Reliable AI-agent guardrails do not depend on a prompt persuading the model to behave. Prompts can express policy, but authorization must be enforced by the system that executes an action: a tool wrapper, policy service, API, or downstream application. Treat every model-proposed action as a request to validate—not as permission to proceed.
Why prompt instructions are not enough
An agent combines model decisions with tools, memory, and information from outside the conversation. It may read a web page, email, document, or API response containing instructions that conflict with the user’s goal. A model can misunderstand those instructions or be manipulated by them, then propose an action with real consequences.

That is why a rule such as “never send confidential information” is not an authorization control by itself. The service that sends a message must still determine whether this actor may send these exact contents to this exact recipient. OWASP’s guidance on excessive agency describes the risk of giving an agent too much functionality, permission, or autonomy. OpenAI’s guardrail and approval guidance likewise distinguishes agent-level checks from checks at the tool that performs a side effect.
A useful security boundary is therefore outside the model’s own judgment: the model can suggest an action, while a separate, deterministic control decides whether it is allowed.

Reduce the agent’s capabilities first
The safest action to authorize is one the agent cannot perform in the first place. Inventory the agent’s tools, operations, data scopes, credentials, and reachable systems, then remove capabilities that are unnecessary for its task. OWASP’s AI Agent Security Cheat Sheet recommends constraining permissions and available tools; its excessive-agency guidance explains why excess functionality, permissions, and autonomy enlarge the possible impact of a failure.
- Offer narrow operations. Prefer a function such as
create_draft_replyto a generic shell, arbitrary SQL executor, or unrestricted browser action. A purpose-built operation should accept only the inputs needed for its job. - Separate reading from writing. Give read and write actions different operations and scopes. An agent that summarizes records should not automatically have the ability to alter or delete them.
- Scope identity and credentials. Use the current user’s identity and task-specific permissions where possible. Avoid shared credentials with broad access, and restrict the resources each credential can reach.
- Limit autonomy. Keep consequential actions behind a policy decision or review step rather than allowing open-ended tool use to continue unchecked.
Least privilege limits the blast radius if the model is confused or its input is manipulated; it does not establish that remaining actions are safe.
Keep untrusted content from becoming policy
Retrieved pages, emails, documents, and tool responses are data to process, not a source of authority over the agent. They can contain instructions designed to redirect the task or trigger disclosure or action. NIST’s CAISI describes agent hijacking as malicious instructions inserted into otherwise ordinary content an agent ingests in its January 2025 evaluation blog.

- Mark retrieved content as untrusted input and keep it distinct from system policy and the user’s request.
- When the task allows it, extract specific fields into a constrained schema rather than letting arbitrary text flow directly into an action plan.
- Where practical, isolate data-reading from tool execution so that text being analyzed cannot itself invoke a privileged operation.
- Before acting, compare the proposed operation with the user’s original intent. Do not let an instruction discovered in an intermediate document silently redefine that intent.
Clear prompt boundaries help the model understand which text is data, but they are not a security guarantee. OWASP’s prompt-injection prevention guidance treats screening and isolation as parts of a broader defense, not a way to prove that malicious content cannot influence a model.
Validate each action where it executes
Put authorization and argument validation in the component immediately responsible for a side effect: the tool wrapper, policy service, API, or downstream application. A check on the user’s initial input or the agent’s final answer is not enough if intermediate tool calls can already change state. OpenAI’s documentation on guardrails and human review notes that agent-level input guardrails run only on the first agent in a chain and output guardrails only on the final agent; checks for each custom tool call belong with the tool that creates the side effect.
For every proposed side-effecting operation, make the decision using the actual actor and normalized request—not merely the model’s explanation of what it intends to do. A practical check includes:

- Actor: Which user or service identity is requesting the action, and is it authenticated?
- Operation and target: Is this exact operation permitted on this resource under that identity’s current scope?
- Arguments: Do the structured arguments meet the schema, allowlists, and bounds for the operation? Reject unexpected fields and invalid values.
- Intent and context: Does the action fit the user’s original request, rather than an instruction found in untrusted content?
- Approval state: If policy requires review, is there a valid approval for this exact action and its parameters?
Authorize each tool that can cause a side effect, including tools called later in a multi-step or multi-agent workflow. An upstream check cannot reliably stand in for authorization at every execution boundary.

