Endovascular robots translate a surgeon’s input into catheter or guidewire movement through a teleoperated control loop: the surgeon operates a console, control software sends commands to a bedside drive, and that drive advances, retracts, or rotates the instrument. The robot mediates the motion rather than copying a hand directly onto a catheter. The surgeon remains in control; how closely the instrument follows, and what feedback the surgeon receives, depends on the system.
How the control chain works
The basic arrangement is a master–slave system. The surgeon’s console is the master; the bedside mechanism that holds and moves the clinical instrument is the slave. A controller maps input at the console into commands for the instrument drive.

- Input: The surgeon moves a joystick or another control at the console. The interface may not resemble the hand movements used in manual catheter work.
- Command mapping: The master-side controller interprets the input and sends corresponding signals to the slave-side controller. The system maps them to instrument actions such as axial advance or retraction and rotation.
- Instrument movement: A bedside drive grips and moves the catheter, guidewire, or other supported instrument. The 2023 technical review describes CorPath GRX as using a friction wheel together with a rotary wheel to provide two-degree-of-freedom guidewire and catheter movement (2023 technical review).
- Observation and adjustment: The surgeon observes the procedure through imaging and adjusts commands as needed. Feedback features vary by system; the operator remains part of the control loop.
In short, the system converts console input into commands, then uses a mechanical drive to move the instrument. It does not directly transmit a surgeon’s hand movement through to the catheter.
What the surgeon controls on a commercial system
CorPath GRX console and signal path
A clinical registry supplement describes a CorPath GRX console with a touchscreen and three joysticks: one each for balloon or stent manipulation, guidewire manipulation, and guide-catheter manipulation. The joysticks send signals over a communication cable to the robotic drive, which operates the cassette holding the instruments (PRECISION Registry data supplement, 2024). This documented layout illustrates why robotic control should not be assumed to feel or look like ordinary hand manipulation.

Regulatory record is product-specific
The FDA’s 510(k) database lists the CorPath GRX System as a “System, Catheter Control, Steerable” and records a substantial-equivalence decision dated March 1, 2018 (FDA 510(k) K173806). That record concerns this device; it does not establish clearance, supported uses, or capabilities for every endovascular robot.
Why the instrument may not follow input perfectly
A command and the resulting physical movement are not always identical. Friction between the tool and tissue, communication delay, backlash, hysteresis, and other nonlinear effects can cause lag or trajectory error. The 2023 review identifies precision, rapid response, tremor reduction, and safety monitoring as important control goals, while noting the challenges these disturbances create (2023 technical review).

Motion scaling or tremor filtering should not be assumed to work the same way across platforms. Those are design choices, and the cited material does not establish one universal mapping ratio or filtering behavior for endovascular robots as a class.
Does the surgeon feel catheter forces?
There is no basis here for treating force or haptic feedback as a standard feature across systems. Imaging and other available system feedback help the surgeon monitor the procedure, but whether a robot senses instrument forces and reproduces them at the console depends on its design.

For example, a 2022 experimental study proposed a system with magnetically controlled haptic force feedback. Its authors reported an average translation-tracking error of 0.94 mm and an average rotation error of 0.89 degrees in experiments with their developed system (2022 ECRS study). These are results for that experimental platform, not general specifications for CorPath GRX or other endovascular robots.
What changes in a neurovascular configuration
A 2024 clinical article on CorPath GRX use in cerebral aneurysm embolization describes adaptations that included active device fixation, accommodation of smaller devices, a longer working length, workflow changes, and a software cap of 6 mm/s for linear guidewire or device movement (2024 clinical article). The 6 mm/s figure applies to the configuration reported in that article; it is not a universal speed limit for endovascular robotics.

Are endovascular robots autonomous?
The systems described here are teleoperated: the surgeon provides input and controls instrument movement through the console and robotic drive. Software interprets and relays commands, but that does not make the procedure autonomous. In September 2026, the FDA issued draft guidance describing robotically assisted surgical devices as teleoperated, software-controlled systems designed to assist qualified practitioners in positioning and controlling instruments. The guidance is explicitly a draft for comment, is nonbinding, and is not for implementation (FDA draft guidance, September 2026).
