A reliable human–AI workflow is not “AI plus a person who clicks approve.” It is an explicit allocation of work and authority: define the human outcome, assign bounded tasks to AI, reserve consequential judgment for an identified role, and build review, escalation, measurement and change controls around the combination. The person must have enough context, time and authority to disagree with the system; otherwise the human is only a decorative step.
This guide uses the National Institute of Standards and Technology (NIST) AI Risk Management Framework (AI RMF) as a design aid. The framework is voluntary and use-case agnostic. It does not replace applicable law, sector requirements, contracts or organizational accountability.

What a human–AI workflow is—and what it is not
A human–AI workflow connects a business or public-service goal to a sequence of actions in which software performs some work and people perform other work. The boundary is deliberate. AI might create a draft, summarize records, detect a pattern or recommend an option. A person might supply context, check evidence, decide among competing interests, authorize an irreversible action or handle an exception.

A reviewer who sees only a confident-looking score, has no source information, handles hundreds of cases per hour or cannot overrule the system is unlikely to provide meaningful oversight. NIST’s AI RMF Appendix C emphasizes that human–AI interaction results vary: a carefully designed combination can be complementary, while a poorly designed one can amplify human bias or error.

Design the workflow around five questions:
- What outcome are we trying to achieve, for whom and under what constraints?
- Which parts of the work can AI perform with an acceptable risk of error?
- Where must a person exercise judgment, and who has authority to decide?
- What evidence, time and controls does that person need to act independently?
- How will we detect failures, learn from them and change or stop the workflow?
Start with context and a bounded purpose
Do not begin with a model feature or a vendor demo. Begin by writing a short purpose statement that defines the intended outcome and the boundary of use. NIST’s Map function supports an initial go/no-go decision: an AI solution may be unsuitable when a simpler rule, search tool, database or human process can meet the need with less risk.
Describe the setting before choosing automation
- Goal and users: State the result the workflow should produce and the people who will operate it.
- Affected people: Identify anyone who may be evaluated, selected, denied, contacted or otherwise affected, including people who never interact with the system.
- Operating conditions: Record languages, data quality, volume, time pressure, accessibility needs, connectivity and likely out-of-scope uses.
- Constraints: Capture privacy, security, fairness, safety, records-retention, intellectual-property and sector-specific obligations.
- Benefits and costs: Describe the expected improvement and the resources, error costs and opportunity costs involved.
- Alternatives: Compare the proposed AI function with a non-AI baseline and with a smaller, more controllable automation.
- Reversibility: Decide whether an error can be corrected before anyone is affected, after notification or only through a costly appeal.
Mapping is not a one-time form. Revisit it when the task, users, data, model capability, operating environment or observed impacts change.
Use a task vocabulary rather than a feature list
NIST’s 2024 AI Use Taxonomy describes 16 AI use activities independently of a particular technique or industry. The value of the taxonomy is its common vocabulary: describe the contribution—such as creating, summarizing, detecting or recommending—before discussing a product. This makes it easier to compare an AI proposal with a rule-based or human alternative and to define what the system is not authorized to do.

What should the AI do?
Assign AI work that is bounded, observable and useful to a human decision-maker. A task is a better candidate when its inputs can be specified, its output can be checked, errors are detectable and a person can recover before consequences become permanent.
Good candidates for assistive work
- Create: Produce a first draft, structured form or code suggestion that a person can edit and validate.
- Summarize: Condense a known set of records while preserving links to the underlying material.
- Detect: Flag possible duplicates, anomalies, missing fields or policy-relevant patterns for investigation.
- Recommend: Rank options or suggest a next action while showing the evidence and uncertainty used.
- Transform: Convert information between approved formats, languages or schemas when the original remains available for checking.
These labels describe contributions, not permission to make a final decision. A recommendation can still influence a person strongly, so the workflow must specify how the recommendation is presented and challenged.

Tasks that need a stronger human boundary
Be cautious when the output affects access to essential services, employment, education, housing, health, safety, legal status, financial resources or reputation; when the evidence is sparse or contested; when context is largely tacit; or when an error is difficult to reverse. In such cases, AI may assist with retrieval, organization or drafting, but the accountable decision-maker needs independent evidence, a defined reason to disagree and a route to escalate.
Write explicit exclusions
Document what the system cannot do. Examples include making a final eligibility decision, contacting an affected person without review, inferring a sensitive attribute, using data outside the approved purpose, or continuing after a confidence, data-quality or system-health threshold is breached. Exclusions reduce ambiguity during busy periods and make testing possible.
Where does a person need to decide?
Place human judgment at points where it can change the outcome, not merely after the outcome is already committed. For every review point, define the reviewer’s task, information, time, authority and escalation path.
Give the reviewer an independent basis
- Show the input records or source passages that support the AI output.
- Display relevant uncertainty, missing data and known limitations in plain language.
- Separate the AI suggestion from the final decision field so acceptance is not the default action.
- Allow the reviewer to edit, reject, request more information, escalate or pause.
- Record the decision and, where appropriate, the reason for an override or escalation.
NIST recommends documenting system knowledge limits, how output may be used and overseen by people, and the proficiency expected of operators. Training should cover both the task and the system’s failure modes; a generic warning to “check the AI” is not enough.
Use risk to choose the review pattern
There is no universal requirement to add a person to every task. Some low-consequence, reversible operations may run without case-by-case review. Other operations require approval, two-person review or specialist escalation. Compare designs using consequence and reversibility of error, task ambiguity, known AI limitations, reviewer workload and authority, information quality, privacy and fairness needs, monitoring feasibility and the viable non-AI baseline.

