The right antibiotic and route depend on the form and severity of illness and the patient’s circumstances. Anyone with possible plague symptoms or a significant exposure should seek emergency assessment immediately; do not try to diagnose or treat it yourself.
How is plague treated?
Plague is a serious bacterial infection caused by Yersinia pestis. Clinicians treat it with antibiotics and supportive care. The U.S. Centers for Disease Control and Prevention (CDC) identifies gentamicin and fluoroquinolones as first-line treatments in the United States. The choice depends on the clinical form, severity, age, pregnancy status, allergies, medical history, and ability to take or absorb oral medicine. [CDC clinical care guidance]

CDC recommendations distinguish among bubonic or pharyngeal plague, pneumonic or septicemic plague, and meningeal disease. Options listed for pneumonic or septicemic plague include ciprofloxacin, levofloxacin, moxifloxacin, gentamicin, and streptomycin. Doxycycline is also listed as a first-line option for bubonic or pharyngeal disease. Other possible antibiotics include tetracyclines, chloramphenicol, and trimethoprim-sulfamethoxazole in selected circumstances. Some options are FDA-approved for plague; others may be used off-label under clinical guidance. These medicines are not interchangeable, and a clinician must select the treatment. Treatment may be given intravenously (IV), intramuscularly (IM), or by mouth. Severe illness may call for treatment by injection or IV; the route and any transition to oral treatment are clinical decisions. Streptomycin is not widely available in the United States. CDC dosing tables are intended for clinicians, not for creating a self-treatment regimen. [CDC clinical care guidance] [CDC treatment recommendations, 2021]
#1 Best Overall
- Spring Assisted Open Design. We use the latest design on this weekly pill organizer to achieve one-handed operation with the little rubber strips for the smart "lid spring open" feature. The lids are all beautifully colored and marked clearly with the date, everything is clear at a glance so you may find it easy to choose the pills you need. It's quite nice for easy daily dispensing and big enough to hold 8 fish oils or 12 large vitamins for each dose (size of each compartment: 1.3"x1.18"x0.82")
- Say Goodbye to Dumpage Pills. No worries about unwanted dumpage of pills found by pets and kids for your slightest unintentional or incidental touch on the medicine organizer. The push button stays closed once locked and securely snap shut keeping pills safe. It's small and perfectly suitable for travel and is ideal for storing small jewelry or items such as earrings, beads, nail art tips, ect.
- Premium Screen Printing. To avoid screen-printing fonts and icons falling off from the surface of the pill box, we use high-end screen printing ink and strictly do the Ink Adhesion Test with 3M Tapes before shipment. Thus, no need to be frustrated with the missing fonts of "Tuesday" or icons of "PM", and worry about mistakenly taking the wrong tablets.
- BPA-Free Material. This pill case is pretty safe because of being made of food grade materials. You would be worry-free with the BPA-free pill boxes and organizer when storing multiple pill regimes inside. With the transparent plastic lid, it's quite easy to distinguish the pills in each dose without opening the lids.
- What You Receive: 1 x AUVON iMedassist Pill Holder, our 12-month warranty, and friendly customer service.
How long does treatment last?
A typical antibiotic course is 10–14 days. Clinicians may extend it if fever or other concerning signs or symptoms continue. [CDC clinical care guidance] [CDC treatment recommendations, 2021]
Should treatment wait for test results?
No. CDC says to begin appropriate therapy as soon as plague is suspected and never to delay or withhold treatment while waiting for laboratory results. If possible, clinicians collect specimens before starting antibiotics, but specimen collection must not postpone treatment. Symptoms, examination, and exposure history help inform the decision. Suspected cases also require prompt notification of local and state health departments. [CDC clinical care guidance] [CDC diagnosis and testing guidance]

For a person who may have plague, the practical response is immediate emergency medical assessment—not waiting to see whether symptoms improve or trying to obtain antibiotics independently. [CDC clinical care guidance]
How effective are antibiotics for plague?
Antibiotics are effective, and prompt treatment is critical to saving lives. CDC’s 2021 treatment guideline describes human evidence that supports treatment but does not establish one universally best antibiotic. Plague is rare, comparative patient groups are small, and much of the evidence comes from case reports and case series rather than large head-to-head trials. Recommendations also draw on animal studies, which cannot fully reproduce human experience. [CDC treatment recommendations, 2021]

Two reported outcomes illustrate both the potential for successful treatment and why results should not be treated as a universal cure rate:
- Aminoglycosides: 61 of 73 reported U.S. plague patients (84%) survived after aminoglycoside monotherapy in the cases summarized by CDC in 2021. This was an observational collection, not a randomized comparison with another antibiotic. [CDC treatment recommendations, 2021]
- Oral doxycycline for bubonic plague: 29 of 30 patients (97%) survived in a Tanzanian randomized clinical trial described by CDC in 2021. The one patient who died had advanced disease and serious complications when treatment began. That result concerns the trial’s population and bubonic plague; it does not show doxycycline is best for every form of plague. [CDC treatment recommendations, 2021]
These figures describe particular patient groups and circumstances. They should not be read as an individual prognosis or as proof that one drug works equally well across disease forms. CDC’s central conclusion is that early recognition and effective antimicrobial treatment are paramount to saving lives. [CDC treatment recommendations, 2021]

Who should receive preventive antibiotics after exposure?
Preventive antibiotics are not recommended simply because someone is worried about plague. CDC recommends seven days of post-exposure prophylaxis for specified significant exposures, including close, sustained contact with a person who has pneumonic plague or direct contact with infected fluids or tissues. A public-health or medical professional should assess the exposure and determine what to do. [CDC clinical care guidance] [CDC treatment recommendations, 2021]
