Researchers identify a neuroinflammation drug target by combining human, molecular and disease-model evidence to nominate a plausible intervention point. They validate it by deliberately changing that target and testing whether the change produces the expected biological and disease-relevant effects. A signal in diseased tissue is a starting clue—not proof that treating the target will help people.
What target identification and validation mean
A drug target is a biological molecule or process that an intervention is intended to change. In neuroinflammation, the candidate might be linked to immune activity in the central nervous system (CNS), but the label alone says little about whether changing it will be safe or useful. The answer depends on which disease, patient group, disease stage, tissue compartment, cell type and biological function are involved.

Identification nominates a candidate: evidence suggests that the target may matter. Validation tests a specific therapeutic hypothesis: if researchers change the target in a defined direction, does relevant disease biology change in a way that could plausibly benefit patients? Detecting a target, finding more of it in diseased tissue, or associating it with disease is not enough to establish that intervention will work. The 2018 Genome Medicine review, “The druggable genome and support for target identification and validation in drug development,” makes this distinction central to the drug-development process.
“Neuroinflammation” is not one disease or one uniform cell state. A candidate can be relevant in one condition, compartment or phase and irrelevant—or harmful—in another. Researchers therefore need to define the intended intervention before interpreting a promising signal.

Start by defining the therapeutic question
Before searching for a target, researchers specify what problem a treatment is supposed to solve. That means more than naming a disease. It includes the intended direction and degree of target modulation, where and when the intervention should act, and which outcome would count as a meaningful effect.
- Disease and stage: Which neuroinflammatory disease and phase are under study?
- Compartment and cell: Is the relevant process peripheral, within the CNS, or both? Which cell type and biological state are implicated?
- Direction of intervention: Should the target be inhibited, increased or otherwise modulated—and what evidence supports that direction?
- Outcome: Which molecular, cellular or disease-relevant function should change if the hypothesis is correct?
- Patient relevance: Which subgroup might benefit, and at what disease stage?
Multiple sclerosis (MS) illustrates why this framing matters. Relapse-associated disease activity is linked to peripheral inflammatory mechanisms, while progression can involve inflammation compartmentalized within the CNS. A 2026 review by Alica Blenkle, Anastasia Geladaris and Martin S. Weber notes that “Disease progression is probably driven by a combination of several mechanisms rather than a single factor.” A target proposed for one process should not automatically be treated as a universal explanation or solution for MS.
How researchers nominate candidates
Candidate identification is strongest when different kinds of evidence converge on the same biological hypothesis. Each evidence type answers a different question; none, in isolation, proves that a drug aimed at the candidate will improve disease.
Human genetics and clinical evidence
Human genetic findings can help prioritize targets when researchers can map a disease-associated signal to the causal gene and establish the direction in which its activity is likely to affect risk. Clinical observations may add context about disease course or relevant outcomes. But a genetic association is not automatically a target: the causal gene, mechanism and direction of effect may be uncertain.

Inherited variation is also not the same exposure as a medicine. A genetic effect may be present throughout life, operate in particular tissues or populations, and differ in magnitude or timing from a drug given after disease begins. The 2013 review “Validating therapeutic targets through human genetics,” by Robert M. Plenge, Edward M. Scolnick and David Altshuler, argues for using genetics to inform therapeutic hypotheses while testing their limitations rather than treating genetic support as a guarantee of clinical success.
Disease tissue and molecular data
Researchers examine gene or protein expression, cell-type-resolved measurements, disease tissue and pathway relationships to ask where a candidate appears and what processes may involve it. These data can reveal that a pathway or cell population differs in disease. However, a difference can be a consequence of disease, a response to injury, or a feature that does not cause the outcome researchers hope to change. Differential expression and tissue association nominate questions; they do not establish intervention effects.
Cell states, not just cell labels
Single-cell and single-nucleus transcriptomics, chromatin-accessibility methods and spatial multi-omics can help place signals in particular cells, states and locations. This matters especially for microglia, whose states vary with factors including age, brain region, sex, genotype, environment and prior exposures. The 2026 review “Microglial states revisited: from homeostasis to disease,” by B. J. L. Eggen and S. M. Kooistra, emphasizes this context-dependent picture.

