented limitations and commitments for notifying and managing changes | Headline scores alone do not show whether evidence transfers to your use or remains valid after changes. |
How do I keep an approved system under control?
Approval should be limited to the use and conditions actually assessed. Put monitoring in place before deployment, with an owner, operating measures, alert thresholds, a review cadence and an incident log. Monitor both system performance and whether the workflow is producing the intended safe decisions.
