Make sure the nose and mouth stay clear, then recheck after putting the baby in, after any adjustment or movement, and regularly while carrying. If you cannot maintain these checks, take the baby out and reset the wrap.
How to check your newborn’s position with TICKS
Australia’s ACCC Product Safety describes the TICKS checks for baby carriers, wraps and slings. The NHS in the UK gives similar positioning guidance. These checks help you spot unsafe positioning; they cannot guarantee safety or replace the wrap’s instructions.

- Tight: Hold your baby close enough that they cannot slump down inside loose fabric. Slumping can restrict breathing.
- In view: You should be able to see your baby’s face by looking down. Keep the face uncovered; fabric or your body must not cover the nose or mouth.
- Close enough to kiss: Your baby’s head should sit high against your chest, close enough to kiss by tipping your head forward.
- Keep chin off chest: Keep the neck straight and the chin lifted away from the chest. A curled chin can restrict the airway.
- Supported back: Hold your baby close with the back supported in a natural position.
The ACCC’s wording is: “Keep your baby: Tight, In view at all times, Close enough to kiss, Keep chin off chest and Supported back.”

Check the position as soon as your baby is in the wrap, and repeat the check after you move or adjust it and regularly while carrying. A baby in distress may not make noise or move obviously, so do not rely on crying or visible movement as your only signal.

Confirm that the nose and mouth remain unobstructed and that you are using the wrap as its manufacturer directs. If the baby slumps, the face is hidden, the chin drops to the chest, or the back is not supported, take the baby out and reset the wrap before continuing.
A Royal Cornwall Hospitals NHS Trust neonatal procedure describes gently pressing a hand against the baby’s back as a practical check: the baby should not uncurl or move closer. This is a check from that trust’s neonatal procedure, not a universal diagnostic test or a substitute for the TICKS checks.

When a newborn may need extra advice
ACCC identifies babies younger than four months, premature babies, babies with low birth weight, and babies with breathing difficulties as having greater risk in carriers, wraps and slings. It advises consulting a paediatrician before using these products with a premature baby. The American Academy of Pediatrics (AAP) also cautions that premature infants and infants with respiratory problems should not be placed in backpacks or other upright positioning devices because the position may make breathing harder.

If your baby was premature or has a medical or breathing concern, ask your clinician for advice specific to your baby before using a wrap. For every baby, check the particular wrap’s stated age, weight and developmental limits; guidance for one model does not establish that another is suitable.

Feeding and sleep in a wrap
Take your baby out to feed
The NHS advises: “If you need to feed your baby, take them out of the sling.” Do not feed your baby while they remain positioned in the wrap.
A wrap is not a safe sleep environment
ACCC says a carrier, wrap or sling is not a safe place for a baby to sleep. If your baby falls asleep in the wrap, keep checking breathing and chin position, and move the baby to a safe sleep environment as soon as possible. Do not let the wrap material cover the baby’s head.

Check the wrap and its instructions
ACCC recommends choosing a wrap made from strong material that suits the baby’s age, size and development and comes with instructions. It also recommends getting a demonstration and help the first time you use it. A suitable product does not by itself ensure safe positioning: follow its instructions and keep checking the baby’s position.
