How to Assess AI Safety Risks Before Production Deployment

A defensible AI safety review assesses the full system in its intended context—not just a model benchmark—and carries risk controls into production.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do not approve an AI model for production because it passed one benchmark or is described as “safe.” Assess the complete system in its intended use: identify who could be harmed and how, test the model and its integrations against those risks, record residual risk and approval rationale, and prepare monitoring, incident response, and rollback procedures. The right assessment depends on the model, application, users, deployment context, and applicable law.

What does it mean for an AI system to be safe enough for production?

There is no context-free safety score that settles whether an AI model is ready to deploy. A model may behave acceptably on a benchmark yet cause harm when connected to private data, given access to tools, used by a different group of people, or placed in a high-impact workflow. The object of assessment is therefore the deployed system and its use—not just the model in isolation.

“Safe enough” is a decision against risks and requirements defined for a particular deployment. Your organization needs to decide which uses are acceptable, what evidence it requires, who can approve the residual risks, and what conditions would block or limit release. No universal pass rate or threshold is established by the official guidance discussed here.

1. Define the system, use, and boundaries

Start by writing down what is actually being deployed. Include the model and version, application components, data sources, users, affected people, deployment geography, connected tools, human review, and operational dependencies. Record both the intended use and foreseeable misuse.

Make the permitted and prohibited behavior concrete. For example, if a system drafts recommendations for a human reviewer, distinguish that from allowing it to make decisions or take actions without review. Specify what the system should do when it lacks information, encounters an out-of-scope request, or cannot safely complete a task. This boundary makes later tests and release decisions meaningful.

  • Map the data flow: note what users submit, what the system retrieves or receives from other services, what it sends onward, and who can access the resulting data.
  • Map authority: list tools, permissions, and actions available to the model or application, and identify which actions require human approval.
  • Map people and context: identify direct users and others affected by outputs, along with relevant languages, environments, and foreseeable use patterns.
  • Separate risk sources: distinguish model behavior from risks introduced by the application, its integration, organizational practices, or downstream use.

2. Set ownership and a release standard

Before testing, identify the decision owner, technical and business owners, affected stakeholders, escalation route, and the evidence needed to approve, restrict, delay, or reject deployment. Agree on acceptance criteria before reviewing results. Criteria should reflect the use and the consequences of failure; a single general benchmark result is not a substitute.

NIST’s voluntary AI Risk Management Framework (AI RMF) organizes risk work into four functions: Govern, Map, Measure, and Manage. Its Playbook offers optional suggested actions. These can help structure responsibilities and evidence, but they are not a certification, a legal compliance determination, or a guarantee that a system is safe.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

3. Map plausible harms and failure modes

Build a risk register around credible ways the system could fail or be misused in its actual setting. Consider likelihood, severity, affected people, exposure, and uncertainty; prioritize plausible consequential harms instead of treating every imaginable scenario as equally likely. NIST’s AI RMF highlights multiple trustworthiness characteristics and cautions that trade-offs exist: considering each characteristic separately does not by itself make a system trustworthy.

  • Validity and reliability: could the system produce incorrect, inconsistent, or poorly supported outputs for the intended task?
  • Safety: could an output or action create physical, psychological, financial, or other foreseeable harm?
  • Security and resilience: could an attacker or failure disrupt the system, alter its behavior, or misuse its access?
  • Privacy: could personal or sensitive information be exposed, inferred, retained, or used in an unintended way?
  • Fairness and harmful bias: could performance or outcomes differ in a harmful way across relevant groups or contexts?
  • Transparency and explainability: will users and decision-makers understand the system’s role, limitations, and basis for outputs well enough to use it appropriately?
  • Accountability and human impact: can people identify who is responsible, challenge consequential outcomes, and obtain meaningful human review where needed?

For generative AI, also examine how risks arise from model design and operation, inputs and outputs, human behavior, and downstream use. The NIST Generative AI Profile provides suggested actions for managing these risks; it does not set a universal pass/fail threshold.

4. Design evaluations around the real use

Translate each prioritized harm into an evaluation question. Define representative tasks, user groups, languages, contexts, and edge cases, then select measures that can reveal the failure at issue. For example, a test intended to detect privacy leakage should examine whether sensitive information appears in outputs; a general quality score alone would not answer that question.