| Configuration | Typical use | Required controls | Main failure risk |
|---|---|---|---|
| Manual | A person performs the task without AI output | Clear procedure, training and ordinary quality checks | Slow or inconsistent work; no automated assistance |
| Assistive | AI drafts, searches, summarizes or flags; a person decides | Source visibility, edit/reject controls and workload limits | Automation bias or unnoticed omissions |
| Gated automation | AI completes routine cases; specified cases stop for approval | Reliable routing criteria, pause controls, audit trail and escalation | Important cases are misclassified as routine |
| Delegated or autonomous | AI acts within a narrowly defined authority | Hard limits, transaction reversibility, continuous monitoring and a human stop mechanism | Fast, repeated errors before detection |
Treat the table as a set of design choices, not maturity levels. A gated workflow can be less safe than an assistive one if its gate is poorly calibrated or reviewers cannot intervene.

Map the complete path from input to outcome. For each step, record the input, action, output, governing instruction, handoff, owner and exception path. Label whether AI creates, summarizes, detects, recommends or performs another defined contribution.
| Workflow record | Question to answer |
|---|---|
| Input | What information enters, from which source, with what quality and permission? |
| Action | What does the person or system do, and what rule or instruction governs it? |
| Output | What artifact, score, recommendation or external action results? |
| Human role | Who operates the system, reviews the output and makes the consequential decision? |
| Authority | Who may accept, edit, reject, escalate, pause or resume the workflow? |
| Evidence | What information must be visible for an independent judgment? |
| Exception | What happens with missing data, uncertainty, policy conflict, outage or suspected harm? |
| Accountability | Which named role owns the result and the risk, even when several teams operate the system? |
Keep system operation, domain judgment, risk oversight and organizational accountability distinct when they belong to different roles. A person who monitors uptime is not automatically the person authorized to approve a high-impact decision.
Example: internal support-ticket triage
Suppose an IT team wants to reduce routing time. AI can classify the ticket, extract affected systems and suggest a priority. The service-desk operator checks the original request, corrects the category and sends routine tickets to a queue. A specialist decides whether a security or safety escalation is warranted. The service owner can pause automated routing when a new incident pattern appears.
The workflow should not let a classification silently close a ticket, expose confidential content to an unauthorized queue or assign an emergency priority without a human-defined rule. The example is useful because routing errors are often reversible, while an overlooked security incident may not be.

