How to Customize the Header Image in Google Forms

Learn where to add a header image in Google Forms, how to replace or crop it, and whether Google requires a particular banner size.

To add a custom banner, open your form, select Customize theme, then choose Choose image under Header. Pick an available image source and follow the prompts. You can replace the image or reposition its crop after it appears.

Add a header image

  1. Open the form in Google Forms.

  2. At the top right, select Customize theme.

  3. In the Theme panel, go to Header and select Choose image.

  4. Choose an available source, then follow the prompts to insert or save the image. Google documents built-in themes and image selection through Upload, Photos, Google Drive, Google Images, or By URL; which options appear can vary. See Google’s instructions for changing a form’s theme.

  5. Preview the form to check how the banner appears above the questions.

Choose the image source that fits your situation

Replace or reposition the header

Replace the image

Click the existing header image and choose Replace Header Image. Select another image source and follow the prompts.

Adjust the crop

Click the header image and select Reposition Header Image. Drag the crop box corners to adjust the framing, then save. Preview the form afterward and make sure important text, logos, or faces remain visible. Google documents the repositioning control, but does not publish an official safe area or pixel specification.

What size should a Google Forms header image be?

Google’s help page does not specify a required pixel dimension for a Forms header. A Google Docs Editors Community respondent, Greg Gambone, recommended 1600 × 400 pixels in an answer dated August 20, 2019, and cautioned that the top of the question area obscures the bottom center of the banner. That is older, unofficial advice—not a current Google requirement or a guarantee of how every form will render. You can use it as a starting canvas, then adjust the crop and check the preview. Read the community thread.

The current Google help page reviewed also does not state a required file-format list. Avoid treating older community troubleshooting posts naming specific formats as an exhaustive current specification.

If an organization theme is missing

Organization-branded themes appear only when an administrator makes them available. If you cannot find your organization’s theme, ask your Workspace administrator whether it has been enabled. Google Workspace Learning Center: Create your first form in Google Forms.