Set acceptance criteria before examining results, document test setup and limitations, and record uncertainty. A result is only as useful as its coverage: state which users, inputs, languages, tools, and operating conditions were included, and which were not. Do not treat a strong general benchmark result as evidence that a particular workflow is safe.

5. Use a layered testing plan

NIST’s AI Research and Assessment (ARIA) program identifies model testing, red-teaming, and field testing as three evaluation levels. They answer different questions and are most informative when used in a way that fits the system and its risks.

Evaluation level What is examined Useful for Important limitation
Model testing The model’s behavior on defined tasks and test inputs. Measuring task-specific performance and probing known classes of failure under controlled conditions. Does not by itself show how the integrated application, users, tools, or operating environment will change outcomes.
Red-teaming Attempts to elicit harmful or policy-violating behavior through adversarial testing. Finding weaknesses that ordinary expected-use tests may miss, including risks involving inputs and system behavior. Findings depend on test scope and coverage; not finding a failure is not proof that none exists.
Field testing The system in a realistic or operational setting. Observing how the integrated system performs with real workflows, users, and conditions. Requires appropriate safeguards and oversight; results from one setting may not generalize to another.

For each level, document the version tested, configuration, test cases or approach, results, and how findings map to predeclared criteria. Make tests repeatable where practical so that changes can be compared over time.

6. Test the integrated application, not only the model

Evaluate the full path from input to outcome: prompts, retrieval or other data sources, tools, permissions, filters, user interface, human review, and operational dependencies. In a generative system, test inputs and outputs, and examine how users and connected components can alter behavior. A model that behaves as expected in isolation may act differently when the surrounding application supplies context, grants capabilities, or presents its output as authoritative.

Include ordinary use as well as edge cases and adversarial scenarios tied to the harm map. Check whether safeguards work at the point where they matter—for example, whether a tool restriction actually prevents an unauthorized action, or whether human review occurs before a consequential step. Where field testing is appropriate, choose a controlled setting and safeguards suited to the potential impact rather than treating a limited trial as proof of general safety.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

7. Mitigate failures and test the changed system again

Choose a mitigation that addresses the identified failure mode. Options may include narrowing permitted use, reducing privileges, protecting data, adding appropriate human review, improving safeguards, or constraining access to tools. If a risk cannot be acceptably reduced, delay or decline deployment.

Retest after a mitigation. A control can fail, leave a path open, or create a new trade-off, so evaluate the resulting system rather than assuming the change worked. Preserve the link between the finding, the mitigation, the retest evidence, and the remaining risk.

8. Make a documented release decision

Before release, assemble a decision record that a responsible reviewer can use to understand what was assessed and why the deployment was approved, limited, or rejected. It should include:

  • the system boundary, intended use, users, and affected people;
  • the prioritized harms and the acceptance criteria set for them;
  • test configurations, results, coverage, limitations, and uncertainty;
  • known limitations, unresolved risks, chosen mitigations, and retest results;
  • the named decision owner, approval rationale, and any use restrictions.

A release decision is not the end of risk management. Define what will be monitored, who reviews incidents, how concerns are escalated, and when the system must be rolled back or disabled. Set reassessment triggers for changes to the model, prompts, data, integrations, or use. Risk management continues through deployment and use.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

9. Check legal duties separately from voluntary guidance

Determine legal applicability for the specific geography, use case, sector, and organizational role. A voluntary framework can help organize risk decisions, but following it does not establish that a deployment complies with every applicable law.

In the EU, distinguish whether an AI system is classified as high-risk under the AI Act from whether a general-purpose AI model is a model with systemic risk. These are distinct classifications with distinct obligations; a requirement scoped to one does not automatically apply to every model, system, provider, or deployer.

The European Commission’s high-risk classification page describes draft guidelines as non-binding and reports that, following a political agreement on the AI Omnibus, rules for certain high-risk areas apply from 2 December 2027, while rules for AI systems integrated into products such as robotics and industrial machinery apply from 2 August 2028. These dates and interpretations are subject to change; check current legislation and official guidance for the relevant case.

The European Commission’s AI Act Service Desk describes duties for providers of GPAI models with systemic risk, including standardized model evaluation and documented adversarial testing, assessment and mitigation of systemic risks, serious-incident tracking and reporting, and adequate cybersecurity for the model and physical infrastructure. Those stated duties are scoped to that category and should not be generalized to every production AI deployment.
