To create a survey in Qualtrics, start a survey project, add and arrange questions in the Survey Builder, set respondent paths in Survey Flow if needed, preview and publish, then distribute it from the Distributions tab. Before sharing, check privacy and response controls—an anonymous link does not automatically prevent collection of IP address and location data.
Start a survey project
In Qualtrics, create a new survey from the catalog or start from a copy of another survey, a file, or the library. The project contains the survey-building, distribution, analysis, and reporting work. Available project types and navigation can vary with account access. See Qualtrics’ survey platform overview.

Add and organize questions
Create questions in Survey Builder
Open the project’s Survey tab and use the Survey Builder to add questions. A project starts with a default question block. Select “Add new question,” choose a question type, then edit the question text and answer choices. Qualtrics groups questions in blocks; consult its Survey Builder guide for the available controls.

Questions receive numbers in the order they were created, not necessarily the order in which respondents see them. You can arrange questions and blocks to match the intended sequence. Qualtrics explains question creation and editing in its question guide.

Choose a simple sequence or conditional paths
If everyone should answer the same questions in the same sequence, arrange the blocks in that order. If answers should determine which questions or elements a respondent sees, configure Survey Flow. Qualtrics defines it this way: “The Survey flow is where you customize the order in which respondents experience the elements of your survey.” Flow elements can include branches, embedded data, and randomization. Read the Survey Flow documentation before adding conditional paths.
Configure the respondent experience
Use the Survey tab’s other areas to configure only what your study requires. Qualtrics lists Look and Feel, Survey Options, quotas, and translations alongside the builder and flow tools. Check the access and privacy settings as part of this setup, especially if the survey asks for identifying information or will be shared publicly.

Preview, publish, and distribute
- Preview the survey. Check the wording, answer choices, layout, navigation, and any conditional paths from the respondent’s perspective. Revise problems before launch.
- Finish structural edits before collecting responses. Changing a question type after responses have started may invalidate earlier responses to that question, according to Qualtrics’ question editing guidance.
- Publish changes. Saving an edit alone may not update the live survey; publish so changes appear at the live link. See Qualtrics’ publishing guidance.
- Open Distributions and choose a method. Qualtrics documents email, SMS, and a single anonymous link as distribution options. Availability depends on the account and project permissions. The Distributions overview describes the distribution workspace.
- Retrieve and share the link if using an anonymous link. Find it in Distributions and share it through the channel you intend to use. Activating the survey to obtain a reusable link opens it to responses, so do this when you are ready for collection to begin. Qualtrics explains the link in its anonymous link guide.
Check privacy and repeat-submission settings
“Anonymous” should not be read as “no potentially identifying data is collected.” Qualtrics says the anonymous link does not collect names or email addresses unless the survey asks for them, but IP address and location are recorded by default. Its documentation describes Anonymize Responses for removing certain personal data and security options for limiting repeat submissions. Review the current project settings and the anonymous link documentation before launch.
Choose distribution based on who should be able to respond, whether you need to contact a known list, whether repeat submissions should be restricted, and what data your settings collect. An anonymous link is straightforward to share broadly but is reusable by default. Email invitations or personal links may suit a known contact list; check your specific project settings and permissions rather than assuming what identity or response tracking is enabled.

After collection begins, use Data & Analysis to inspect or edit individual responses, Results for a dashboard of collected results, and Reports to create PDF-friendly reports. Access to these areas and their functions may depend on permissions. Qualtrics outlines these workspaces in its survey platform overview.
