How Do PagedAttention and RadixAttention Differ in LLM Serving?

PagedAttention manages KV-cache organization; RadixAttention optimizes reuse. They solve related but distinct serving problems, and published headline results are not a head-to-head comparison.

PagedAttention and RadixAttention address different parts of LLM serving, so neither is a universal alternative to the other. PagedAttention organizes per-request key-value (KV) cache into fixed-token blocks; RadixAttention is a runtime optimization for reusing KV cache. Their roles can be complementary, and the cited papers do not provide a controlled head-to-head test that establishes a winner.

Why KV-cache management matters

During autoregressive generation, a model keeps key and value tensors from earlier tokens so it can use them again for subsequent tokens. This KV cache grows as requests progress and can consume substantial GPU memory. The PagedAttention paper identifies inefficient handling—especially fragmentation and redundant duplication—as a serving-system problem that can limit how effectively memory is used. What PagedAttention does

Fixed-token blocks organize cache storage

PagedAttention applies a paging-inspired approach to attention and KV-cache management. Instead of requiring each request’s cache to occupy one contiguous allocation, the cache is organized in blocks, each holding a fixed number of tokens. This makes allocation and access block-based and helps address memory-management inefficiencies as requests grow.

The official vLLM Paged Attention documentation describes the paged-cache blocks and notes that they are not GPU thread blocks. The same page marks its kernel walkthrough as historical; it should not be read as a complete description of current vLLM implementation internals.

What the original performance figure means

In its 2023 paper, the PagedAttention authors reported that vLLM achieved 2–4× the throughput of the systems they evaluated at the same latency. That is a result for the paper’s systems and test conditions, not a measurement of PagedAttention against RadixAttention. The paper’s abstract and evaluation scope are available in the PagedAttention paper.

What RadixAttention does

Runtime KV-cache reuse

The SGLang paper presents RadixAttention as a runtime optimization for KV-cache reuse in a system designed to execute structured language-model programs. Its purpose, as described in the paper, is to reuse cached KV state where execution offers an opportunity to do so. That is a different primary role from organizing a request’s cache into fixed-token blocks.

The paper also describes other runtime optimizations, including compressed finite state machines for faster structured output decoding. Its abstract reports up to 6.4× higher throughput than the state-of-the-art inference systems evaluated on the paper’s tasks and models. This is an overall SGLang system result; the abstract does not isolate RadixAttention’s contribution or make it a direct comparison with vLLM. See the SGLang paper (submitted in 2023 and revised in 2024).

How the approaches compare

Comparison PagedAttention RadixAttention
Primary role Block-based organization and access for KV cache Runtime reuse of KV cache
Problem emphasized Managing dynamically growing cache efficiently, including fragmentation and redundant duplication Taking advantage of opportunities to reuse KV state
Published headline result vLLM reported 2–4× throughput at the same latency versus systems evaluated in the 2023 paper SGLang reported up to 6.4× higher throughput versus evaluated inference systems; the result is for the overall system, not RadixAttention alone
Direct comparison between the techniques? No matched head-to-head benchmark established in the cited papers No matched head-to-head benchmark established in the cited papers

The mechanisms are not inherently mutually exclusive: one concerns cache representation and access, while the other concerns reuse. The cited material does not establish whether, or how, particular implementations combine them, so treat this distinction as one of roles rather than a claim about every system’s architecture.

Which matters more for a workload?

Consider cache organization and memory pressure

If the practical concern is how growing per-request KV state is allocated and accessed, block-based cache organization is the relevant PagedAttention concept. The PagedAttention paper motivates its approach around memory use and serving efficiency; it does not supply a universal threshold at which it becomes preferable to a reuse optimization.

Consider repeated or shared context

If requests or execution paths offer repeated context whose KV state can be reused, RadixAttention’s stated purpose is pertinent. How much benefit a workload receives depends on the reuse opportunities it actually presents. The SGLang abstract does not establish a general break-even point or a RadixAttention-only performance gain.

How to interpret the headline numbers

The 2–4× and up-to-6.4× figures come from separate papers, with different systems and evaluation contexts. They are not a fair numerical ranking: the available results do not hold hardware, model, workload, software versions, and latency constraints constant while changing only the cache technique. Use each number only to understand what its authors reported in their own evaluation, not to predict which approach will be faster in a particular deployment.

How to choose in practice

For an engineering decision, compare complete serving configurations on the workload you need to support rather than choosing by technique name alone. Keep the model, hardware, prompt and generation patterns, concurrency, software versions, and latency target consistent between runs. Measure throughput and latency alongside memory use, and include cases with and without meaningful shared-prefix or repeated-context opportunities. Such a test can answer a deployment-specific question; the cited papers alone cannot declare a universal winner.
