Document a road construction defect by pairing dated, identifiable photographs with field notes made during or immediately after the inspection. Record the project, precise location, inspection time, work phase, relevant conditions, and what is visibly present. Keep observations separate from conclusions about cause, responsibility, or compliance, and follow the project’s contract and agency procedures.
What makes a site photo useful later?
A photograph can show a condition, but without context it may be difficult to establish which project, location, or phase of work it depicts. Give each image a clear connection to the inspection record: identify the project and location in your notes, describe the work being inspected, and reference the photo by a consistent identifier or filename.

The Federal Highway Administration’s Construction Program Management and Inspection Guide says observations should be recorded while on the project or immediately following inspection. It also recommends photographs and sketches to depict details and document the inspection. Treat images as supporting material for the written record, not a replacement for it.

How to document a defect in the field
- Identify the inspection. Record the project or contract identifier, road or segment, lane and direction where relevant, and the location reference used by the project, such as stationing. Include the inspector’s name and role.
- Record when and what was happening. Write the inspection date and local time, work phase or activity, and relevant weather or roadway conditions if they affect what can be seen. Add a time zone if records may later be compared across zones.
- Photograph context and detail. Take a wider view that helps orient a later reader, then closer views that show the condition. This is a practical framing approach based on FHWA’s guidance to use photos to depict details, not a prescribed agency shot list.
- Make dimensions legible when they matter. If measurement is appropriate and within your role, record the value, units, method, and who measured it. A scale in the photograph may help explain the measurement. FHWA discusses measurements or tests as a way to support disputed observations, but does not prescribe a particular measuring tool or photo marker.
- Describe visible facts. State what you can observe, where it is, and any measurement you actually took. Do not infer from a photograph alone when the condition formed, what caused it, who is responsible, or whether a specification was breached.
- Link photos to notes and track follow-up. Reference each image using a consistent identifier. Record who was notified, what action was requested or agreed, and when the item should be checked again, following project procedures.
Example of factual wording
“At the eastbound outside lane near station [project station], the surface has a visible longitudinal crack approximately [measured dimension] long.” Replace bracketed text only with information actually observed or measured. If the dimension was not measured, omit it rather than estimate it from the image.

This is a writing aid, not an official form. The project’s agency and contract may set different fields, signature requirements, retention rules, or submission routes.
How to make the record stronger when a condition is disputed
Keep the original observation distinct from any interpretation. If the issue is technical or disputed, appropriate measurements or tests may help verify what was observed; follow the project procedure and your authority as an inspector. FHWA’s inspection guide discusses this supporting role for measurements and tests. Do not present a visual impression as a test result or a determination of contract compliance.

Use the governing contract, current agency procedures, and applicable local law as the controlling requirements. FHWA’s Construction Program Management and Inspection Guide dates to August 2004; it offers general federal inspection guidance, not a substitute for current project documents or state procedures. FHWA lists it among its construction technical guidance.

Requirements are not universal. For Federal Lands highway projects where applicable, the 2024 FP-24 specifications replaced FP-14 and include daily reporting provisions for work locations, inspection results, and observed deficiencies. Confirm the edition and contract modifications that govern the project. In England, the Department for Transport’s third edition Inspecting street works code came into force on 1 April 2023 and addresses street-works and reinstatement inspections; it is specific to that jurisdiction.
