Build those rules before deployment, test them in complete spoken interactions, and keep monitoring them after launch.
Decide what “human-in-the-loop” means for your agent
The phrase can describe two different controls. One keeps a person in the decision path before the AI takes an important action; the other transfers an ongoing caller conversation to a person. A voice system may need both.

| Control | What happens | Use it when |
|---|---|---|
| Human approval | The agent pauses before an action, and an authorized person approves or rejects it. | An action affects people, money, compliance, or is difficult to reverse. |
| Conversation handoff | The agent routes the caller to a person and passes along relevant context. | The caller asks for help, the issue is out of scope or ambiguous, or the agent cannot safely complete the task. |
Microsoft responsible-AI guidance recommends human approval for actions that are hard to reverse or affect people, money, or compliance. Australian Government Digital Transformation Agency guidance likewise emphasizes human accountability and ongoing oversight, including when agents perform multi-step work. Keep records sufficient to explain and audit consequential decisions.

Set ownership, scope, and action boundaries
Name the accountable people
Assign a named owner for the agent’s behavior and an operational owner for the human queue or approval process. Define who can change escalation rules, approve consequential actions, review incidents, and authorize a rollback. A handoff is not a control if no one is responsible for receiving or acting on it.

Document what the agent can do
Write down the agent’s purpose, supported tasks, connected tools, permitted data, and out-of-scope requests. Classify each tool action as informational, reversible, consequential, or difficult to reverse. Make approval requirements explicit in the workflow rather than relying on the model to infer when an action is risky.
Do not assume that the same approval threshold fits every action. A request for general information may be completed automatically, while a change to an account or a step with financial or compliance consequences may require a person to review it first.
Write explicit escalation rules
Microsoft’s voice-agent guidance frames handoff as a design choice, including which intents require a person and whether a caller can ask for one at any time. Turn that choice into a short policy that engineers, agents, and reviewers can apply consistently.

- Honor a caller’s request to speak with a person, if the service offers that option.
- Escalate sensitive, ambiguous, or unsupported requests rather than guessing.
- Route repeated misunderstandings or low-confidence recognition to an appropriate recovery path.
- Escalate failed identity verification and tool errors when the agent cannot safely proceed.
- Define special handling for emergency or crisis-related requests, including the boundary of what the agent can and cannot do.
- Require human approval before designated consequential or hard-to-reverse actions.
Choose thresholds based on the application and validate them with representative calls. The cited guidance supports deliberate evaluation but does not establish universal numeric confidence or retry thresholds. For each trigger, specify whether the agent should clarify, retry, stop an action, seek approval, or transfer the conversation.
Design the spoken handoff and context transfer
Tell the caller what is happening
Use brief, plain language. The agent should explain that it is arranging a transfer, not claim that a person has joined before the telephony or contact-center system confirms the connection.
Example wording, not a tested script: “I’m connecting you with a specialist and will share what we’ve covered so far. If no one is available, I’ll explain the next option.”

Decide what identifying details and conversation information may be shared, and apply authentication and privacy rules before sending them. Give the receiving person enough context to continue without asking the caller to repeat everything, while avoiding collection or disclosure of information that is not needed.

Pass useful context, not just a transcript
Provide the human with a concise interaction summary and the details needed for the next decision: the caller’s stated goal, relevant answers already provided, verification status where appropriate, actions attempted, tool results or errors, and the reason for escalation. Preserve session state where the platform supports it, and test that the receiving workflow can access the information under realistic load.
Plan for unavailable staff, failed transfers, and interruptions
A transfer can fail even when the agent chooses the right trigger. Define a caller-facing outcome for an unavailable queue, timeout, unconfirmed connection, disconnected call, and failed downstream tool. Offer an appropriate next step—such as retrying, providing a callback or alternate contact route, or explaining what the agent can still do—rather than going silent.

