Do Vertical Mice Help With Wrist Pain—and Are They Good for Gaming?

Short answer: A vertical mouse changes how your hand and forearm are positioned, but the available evidence does not establish that it reliably relieves wrist pain or reduces carpal tunnel pressure. It may work for casual gaming, but small laboratory studies found slower pointing with the vertical models they tested, and none directly tested gaming or esports. Consider one as a comfort option to try—not a proven treatment or guaranteed gaming upgrade.

What a vertical mouse changes—and what that does not prove

A conventional mouse encourages a palm-down position. A vertical design rotates the hand toward a handshake-like grip, which can reduce forearm pronation (rotation of the forearm so the palm faces down). That is a measurable posture change, not proof that pain will improve or that an injury will be prevented.

In one laboratory comparison, the vertical mouse was associated with 28° of pronation versus 42° with a standard mouse, and 13% versus 16% extensor carpi activity. The same study recorded a time to target of 4.2 seconds with its vertical mouse versus 3.4 seconds with the standard mouse. These are measurements from that study’s setup, not results every user should expect. The study abstract describes less wrist pronation and lower wrist extensor muscle activity when using its vertical mouse.

Does a vertical mouse relieve wrist pain or help carpal tunnel syndrome?

There is not enough evidence here to say that vertical mice reliably relieve wrist pain. In a study of 21 people with carpal tunnel syndrome, a vertical mouse significantly reduced ulnar deviation, meaning the wrist bent less toward the little-finger side. However, the tested ergonomic devices did not reduce measured carpal tunnel pressure. The authors said the findings did not support a strong recommendation for or against the devices and concluded that selection remained a matter of personal preference. Those findings concern a measured posture and pressure in a small clinical study; they do not diagnose an individual’s symptoms or establish whether a particular mouse will help. If wrist or hand symptoms persist or interfere with daily activities, seek advice from a qualified health professional rather than relying on a mouse change as treatment.

Are vertical mice good for gaming?

They may be usable for casual games, but the cited evidence cannot establish whether vertical mice are good for gaming overall. The studies tested laboratory pointing tasks, not game aim, latency, match outcomes, or esports performance.

In a 12-person evaluation of flat, angled, and vertical mice, the fully vertical design reduced pronation but had the poorest pointing performance in the task. The researchers reported that raising a mouse and angling its topcase could improve wrist posture without a measured performance penalty in that task. See the study’s abstract. Together with the slower target time in the other laboratory comparison, this suggests a possible speed or precision trade-off—but not a proven gaming disadvantage.

If fast, precise aiming matters to you, test the mouse in the games you actually play and compare it with your usual mouse or an angled ergonomic design. Give yourself time to adjust, then judge whether your control and comfort are acceptable. No direct gaming-latency comparison is established by these studies.

How to decide whether to try one

  • Grip and angle: Vertical designs differ in how far the hand is rotated. Comfort depends on the particular angle and how you hold the mouse.
  • Fit and handedness: Check whether the mouse suits your hand size and dominant hand. A model that feels awkward to grip may not be a useful alternative.
  • Tracking and controls: Consider adjustable sensitivity, button placement, weight, and whether the controls suit your work or games.
  • Connection: Check whether it uses a cable, wireless receiver, Bluetooth, or more than one option. The cited evidence does not establish a gaming-latency advantage for any vertical model.
  • Real-use trial: Try ordinary pointing as well as your usual games, and assess both control and comfort. Check the seller’s trial or return terms before buying.