Scale human review to the action’s impact
Human review is a gate for actions that policy identifies as high-impact or difficult to reverse; it is not a substitute for authorization. Define risk categories for the deployment instead of treating every operation alike. A scoped read may be allowed automatically, while deletion, payment, privilege changes, external messages, or production changes may require review, depending on the system and its risk model.
| Action category | Example handling | Key control |
|---|---|---|
| Scoped, low-impact read | Proceed when the identity and requested resource are in scope. | Enforce read permission and resource scope at execution. |
| Reversible or limited write | Apply only if the operation and arguments satisfy policy; require review if the deployment classifies the change as consequential. | Validate the target and bounded arguments; record the policy decision. |
| High-impact or irreversible change | Pause for review when required by policy; do not execute on an ambiguous decision. | Bind approval to the exact normalized operation and parameters, expire it, and prevent replay. |
Present reviewers with the action that will actually execute, including its target and parameters. If those change after approval, require a new decision. A reviewer’s approval does not grant permissions the requesting identity lacks. OWASP’s agent security guidance recommends human approval for sensitive actions as part of a layered control system.

Test the complete workflow against hostile inputs
Test the deployed task, data sources, and tool paths together—not just whether the model follows a clean prompt. Build scenarios in which a realistic document, message, or page contains an instruction that conflicts with the task, then inspect whether the agent proposes a dangerous action and whether execution controls block it.
- Cover both direct instructions and indirect instructions embedded in content the agent retrieves.
- Measure outcomes for each task and attack scenario, not only one aggregate score.
- Repeat attempts: one successful defense does not show how the workflow behaves across varied inputs or runs.
- Inspect traces and tool-call arguments as well as final answers. A safe-sounding response can coexist with an unsafe attempted call.
- Add new scenarios when tools, data sources, policies, or defenses change.
NIST’s January 2025 CAISI blog discusses adaptive, task-specific, multi-attempt evaluations. Its described experiment used simulated Workspace, Travel, Slack, and Banking environments; those examples do not establish coverage of every real deployment.

Monitor and constrain behavior in production
Preventive authorization decides whether an action may execute. Operational controls help teams notice failures, contain unusual behavior, and recover; they do not replace that authorization boundary.
- Record policy decisions and execution outcomes, including the operation and result needed for investigation.
- Protect audit logs and avoid storing secrets or sensitive payloads unless they are necessary and appropriately secured.
- Alert on unusual tool use, repeated denials, unexpected targets, or changes in action patterns that may indicate misuse or guardrail drift.
- Set rate and resource limits, bound retries, and stop unbounded loops so a faulty workflow cannot consume resources or issue actions indefinitely.
- Fail closed when a critical policy check or required approval service is unavailable: do not execute the protected action by default.
OWASP’s excessive-agency guidance and AI Agent Security Cheat Sheet discuss logging and rate limits among operational controls, while its prompt-injection guidance supports defense in depth. These controls contain and surface problems; the execution-time check remains the decision point for permission.
A practical design review checklist
- Can the agent reach only the tools, resources, identities, and operations needed for its task?
- Are retrieved data and privileged instructions represented separately?
- Does every side-effecting tool validate actor, operation, target, arguments, intent, and required approval before execution?
- Are high-impact actions reviewed against the exact parameters that will execute, with expiry and replay protection?
- Do tests include hostile content, repeated attempts, and inspection of tool traces?
- Are policy failures, unusual behavior, retries, and resource consumption handled safely at runtime?
OWASP’s AI Agent Security Cheat Sheet summarizes the core separation: “The agent can propose an action, but a policy service or execution component should independently validate scope, privilege, and approval state before execution.”