How do we make review meaningful?
Define a review contract before launch. It should state what the reviewer must inspect, how much time is reasonable, which cases require independent research and what actions are available. Set escalation criteria for uncertainty, out-of-scope requests, missing evidence, conflicting records and potentially high-impact outcomes.
Design the review screen around decisions
- Put the proposed action and its consequence where they are visible before approval.
- Provide relevant source material, not just a generated explanation.
- Make uncertainty and data freshness apparent.
- Offer separate controls for accept, edit, reject, request information, escalate and pause.
- Prevent silent submission when required evidence is missing.
- Show whether the system or a person made each change.
Measure workload as part of safety. A theoretically empowered reviewer may be unable to review thousands of cases, work under conflicting incentives or lack the domain expertise needed to recognize a plausible error. Define maximum queue size, response time, staffing and backup arrangements for the risk level.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How do we test the workflow?
Test the interaction, not only the model. NIST calls for testing before deployment and regularly during operation, including the human–AI configuration and relevant trustworthy characteristics and impacts.
Build a test set that resembles real use
- Normal cases: Use representative inputs and expected operating volumes.
- Edge cases: Include unusual language, rare categories, contradictory records and borderline decisions.
- Missing or poor-quality inputs: Remove fields, introduce stale data and test unreadable or malformed content.
- Ambiguous cases: Give reviewers plausible alternatives and conflicting evidence.
- Uncertainty cases: Check whether low-confidence or out-of-scope inputs trigger the intended pause or escalation.
- Interruption cases: Simulate an outage, delayed response, duplicate submission, partial handoff and recovery after restart.
- Adversarial or misuse cases: Test prompt injection, unauthorized data requests, deliberate manipulation and attempts to bypass approval.
- Human factors: Observe whether reviewers notice errors, understand the display and use the available controls under realistic time pressure.
Document methods and uncertainty
Record the test population, benchmark or comparison process, thresholds, known limitations, uncertainty and results. Do not report a model score as if it were workflow safety. A technically accurate classifier can still cause harm if routing, information display or escalation fails.
Recommended Free Tools
Run tests with the actual tools, handoffs, permissions and user roles. Re-test after changing prompts, models, data sources, interfaces, policies or staffing. Include affected-person feedback where appropriate and lawful.
Best Value
What should we measure after launch?
Choose measures tied to the task and its risks. A useful dashboard combines operational performance with evidence of control.
- Outcome quality: Error, omission, rework and resolution measures appropriate to the task.
- Coverage: The share of cases with usable inputs and the share sent to appropriate review.
- Escalation: Counts, reasons, response times and unresolved cases.
- Overrides: How often people change or reject AI output and why.
- Equity and harm signals: Disparate error patterns, complaints and adverse outcomes where measurement is appropriate.
- Security and privacy: Unauthorized access attempts, data leakage indicators and policy violations.
- Reliability: Downtime, latency, failed handoffs, duplicate actions and recovery time.
- Human conditions: Queue size, review time, training completion and evidence that reviewers can exercise authority.
NIST identifies override frequency and rationale as potentially useful data, while noting that further study is needed on how to empower people to challenge AI output. Interpret the rate diagnostically: a low rate may indicate good performance, excessive trust or lack of authority; a high rate may indicate useful correction, poor system quality or a changed task.
How should we handle errors or changing conditions?
Define an incident path before the first real case. Specify who can pause the workflow, who investigates, who communicates with affected people, how records are preserved and what conditions permit restart.
Use a practical response sequence
- Contain: Pause the affected function or restrict it to manual handling when harm may be continuing.
- Preserve: Keep the relevant input, output, version, instruction, reviewer action, timestamp and downstream action.
- Assess: Determine who may be affected, whether the error is isolated, and whether legal or contractual notification duties apply.
- Correct: Repair records or decisions where possible and provide a route for review or appeal.
- Learn: Identify whether the cause was data, model behavior, interface design, staffing, policy or an unsafe handoff.
- Change: Update the workflow, controls, training or scope; re-test before resuming.
Monitor for changes in data distribution, user behavior, regulations, threat conditions, system capability and the consequences of error. A workflow that was acceptable for low-volume internal drafting may become unsuitable when reused for a different population or a higher-stakes decision.
How does governance fit the build?
NIST’s AI RMF has four functions: Govern, Map, Measure and Manage. Governance is cross-cutting, and the functions form an iterative risk-management process rather than a compulsory fixed sequence. The NIST AI RMF Playbook offers voluntary suggested actions; NIST states that it is not a mandatory checklist or a set of steps every organization must follow. AI RMF 1.0 is being revised, so check NIST’s current framework and any sector-specific requirements before adopting a version.
Governance should establish policy, roles, accountability, documentation, training, risk tolerance and decision rights. Mapping defines context, purpose, affected people, alternatives and impacts. Measuring supplies evidence from tests and operation. Managing prioritizes risks, applies controls, responds to incidents and decides whether to continue, change or retire the system.
The framework’s central principle is lifecycle attention. As NIST’s AI RMF Core puts it: “Risk management should be continuous, timely, and performed throughout the AI system lifecycle dimensions.”
A build-and-review checklist
- Is the desired human outcome specific, bounded and worth automating?
- Have affected people, operating conditions, constraints and a non-AI baseline been documented?
- Is the AI contribution named in task terms rather than as a vague promise?
- Are prohibited uses and out-of-scope inputs explicit?
- Are operator, reviewer, decision-maker, monitor and accountable owner roles distinct where needed?
- Can a reviewer see relevant evidence and uncertainty?
- Can the reviewer independently accept, edit, reject, request information, escalate or pause?
- Are review staffing, training, time and workload realistic?
- Do test cases include normal, edge, missing-data, ambiguous, interruption and misuse scenarios?
- Are performance, incidents, escalations, overrides and reasons monitored?
- Is there a documented pause, investigation, correction and restart process?
- Are changes in context, capability, data and impacts triggers for remapping and re-testing?
Bottom line: allocate authority, then automate
Build the workflow from the decision backward. First define the outcome and risks; then assign AI a bounded contribution; then give a named person the information and authority required to exercise judgment. Test the complete interaction, measure what happens in operation and keep a credible path to pause, correct and change it. Automation is valuable when it strengthens a human process without hiding who decides or who is accountable.