- Supports MP3 audio playing: TF (Micro SD) card playing & USB flash drive playing. Can repeat single tune, loop all music and switch songs
- Do not announce a successful connection until the downstream system confirms it.
- Set a clear timeout and fallback for unanswered or unavailable queues.
- On tool failure, explain the problem in plain language and offer a safe retry or human assistance.
- Use idempotent operations where possible so a retry does not duplicate an action.
- Test caller interruptions mid-response, unexpected disconnects, reconnects, poor network conditions, and DTMF input where relevant.
Keep the recovery behavior safe: if the system cannot tell whether a consequential action completed, it should not silently repeat it. Route the uncertainty to a person or another verified process.
Test the full voice interaction before release
Text transcripts alone cannot establish whether the spoken experience works. Assess audio and end-to-end outcomes, including recognition, routing, tool execution, handoff, and recovery. Microsoft transparency guidance notes that real-time agents can produce errors; evaluation should therefore cover both normal calls and cases where the system is uncertain or a dependency fails.

Build a representative scenario set
- Routine supported requests and successful completion.
- Different accents, speaking styles, background noise, microphone or headset conditions, and network quality.
- Caller requests for a person, ambiguous intents, repeated misunderstandings, and out-of-scope requests.
- Failed verification, tool timeouts, unavailable human queues, and unconfirmed transfers.
- Interruptions, silence, disconnects, reconnects, and duplicate-action risks.
- Approval-required actions, including cases where the reviewer rejects or cannot approve the request.
Set a release gate
- Run complete conversations: Check whether the agent reaches the right outcome across multiple turns, not only whether individual responses sound plausible.
- Review transcripts and audio: Inspect recognition errors, pronunciation, prosody, interruptions, acoustic quality, and whether the caller understands the next step.
- Verify routing and context: Confirm each escalation reaches the intended person or queue with the right session information and approval state.
- Exercise failure paths: Confirm the caller hears the planned recovery behavior when tools, networks, or human availability fail.
- Repeat after changes: Rerun the scenario set when prompts, models, tools, integrations, or routing rules change.
- Confirm operational readiness: Check roles and identities, monitoring and retention settings, and a tested rollback path before release.
Monitor the system after launch
Track operational signals together, rather than treating a high rate of calls handled without a person as success by itself. Excessive escalation can frustrate callers; missed escalation can leave unsupported or risky requests with the agent.

- Escalation frequency and the reasons for escalation.
- Resolution and session-completion outcomes, including transfer completion.
- Tool success and error rates, latency, and time to first audio.
- Turn counts, interruptions or barge-ins, and disconnects.
- Safety signals, groundedness concerns, user reports, and reviewer feedback.
Investigate meaningful shifts in these measures, review user feedback and drift, and validate each version before release. Keep a tested rollback path so a problematic change can be reversed. Australian Government lifecycle guidance supports maintaining human oversight; operationally, that means assigning people to review the signals and act on them, not merely collecting dashboards.
Choose an implementation by operational fit
There is no neutral product ranking established by the cited implementation materials. Compare candidate platforms or designs against the controls your service needs, and test them in your actual call flow.
| Evaluation area | What to verify |
|---|---|
| Caller access and triggers | Can callers request a person, and can you configure the escalation conditions you need? |
| Context and session state | What conversation details reach the human, and is state preserved through the transfer? |
| Transfer failure | What happens when a queue is unavailable or a transfer is not confirmed? |
| Voice behavior | Can you evaluate latency, interruptions, audio quality, and the channels your service uses? |
| Integrations | Does the workflow connect appropriately to authentication, case systems, and tools? |
| Observability and evaluation | Can your team inspect sessions, tool activity, outcomes, and version-specific performance? |
| Privacy and data controls | Can you configure access, data handling, and retention to meet your requirements? |
| Versioning and recovery | Can you validate changes and restore a known-good version if needed? |
Microsoft’s voice-agent documentation describes a 2,000-millisecond default latency-threshold setting for one platform configuration and says to tune it to the conversational scenario. Treat that as a platform-specific setting, not a universal performance target. Teams should verify current platform behavior and applicable local recording, consent, and privacy requirements before deployment.
