An online screening can be a convenient first step, but it is not the same as a diagnostic hearing assessment.
Screening or diagnostic hearing test: what is the difference?
A screening is a preliminary check that may help you decide whether to seek care. A diagnostic evaluation aims to identify the type and degree of hearing loss and guide next steps. The American Academy of Otolaryngology–Head and Neck Surgery’s 2024 age-related hearing-loss guideline identifies pure-tone audiometry as the gold standard for determining ear-specific severity and type. It also notes that online testing cannot ensure sound-controlled conditions. Read the 2024 guideline.

An audiological examination can assess hearing ability and help determine whether you may benefit from hearing-related care or need referral for medical or surgical treatment. A medical examination can check for potentially treatable causes such as infection, injury, deformity, or earwax. FDA guidance on getting hearing aids explains the role of professional assessment.

| Option | What it can offer | What to check |
|---|---|---|
| Audiologist | A professional evaluation that can characterize hearing loss and guide care or referral. | Confirm the clinician’s credentials and local license; ask which tests and follow-up are included. |
| Hearing-care clinic | An in-person setting that may provide professional evaluation and hearing-aid assessment when qualified staff are available. | Ask who will perform the test, what their credentials are, what the assessment covers, and how medical referrals are handled. |
| Online screening | A convenient preliminary check, potentially useful when access to in-person care is limited. | It may be done outside controlled listening conditions and does not establish a complete medical diagnosis. Seek professional assessment if symptoms continue or the result concerns you. |
| Tele-audiology | Remote delivery of some professional audiology services, including screening, assessment, treatment, and device follow-up. | Available services depend on local licensure, applicable rules, and standards of care. Ask whether an in-person test is needed. |
The American Academy of Audiology’s 2021 telehealth position statement includes screening, assessment, treatment, device follow-up, monitoring, and some vestibular services within tele-audiology, provided applicable licensure and standards of care are met.

When should you get a hearing test?
Consider contacting a doctor or hearing professional if you often struggle to understand speech, need the television louder than others, ask people to repeat themselves, strain to hear, or hear better in one ear. These are possible signs of hearing loss, not a diagnosis. The FDA’s hearing-loss information recommends considering professional testing.
NIDCD’s adult questionnaire suggests considering an audiologist or ENT if you answer “yes” to three or more questions. This is a screening prompt, not a diagnosis; the page was updated August 16, 2022, and the questionnaire was originally published in March 2016. See NIDCD’s self-check.

The need for accessible assessment is substantial in the United States: NIDCD reported that approximately 37.5 million adults said they had some degree of hearing loss, and that about one in four adults who could benefit from hearing aids had ever used them. Both figures are from its page last updated March 17, 2023, and describe U.S. adults. NIDCD’s adult hearing health care page.
Can you take a hearing test online?
Yes. An online screening can be a useful first check, particularly if an in-person appointment is difficult to access. Its result should not be treated as a diagnosis: the listening environment may not be controlled, and the test may not establish ear-specific severity or type of loss. If you have persistent difficulty or a concerning result, follow up with a professional.

How to choose a provider
- Ask who conducts the evaluation. Confirm the person’s professional credentials and whether they are licensed under the rules where you live.
- Clarify the scope. Ask whether the service is only a screening or includes diagnostic assessment, and whether it can assess each ear and characterize the type and degree of loss.
- Check the testing conditions. Ask how the provider controls sound during testing and whether an in-person visit is needed for a complete evaluation.
- Ask what happens after the test. Find out whether the provider explains results, arranges follow-up, and refers you for medical evaluation when indicated.
- Confirm local costs and coverage. Prices and insurance coverage vary by location and provider; ask what the visit and any follow-up will cost before booking.
Is choosing a hearing test the same as choosing a hearing aid?
No. An assessment helps identify hearing status and appropriate next steps; a hearing aid is a treatment device. In the United States, adults aged 18 or older with perceived mild to moderate hearing loss may buy over-the-counter hearing aids without an audiological exam. FDA says OTC devices are not intended for children or people with severe or profound hearing loss. Buying a device does not establish the cause, type, or degree of your hearing loss. FDA’s OTC hearing-aid guidance describes who these devices are for.
For prescription hearing aids, FDA states: “The FDA does not require a medical examination or hearing test for prescription hearing aids, but your State may require you to visit an audiologist or other licensed professional first.” Check the rules where you live and do not treat access to a device as a substitute for evaluation when you have hearing concerns.
