Crew watches keep the vessel operating around the clock. Food, which must be carried onboard, is a major limit on patrol length in the U.S. Navy and Royal Navy examples described here; the details vary by submarine class and mission.
How does a nuclear submarine make power underwater?
A reactor does not push the submarine directly. It produces heat, which is used in a steam-power system. In the U.S. Navy’s description, pressurized water in a closed primary loop is heated by the reactor. That heat passes through steam generators to water in a separate secondary loop, where steam is produced. The steam drives the propulsion turbines and turbine generators; after use, it is condensed and returned as water for reuse. The two loops remain separate, so their water does not mix. This power-generation process does not require atmospheric air or oxygen. That is why a nuclear submarine does not have to surface to breathe for its reactor-powered propulsion and electrical systems to keep working. It is a different matter from how the crew breathes: people onboard still need a supply of breathable air.
The Royal Navy gives a class-specific example: the Vanguard-class PWR 2 reactor converts water into steam to drive the engines and generate electricity. Submarine designs differ, so that example should not be taken as a description of every nuclear boat. Royal Navy: Vanguard class

How are breathable air and drinking water replenished?
Submarines can produce oxygen and purified water from seawater, while air-treatment equipment filters unwanted contaminants. The U.S. Navy describes these functions at a general level; Royal Navy pages also describe oxygen and drinking-water production on Astute-class submarines and onboard water and oxygen generation for HMS Vanguard. U.S. Navy overview; Royal Navy: Astute class; Royal Navy: Vanguard class
In practical terms, these systems help replenish breathable oxygen, remove unwanted substances from the atmosphere, and turn seawater into usable fresh water. The precise equipment and arrangements are not established as identical across all submarine classes. The Naval History and Heritage Command likewise describes modern submarines as making fresh air and water, filtering toxic vapors, and distilling seawater. Why is food a major limit on patrol length?
Oxygen and water can be replenished onboard, but food has to be stored in advance. The U.S. Navy says that, once the submarine can produce oxygen and purified water, patrol duration is limited primarily by the food it can carry. Royal Navy descriptions of the Astute and Vanguard classes make a similar point about food limiting range. These are statements about the cited official examples, not a universal patrol-duration figure for every submarine. U.S. Navy overview; Royal Navy: Astute class; Royal Navy: Vanguard class
A provisioning example from a Royal Navy patrol
In a report published on 15 April 2019, the Royal Navy described the challenge of supplying meals and snacks for about 140 people aboard HMS Vengeance, a Vanguard-class submarine, for months. The account notes that fresh produce runs out and describes no replenishment at sea or port top-up for the team during the patrol discussed. The figure is an example from that report—not a standard crew size or a provisioning formula for all submarines. Royal Navy catering report, 15 April 2019

How does the crew keep the submarine operating continuously?
Submariners work in rotating watches so essential duties continue while individuals rest. The Royal Navy says two rotating watches allow Astute-class submarines to operate continuously, 24 hours a day, seven days a week. Its Vanguard-class description also says two crews take turns on duty, with the other crew on leave or training at base. These are class-specific descriptions of crew arrangements, not a claim that every fleet uses the same system. Royal Navy: Astute class; Royal Navy: Vanguard class

Reactor refuelling and food resupply are different constraints. For HMS Ambush, the Royal Navy says the reactor is not expected to need refuelling during the vessel’s 25-year service period. The same page says the boat purifies its own water and air and that its range is limited by onboard food. The 25-year statement is specific to HMS Ambush as described on that page; it is not a patrol-length estimate or a general figure for every nuclear submarine. The page does not state a publication date. Royal Navy: HMS Ambush
