Set up a vertical mouse correctly
Start with position rather than speed. Manufacturer recommendations are ergonomic guidance, not a medical prescription.

- Bring the mouse close. Place it near enough that you do not have to reach your arm outward. Aim for a relaxed shoulder and a forearm that is roughly horizontal.
- Keep your wrist comfortable and straight. Avoid cocking it upward or sideways. Logitech advises users to keep the hand natural and relaxed and avoid unnecessary wrist extension in its MX Vertical setup guidance.
- Use a relaxed grip. Let the mouse shape support your hand; do not squeeze to keep it stable. Evoluent’s VerticalMouse support guidance describes holding that model with the fingertips while resting the edge of the hand on the desk. That grip is model-specific, not a rule for every vertical mouse. If control requires a tight grip, reassess the fit and placement.
Adjust cursor speed in small steps
Begin with your existing operating-system pointer setting or the mouse’s default. Make small changes, then check both broad screen movement and precise clicks on small targets. A setting that works for browsing may not feel right in a game, so judge it by the tasks you actually do rather than by a single sensitivity number.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsControls vary by model. Logitech documents a cursor-speed switch on the MX Vertical and a sensor adjustable up to 4000 DPI; those specifications describe that mouse, not a recommended setting for all users. The Razer Pro Click V2 Vertical Edition advertises one-click switching between work and gaming DPI settings. Profile switching can be convenient, but a DPI feature alone does not show that a vertical mouse will match a conventional gaming mouse for competitive play.

Build familiarity before testing games
Start with everyday pointing
Use routine browsing, documents, and other low-pressure cursor work to learn the grip and how far the pointer moves with your hand. Focus on comfortable, controlled movement rather than trying to move quickly from the outset.
Move to low-stakes game practice
Before ranked or competitive sessions, try game menus, camera movement, and aiming in a practice range or other low-stakes mode. Check whether the mouse stays stable during repeated clicks, whether buttons are easy to reach without tightening your grip, and whether pointer or in-game sensitivity feels controllable. Everyday cursor work and game aiming are different tasks, so comfort at the desktop does not by itself establish game performance.

What studies say about posture and pointing
In a 2015 repeated-measures study, Odell and Johnson had 12 participants perform a standardized point-and-click task using flat, angled, and vertical mouse designs. The tested concepts reduced forearm pronation compared with a traditional flat mouse; the fully vertical design also had the poorest pointing performance among the designs evaluated. The authors concluded that increasing mouse height and angling the top case could improve wrist posture without negatively affecting performance in the tested designs. These findings do not establish how every current product performs or predict results in every game. Read the study abstract on PubMed.
A separate study indexed by PubMed concluded that using a vertical mouse decreased wrist pronation and lowered wrist-extensor muscle activity, while noting that additional training and familiarization may be needed to improve performance. It supports the possibility of a learning period, not a fixed adaptation timeline or a guaranteed benefit for an individual. Read the study abstract on PubMed.

The Cleveland Clinic’s esports ergonomics guide includes an ergonomic or vertical mouse among options to consider. That makes it one possible part of a broader ergonomic setup, not a promise that a device will prevent or cure injury. See the Cleveland Clinic Gamer’s Health Guide.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Decide whether the mouse fits your work and games
Compare devices by hand fit, grip stability, control settings, button layout, and how accurately you can point and click. Logitech says the MX Vertical is intended for medium-to-large hands; its fit may not suit everyone. A slanted mouse, fully vertical mouse, and conventional mouse can each work differently for different people and tasks, and the available study does not identify a universal best design.

- Can you move and click without squeezing?
- Can you reach the buttons comfortably during repeated use?
- Can you control small targets and game-specific movement at a sensible cursor speed?
- Does the mouse track reliably on your desk surface? Evoluent advises using a non-reflective mouse pad if a reflective or unsuitable surface causes tracking issues; this is vendor troubleshooting advice, not a requirement for every setup.
Do not treat adaptation as a test of willpower. If the device remains unstable unless you grip hard, or it causes persistent discomfort, reconsider its fit, your positioning, or another pointing device. The evidence cited here does not diagnose the cause of discomfort or prescribe treatment.
