How to Care for Someone After a Heart Transplant

A practical caregiver guide to medication routines, home readings, infection precautions, follow-up appointments, warning signs, and recovery after a heart transplant.

After a heart transplant, the most important ways to help are to support the recipient’s exact medication routine, keep follow-up appointments, watch for changes, and contact the transplant team promptly when something seems wrong. The team’s current discharge plan takes priority over general guidance, including for medication instructions, monitoring thresholds, activity limits, infection precautions, and emergencies.

What does a caregiver do after a heart transplant?

Care needs vary with the recipient’s condition and the transplant center’s plan. At first, a caregiver may help manage medicines, record home readings, arrange transportation, prepare meals, and follow activity restrictions while the recipient regains independence. Agree with the recipient and care team on who is responsible for each task and how concerns will be reported.

Before leaving the hospital, make sure you know the transplant team’s contact numbers, after-hours process, and emergency instructions. Ask for written directions on medication timing, home measurements, activity, wound care, and when to call. If instructions are unclear or seem to conflict, contact the team rather than guessing.

How can I help them take medicines safely?

Anti-rejection medicines are a lifelong part of protecting the transplanted heart. Help the recipient follow the prescribed schedule exactly, and do not stop, change, or skip a dose unless the transplant team directs it. Tell the team promptly about missed doses, possible side effects, or concerns about the regimen.

  • Keep a current list of medication names, doses, and scheduled times, using the transplant center’s format if it provided one.
  • A pill organizer can help with routine, but check it against the current medication list whenever a prescription changes.
  • If a dose is missed, vomited, or followed by a possible reaction, ask the transplant team or pharmacist what to do. Do not guess or double a dose.
  • Tell the transplant team before a new prescription or other medication is started, and report medication changes made by any clinician. This includes antimicrobial treatment.

The American Society of Transplantation (AST) caregiver toolkit recommends learning the medicines’ purposes and possible side effects as well as helping the recipient take them as prescribed.

What should I track at home?

AST identifies blood pressure, temperature, and weight as measurements a caregiver may help record. Follow the transplant center’s directions for when and how to take them, how often to record them, and which changes require a call. A home blood pressure monitor may be useful if the team asks you to track blood pressure; it does not replace the center’s monitoring or instructions.

The 2022 International Society for Heart and Lung Transplantation (ISHLT) guideline includes examples of changes clinicians should report to the transplant center, such as fever at or above 101°F, a weight gain of at least 2 pounds in a week, unexplained weight loss of more than 5 pounds, or an unexplained drop in blood pressure. These are guideline examples, not universal thresholds for every recipient. Use the patient’s own written thresholds and call plan.

How can we reduce infection risk?

Immunosuppressive treatment helps prevent rejection but increases the risk of infection. Practical precautions in guidance from Cleveland Clinic and Johns Hopkins include washing hands carefully, avoiding close contact with people who are ill, and keeping cuts and wounds clean. Johns Hopkins also advises avoiding crowds, especially in the first months, and following the doctor’s vaccine recommendations. Ask the transplant team which precautions and vaccines are appropriate for this recipient.

Some medicines, including prednisone, can mask usual signs of infection, so a person may not show the symptoms you expect. Contact the transplant team promptly about concerning changes, including fever or chills; incision redness, warmth, opening, or drainage; wounds that do not heal; persistent cough or sore throat; mouth patches; or nausea, vomiting, or diarrhea. Fever thresholds in published guidance are not identical, so follow the center’s specific written threshold and call plan.

Which changes need urgent attention?

Call the transplant team promptly about breathing difficulty, unusual fatigue or reduced activity, fainting, chest pain, a notable blood-pressure change, rapid weight gain or unexplained weight loss, new swelling, new gastrointestinal symptoms, or a change in mental status. These can have different causes; the transplant team should decide what assessment is needed.

Use the emergency route in the recipient’s instructions for severe or rapidly worsening symptoms, or when the plan says to call emergency services. Do not delay emergency care while waiting for a routine callback. Keep the transplant team’s number and the patient’s written instructions easy to find.

Why are follow-up visits and tests so frequent?

Follow-up is part of transplant care, not optional administration. Blood tests help clinicians assess medication levels and side effects; tests such as an ECG or echocardiogram can assess heart function; and the team may monitor the transplanted heart’s blood vessels. Biopsies may be used to check for rejection, particularly during the first year. The transplant program sets the schedule and tests according to the recipient’s circumstances.

Rejection may not be obvious from symptoms alone, so keep scheduled appointments even when the recipient feels well. The American Heart Association describes medication monitoring, heart-function testing, and biopsy surveillance as parts of follow-up. AST notes that biopsy findings can lead clinicians to adjust medication or readmit a patient.

Early visits can be very frequent. AST’s toolkit describes visits several times a week at first as an example, not a schedule that applies to every center. A caregiver can maintain a calendar, arrange transportation, and bring the recipient’s medication and symptom records to appointments.

How can I support recovery without overstepping restrictions?

Follow the surgical team’s specific instructions for activity, lifting, and wound care. AST describes transportation, meal preparation, and help with restricted lifting as possible caregiver tasks during at least the first four to six weeks; this is general toolkit guidance, not a universal recovery timetable or lifting limit. Ask the team what the recipient may do and when restrictions can change.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Encourage the recipient to take part in their care as they are able, while helping with tasks that the team or recipient identifies. If activity or symptoms change, report that information as directed rather than making a new exercise or lifting plan independently.

How should I prepare to contact the transplant team?

Keep the contact plan and written instructions somewhere accessible. When calling, have the recipient’s recent readings, medication list, and a clear description of what changed and when. Share any new medicines or treatment changes and follow the team’s instructions for next steps. If you cannot reach the team and the situation is urgent, use the emergency instructions in the care plan.

How can caregivers look after themselves?

Caregiving can involve frequent travel, scheduling, and close attention to daily routines. Ask family, friends, or the transplant team about practical support when tasks become difficult to manage, and tell the team if you are unable to carry out part of the care plan. Protecting your own rest and wellbeing helps make ongoing support more sustainable.