Consequently, “activated microglia” is too broad to serve as a complete mechanistic explanation. The older M1/M2 shorthand is also not a full account of microglial biology. Microglia participate in homeostasis, immune defense, repair and disease-associated functions; a state associated with pathology may coexist with functions that should be preserved. Researchers need to ask which state, location and function are relevant in the particular disease setting.

Pathways and perturbation signatures
Pathway knowledge and signatures produced by perturbing biological systems can help connect a candidate to a mechanism. They can suggest what downstream changes to measure and whether independent evidence points toward a consistent direction of effect. A pathway diagram or signature remains a hypothesis, though: researchers must test whether changing the proposed target actually causes the predicted consequences in relevant systems.
How validation tests causality
Validation moves from observing a candidate to intervening on it. A useful test defines what was changed, in which direction, in which context, and which predicted outcomes followed. Researchers look for a linked chain of evidence: the intervention reaches and changes the target, downstream biology responds as predicted, and a relevant function or endpoint changes.
- Specify the hypothesis. State the disease context, target cell or compartment, desired direction of modulation, proposed mechanism and disease-relevant outcome.
- Choose complementary perturbations. Where feasible, use genetic perturbation and pharmacological modulation to test the target through different means. Agreement across orthogonal approaches is more informative than repeating one method alone.
- Use suitable controls and replication. Include appropriate controls and seek replication. Rescue experiments—testing whether restoring the target or its function reverses an effect—can help distinguish a target-linked effect from unrelated consequences of the perturbation.
- Measure target engagement. Establish that the intervention changed the intended target in the relevant system. Without evidence of engagement, a negative downstream result may not test the hypothesis adequately.
- Measure the proposed mechanism and function. Assess downstream biology and a function or endpoint relevant to the disease question, not just target abundance or a convenient molecular marker.
- Test in complementary contexts. Repeat the reasoning across suitable models and compare the results with human evidence, while being explicit about what each system can and cannot represent.
A finding that an intervention shifts a marker is not automatically evidence of therapeutic value. The marker must relate to the proposed mechanism, and the mechanism must connect to a disease-relevant function. Similarly, changing an outcome in one experimental system does not establish benefit in people. Validation is cumulative: each experiment narrows uncertainty about a defined hypothesis.
Use genetics as one part of the evidence, not a shortcut
Mendelian randomization and related analyses use genetic variation as a proxy for perturbing a biological exposure or target. They can help assess whether naturally occurring differences are consistent with a causal effect and can inform dose-response or safety hypotheses. The 2021 article “Mendelian randomization for studying the effects of perturbing drug targets” discusses this use of genetic evidence.

