How to Compare Biological Age Tests: Biomarkers, Methods and Limits

Biological age tests measure different signals and predict different outcomes. Compare the sample, assay, algorithm, validation and uncertainty before interpreting a score.

Compare biological age tests by what they measure, which sample and assay they use, what their score is designed to predict, and how well that score has been validated and repeated. “Biological age” is not one standardized quantity: tests can produce different results because they capture different aspects of biology. An age-in-years score is not a diagnosis, a direct estimate of lifespan, or proof that every organ is aging at the same rate.

What does a biological age test actually measure?

The label alone does not identify the biological signal. A test may analyze DNA marks, proteins, routine clinical measurements, telomere length, RNA, or physical and cognitive function. Its result depends not only on the biomarkers but also on the sample, laboratory method, algorithm and intended target.

A 2023 review in npj Aging notes that there is no standardized, widely accepted set of aging biomarkers or mathematical model for quantifying biological age. Methods can include linear regression, principal-components analysis, the Klemera-Doubal method, machine learning and epigenetic calculators. That is why a score’s method and purpose matter as much as the number it reports.

How do the main test methods differ?

Method Signal and common sample What to check Main limitation
DNA methylation (epigenetic clocks) Age-associated chemical marks on DNA, often measured in cells from blood or another tissue. Clock name and version; sample tissue; assay; whether it estimates chronological age, risk or pace of aging; and validation evidence. Different clocks target different outcomes. A blood result reflects cells represented in the sample, not a direct measurement of how every organ is aging. Cell composition and measurement noise can affect the result.
Proteomic clocks Protein levels in blood or other bodily fluids; some profiles are associated with particular organs. Which proteins and measurement platform are used; whether the score is whole-body or organ-associated; and the population and outcomes used for validation. Protein patterns can reflect several biological processes. An organ-associated estimate is not a diagnosis of that organ.
Clinical or metabolic composites Routine measures such as glucose, cholesterol, inflammatory markers or immune markers combined mathematically. Which measurements are included; the algorithm, such as KDM or another composite; the reference population; and whether it adds predictive value beyond chronological age. The score may describe current health and risk factors rather than an aging trajectory.
Telomere length The length of protective chromosome ends, generally measured in sampled cells. Assay method; cell or sample type; repeatability; and whether the evidence supports the test’s specific claim. Telomere length varies substantially among people and, by itself, provides limited information about biological age.
RNA or transcriptomic clocks RNA transcripts that reflect active gene expression. Collection and processing conditions, assay reproducibility and external validation. Gene transcription can change quickly, making results potentially noisy.
Physiological, cognitive or wearable measures Function or signals such as blood pressure, fitness, strength, mobility, cognition or wearable-derived vital signs. The exact function or signal, measurement duration, age reference and whether the result is a functional measure or a biological-age claim. These measures are not interchangeable with molecular clocks and may primarily describe current function or health status.

A 2017 analysis by Belsky and colleagues compared seven measures in 800 members of the Dunedin birth cohort and examined their relationships with physical, cognitive and subjective health-span-related characteristics. The comparison illustrates why results from distinct measures should not be treated as readings of one identical underlying quantity.

What is each score designed to predict?

Find the test’s intended target before interpreting its age number. Estimating chronological age, predicting mortality or disease risk, measuring pace of aging, and producing a composite “age in years” are different tasks. Performance on one does not establish performance on another.

Stanford Medicine describes early Hannum and Horvath clocks as designed to estimate chronological age, while GrimAge was designed around mortality or future disease risk. These examples show why a test should name its clock and explain its purpose rather than rely on a generic “biological age” label.

A clock that closely tracks chronological age has not necessarily shown that it predicts clinically meaningful outcomes or improves medical decisions. Look for peer-reviewed external validation in a population relevant to you, the outcomes assessed, and evidence that the score adds information beyond chronological age. If the service claims to measure change, look for longitudinal evidence and repeat measurements—not just a cross-sectional association.

How can you compare tests before choosing one?

  1. Identify the specimen and assay. Check whether the test uses, for example, blood-cell DNA methylation, plasma proteins, telomere length or routine clinical biomarkers. Ask which laboratory method is used and whether the reported result is based on the sample actually collected.
  2. Pin down the algorithm and target. Find the clock or composite’s name and version, what it was trained to predict, and whether the output represents chronological age, risk, pace or another score. If those details are not disclosed, the number is difficult to interpret.
  3. Inspect the validation. Look for external, peer-reviewed testing; the population and outcomes; and whether the score predicts useful outcomes beyond chronological age. A correlation with age alone does not demonstrate clinical utility.
  4. Check reproducibility and uncertainty. Ask what happens when a comparable sample is tested again, what size of change is meaningful, and how sample handling or cell composition affects the result. Stanford Medicine reports that many epigenetic clocks have roughly three years of error; it also notes that blood-cell proportions can vary over a day. Treat that as a general caution, not a guaranteed error range for every test.
  5. Match the sample to the claim. A blood-based test measures signals in cells or substances represented in blood. It does not directly sample the brain, heart or kidneys. Organ-specific aging signatures remain an active research area rather than an established individual diagnostic tool.
  6. Decide what you will do with the result. Treat a consumer score as informational unless there is a clearly established clinical use. Do not use a non-diagnostic result as a substitute for clinical evaluation or as the sole basis for a health decision.

Why can two tests give different ages?

Tests can disagree without either having measured the same thing incorrectly: one may estimate chronological age from DNA methylation, another may summarize protein patterns or routine health markers, and another may measure a functional ability. Even tests in the same category can use different biomarkers, tissues, algorithms, reference populations and targets.

For molecular tests, the sampled tissue matters. A blood-derived result describes signals in the blood sample; it cannot establish that all organs share that estimate. For any method, measurement noise and biological variation can also make small changes difficult to interpret. A difference between two scores is therefore not, by itself, evidence that one test is accurate and the other is not—or that a person has accelerated or reversed aging.

Are biological age tests accurate, and are they medical tests?

There is no single accuracy figure that applies to all methods or providers. Accuracy must be judged against the test’s stated target, the validation population, the assay, and the uncertainty of the result. A test may estimate chronological age reasonably well while offering no established diagnosis or proof that its score can guide treatment.

In a U.S.-specific explainer dated September 23, 2026, Stanford Medicine states that direct-to-consumer biological-age clocks are not FDA-approved as medical diagnostic tools. This is not a global regulatory conclusion. Separately, Singapore’s Ministry of Health stated on September 9, 2026 that tests intended solely for general wellness, with no claim to assess, diagnose, treat or prevent a medical condition, are generally non-clinical and are not regulated as medical devices under its Health Products Act. Regulatory status depends on jurisdiction and on the test’s claims.

Before relying on a result, distinguish an informational wellness score from a medically validated test for a specific condition. If a score raises a health concern, discuss the concern with a qualified clinician rather than treating the age number as a diagnosis.
