ECG, sleep-apnoea notifications, hypertension notifications, and newer readiness insights have different model requirements, age limits, and regional availability. First confirm the feature is supported on the exact model sold in your country; then weigh comfort, battery, display, and price.
Start with the health feature you want
Apple’s feature comparison is the quickest way to screen models, but Apple’s lineup and feature availability can vary by region. Its current materials identify Series 12 and Ultra 4 as introducing a newer Health Sensing System and readiness experience; other regional comparison pages list different lineups. Check the Apple Watch comparison page for your country before buying: Apple Watch models compared.

- For ECG: Look for a supported Series or Ultra model, not an SE.
- For sleep-apnoea notifications: Check the listed model eligibility and whether you meet the intended-use criteria.
- For hypertension notifications: Treat this as a notification feature, not a blood-pressure monitor or diagnosis.
- For sleep and everyday wellness: Compare the specific sleep and wellness features on each model, then decide whether the newer readiness experience matters to you.
Once you have eliminated models that do not offer the feature you need, confirm age and intended-use limits, regional availability, and then compare physical fit and cost.
Compare health features and eligibility
| Feature | Models Apple lists | Important limits |
|---|---|---|
| ECG app | Series 4 and later, except SE models; all Ultra models | Produces an ECG similar to a single-lead electrocardiogram. Intended for people aged 22 or older. |
| Sleep-apnoea notifications | Series 9 and later, Ultra 2 and later, and SE 3 | Designed to identify signs of moderate to severe sleep apnoea in people aged 18 or older who have not been diagnosed with sleep apnoea. |
| Hypertension notifications | Check Apple’s comparison for your country and model | Not a blood-pressure reading. Not intended for people under 22, people previously diagnosed with hypertension, or pregnant people. The algorithm looks for patterns over time using optical heart-sensor data. |
| New Health Sensing System and readiness experience | Series 12 and Ultra 4, in Apple’s September 2026 announcement | Apple describes readiness as insight into the body’s capacity to take on the day; it is not a clinical assessment. |
Feature eligibility and availability can change across model generations and markets. Verify the exact regional model rather than assuming that a feature listed for one country or lineup is offered everywhere.

What the main features do—and do not—tell you
ECG
Apple says the ECG app can generate a recording similar to a single-lead electrocardiogram. That is a specific measurement capability, not a guarantee that the watch will identify every heart problem. Eligibility also matters: Apple’s comparison lists the app for Series 4 and later except SE models, and for all Ultra models, with use intended for people aged 22 or older.
Sleep-apnoea notifications
Apple describes this feature as looking for signs of moderate to severe sleep apnoea, not as diagnosing the condition or detecting every instance. Apple lists support on Series 9 and later, Ultra 2 and later, and SE 3. It is intended for people aged 18 or older who have not already been diagnosed with sleep apnoea.

Hypertension notifications
These are pattern-based notifications, not a cuff-style blood-pressure measurement. Apple says the algorithm uses optical heart-sensor data over time. The feature is not intended for people under 22, people previously diagnosed with hypertension, or pregnant people. Apple says more than 5 million nights of sleep data from the Apple Heart and Movement Study were used to develop and test its sleep-score algorithms. Apple also says its scoring approach and prioritization algorithm were informed by guidance from the American Academy of Sleep Medicine, National Sleep Foundation, and World Sleep Society. This is Apple’s account of its methodology, not independent proof of comparative performance: Apple’s watchOS 26 announcement.
What Series 12 and Ultra 4 add
In its September 2026 announcement, Apple said Series 12 and Ultra 4 introduce a new Health Sensing System with higher-frequency heart-rate and heart-rate-variability measurements, plus a readiness experience. Apple describes readiness as insight into the body’s capacity to take on the day. These are Apple’s product descriptions; they should not be read as an independent clinical assessment or as evidence that either model is more accurate for every health purpose.

Apple’s vice president of Worldwide Apple Watch Product Marketing, Kaiann Drance, called Series 12 “the most advanced health and fitness companion we’ve ever created.” That is a company marketing claim, not an independent comparison of the watches.

Apple’s regional comparison pages do not all show the same models: one surfaced Series 12 and SE 3, while other regional results surfaced Series 11, SE 3, and Ultra 3. Availability and feature matrices are market-specific, so use the comparison page for the buyer’s country and confirm the exact generation before purchasing.

After narrowing the choice by health feature, compare the factors that affect everyday use:
- Fit and comfort: Choose a size and case you can wear comfortably during the activities you want to track, including sleep if overnight data matters to you.
- Battery: Consider whether the watch’s charging routine works with your intended use, especially overnight wear.
- Display and size: Compare the physical dimensions and screen experience against your comfort and visibility preferences.
- Price: Check Apple or a reputable retailer in your market for current local pricing. The available information does not establish comparable current prices across regions.
A practical buying sequence
- Name your priority: Decide whether you chiefly want ECG, sleep-apnoea notifications, hypertension notifications, sleep insights, or broad wellness tracking.
- Eliminate unsupported models: Use Apple’s comparison page for your country and check the exact model generation against the feature you want.
- Check eligibility: Review relevant age thresholds and intended-use limits, particularly for ECG, sleep-apnoea, and hypertension features.
- Confirm local availability: Make sure the model and feature are available where you live, rather than relying on a comparison page from another region.
- Compare daily wear: Choose among the remaining models based on fit, display, battery routine, and current local price.
Use health features as information, not a diagnosis
Apple Watch metrics and notifications can provide information, but they are not a substitute for medical care. A notification is not a diagnosis, and the absence of one does not establish that a condition is absent. For symptoms or health concerns, speak with a qualified healthcare professional.