Interpretation depends on whether the genetic instruments are valid and sufficiently informative, whether effects operate through the proposed target rather than other pathways, and whether the tissue, ancestry and population context fit the therapeutic question. Pleiotropy, weak instruments and tissue specificity can complicate conclusions. Even a convincing inherited effect may not predict the impact of a treatment started at a particular disease stage. Mendelian randomization therefore contributes to triangulation; it does not replace experimental perturbation or clinical trials.
Match each model to the question it can answer
Models are tools for testing particular parts of a therapeutic hypothesis, not interchangeable miniature versions of human disease. The best choice depends on whether the question concerns a cell mechanism, a multicellular interaction or an organism-level outcome.
| Evidence system | What it can help test | Important limitation |
|---|---|---|
| Human disease tissue and clinical evidence | Whether a candidate, cell state or pathway is relevant in human disease. | Association in people does not by itself show that intervening on the candidate changes disease. |
| Human genetics | Whether naturally occurring perturbations support a causal hypothesis and its likely direction. | Variant mapping, pleiotropy, tissue and population context, and lifelong exposure complicate translation to a timed drug intervention. |
| Animal models | Organism-level mechanisms and intervention effects in an intact system. | Species differences, model fidelity, design and endpoint choices can limit prediction of human disease. |
| Human iPSC-derived microglia and multicellular models | Human cell biology and, in suitable systems, high-throughput perturbation of microglia-related mechanisms. | In vitro systems cannot fully reproduce the CNS environment or the full complexity of MS progression. |
The 2026 review by Blenkle, Geladaris and Weber considers human in vitro models in the context of microglia-associated MS progression, while cautioning against treating any one model as the whole disease. Animal models can supply organism-level tests; human-derived systems offer access to relevant human cellular biology. Replication across complementary systems and alignment with human evidence make a stronger case than relying on one model class.
Compare candidates on evidence and therapeutic feasibility
When several targets look plausible, researchers need a structured comparison rather than a single “best target” score. The following questions expose differences in both causal support and the feasibility of testing or treating the target. They are a decision framework, not a validated numerical scoring system.
Best Value
- Causal strength: Does evidence go beyond association, and is the causal gene or mechanism clear?
- Cell and tissue specificity: Is the candidate present in the relevant cell state and compartment, or does the evidence point to broader activity?
- Stage and timing: Does the evidence fit the disease phase when treatment is intended to act?
- Direction and degree: Does the evidence indicate whether to increase or reduce activity, and what degree of modulation is being tested?
- Reproducibility: Do independent methods and complementary models support the same mechanism?
- Engagement and endpoints: Can researchers measure target engagement, downstream pharmacodynamics and a function relevant to disease?
- Safety and beneficial functions: Could modulation impair useful roles of the target, such as immune defense, homeostasis or repair?
- CNS access: Can the proposed intervention reach the CNS compartment where the target is meant to act?
For genetic evidence, add questions about instrument validity, pleiotropy, ancestry, tissue context and whether lifelong variation is informative about treatment at the proposed time. A target may be biologically compelling yet difficult to engage in the relevant compartment; another may be accessible but supported mainly by correlation. Separating these issues keeps “interesting biology” from being mistaken for “therapeutically tractable.”
Plan translation before a clinical test
A translational plan should specify how researchers will tell whether the intervention reaches and engages its target, what downstream pharmacodynamic changes would support the mechanism, and which patient group, disease stage and clinical outcome are being tested. For a CNS-directed hypothesis, access to the relevant CNS compartment is part of the target’s practical feasibility, not an afterthought.
There is no single target-engagement threshold established across neuroinflammatory diseases and target classes. The required measure depends on the target and intervention. Researchers should define the engagement and downstream readouts for the particular hypothesis instead of presenting one universal biomarker cutoff as standard. Clinical evidence is ultimately needed to determine whether a carefully specified intervention benefits patients.

Why promising targets still fail
Preclinical findings can be compelling without predicting clinical benefit. In their 2013 review, Plenge, Scolnick and Altshuler wrote: “Existing preclinical models have a limited ability to test ‘therapeutic hypotheses’”. They attributed many drug-development failures to limited predictive value of preclinical models and uncertainty about the long-term consequences of perturbing targets in humans.
The scale of the challenge is illustrated by two broad estimates that use different denominators. Plenge, Scolnick and Altshuler’s 2013 review reported that more than 90% of compounds entering clinical trials failed to demonstrate sufficient safety and efficacy for regulatory approval. A 2018 Genome Medicine article reported an estimate that 4% of drug-development programs yield licensed drugs. These are historical, broad drug-development figures—not neuroinflammation-specific target-validation rates—and the different units mean the percentages should not be compared directly.
A rigorous target case cannot eliminate uncertainty, but it can make the uncertainty visible: distinguish human association from causality, verify the direction of intervention, test engagement and mechanism, use models for the questions they can answer, and match the eventual clinical test to the disease context. That is what turns a promising signal into a testable therapeutic hypothesis.
