Skyrim remembers choices through quest-specific states and conditions, not through a documented universal choice log. A quest can advance to an authored stage, update its journal or objectives, and use that state to change which dialogue or behavior is available later. Exactly what gets recorded and what follows depends on how that quest was built.
How does Skyrim remember my choices?
A useful way to understand Skyrim’s quest system is as a set of connected checks and actions: an event or choice may satisfy a condition; quest logic may then advance a stage, run an associated result, or update an objective or journal entry. Later dialogue can check the resulting state. This is a general model of the documented Creation Kit authoring tools, not a sequence every quest necessarily uses.

The Conditions System documentation describes conditions as a way to branch quests, storylines, and dialogue. It says conditions can determine whether a dialogue line plays or a quest is added, including by checking whether a quest stage has been completed. Read the Conditions System documentation.

Quest stages are authored progress points, not universal percentages or a complete record of everything the player chose. Quest creators decide which stages exist and what happens at each one. A stage can have journal text, conditions, and results associated with it; advancing to one may change what the player sees and trigger quest behavior. The Arcane University quest implementation guide describes stages, journal updates, objectives, aliases, and branching as parts of quest structure.

A completion setting can be associated with a stage, so quest completion is an authored state change. The Creation Kit’s Quest Data tab reference also notes that a completed quest can still run. Completion therefore does not, by itself, mean every related process is permanently frozen.
How earlier actions can affect later dialogue
Dialogue can be gated by quest state. For example, a later line can be available only after a particular stage is complete. Dialogue choices can also lead to quest changes, and those changes can affect later conversations. The specific branch and consequence depend on the content; the documentation supports this mechanism in general, but does not establish the exact flags or scripts behind any unexamined vanilla quest.

That distinction helps explain why an NPC may appear to know what happened earlier: later dialogue or behavior can check a state created by previous quest progression. It does not establish that every spoken response is saved forever or that the NPC consults a complete transcript of the player’s conversations.
What can advance a quest besides dialogue?
Quest progression can respond to more than a menu choice. The quest implementation tutorial describes reactions to items being picked up, targets dying, books being read, and hand-placed triggers or activators. Quest aliases provide named roles for actors, places, or items, helping quest content refer to the relevant elements.

More complex world reactions can use the Story Manager, aliases, and scenes. The Arcane University World Interactions tutorial gives a vanilla example involving the WICommentMagicFlames quest and a GameDaysPassed condition that controls when an NPC comment can occur. It illustrates one authored interaction; it does not show that every comment or choice is stored in the same way.
Does Skyrim track completed quests?
Quest content can represent progress through stages and mark a stage as completing the quest. The journal and objectives provide visible progress information, while quest conditions can make later content depend on earlier progression. The available Creation Kit references describe those authoring mechanisms; they are not a comprehensive specification of every base-game or expansion quest.

Is there a complete choice log?
The Creation Kit references describe how quest states and conditions can be authored, but they do not document Skyrim’s saved-game format or prove that the game maintains one central ledger of every player decision. The journal is a visible record of quest progress, not evidence that every consequential choice appears there. How any particular choice is persisted beyond the documented quest-state mechanisms is not established by these references.
