Doctors diagnose cancer in newborns and infants by combining the child’s health history and examination with tests chosen for the specific condition they suspect. There is no single test that diagnoses every cancer: blood tests and scans can provide important clues, while some diagnoses require examination of tissue or bone marrow by specialists.
Why diagnosis is a process, not one test
A symptom, blood-test result, or scan finding does not by itself establish that an infant has cancer. Each test answers a different question, and clinicians interpret the results together with the child’s history and examination. The next test depends on what the earlier findings suggest and which disease is being considered.

The National Cancer Institute (NCI) puts the central point plainly: “There is no single test that can diagnose cancer.” The workup is therefore tailored rather than a standard checklist every baby receives. Doctors consider the infant’s symptoms, age, medical history, examination findings, and the suspected cancer when choosing what to investigate.

A symptom alone is not a diagnosis, and the presence of a concern does not mean a baby has cancer. What happens first: history and examination
The clinician begins by asking about the infant’s health and the concerns that led to the visit, including how long they have been present and how they have changed. The clinician also examines the child. Together, this information helps determine whether further testing is appropriate and which tests could be useful.
This first step matters because the same symptom can have different explanations. Doctors do not interpret it in isolation; they consider the overall pattern and the infant’s medical history. If the initial findings point toward a particular problem, testing can be selected to investigate that possibility rather than searching indiscriminately for every cancer.
What the main tests can—and cannot—show
Tests differ in what they examine. Some measure cells or substances in blood, some create images of internal areas, and others allow a pathologist to examine cells or tissue directly. The test’s role depends on the suspected disease; these options are not interchangeable routes that apply to every infant.

| Test or step | What it can contribute | What it cannot establish alone |
|---|---|---|
| Health history and examination | Provides context about symptoms, their pattern and duration, and the infant’s health. | A symptom or examination finding alone does not diagnose cancer. |
| Blood tests, including a complete blood count (CBC) | Measures blood-cell counts and may assess substances associated with organ function or disease. A CBC can reveal abnormalities relevant to conditions such as leukemia. | An abnormal result does not prove cancer, and normal or abnormal laboratory results alone do not settle whether cancer is present. |
| Imaging | Creates pictures of internal areas to help investigate a suspected problem. | A scan is one part of the workup; it is not a universal cancer confirmation test. |
| Biopsy or other tissue examination | Allows a pathologist to examine cells or tissue when that is needed to identify a tumor. | It is not required as the same procedure for every suspected cancer; the appropriate evaluation depends on the disease being considered. |
| Blood and bone marrow studies | Can contribute to evaluation of suspected blood cancers. Testing may include examination of marrow and cell markers. | No one test in this group serves as a universal diagnostic route for all cancers. |
How blood tests fit into an infant’s evaluation
Blood tests can show whether blood-cell counts or measured substances are outside expected patterns and can help clinicians decide what to investigate next. A CBC, for example, can identify abnormalities relevant to leukemia. Blood chemistry tests can assess substances associated with organ function or disease.

These tests provide evidence, not a standalone verdict. An abnormal result can have more than one possible explanation and does not prove that cancer is present. A normal result also cannot rule out every cancer. Doctors interpret laboratory findings alongside the history, examination, imaging, and any tissue or marrow results that are appropriate to the suspected condition.
That distinction answers a common question: a blood test may contribute to diagnosis, but there is no general blood test that confirms or excludes every cancer in a newborn or infant. The clinician uses the results to guide the next decision rather than treating one number as the answer.
What imaging contributes
Imaging produces pictures of internal areas so clinicians can investigate a suspected problem. Which kind of imaging is chosen depends on the infant’s symptoms and the cancer doctors are considering. A scan can help locate or assess an area of concern, but it is interpreted as part of the wider evaluation.
An abnormal scan does not automatically mean cancer. The finding needs to be considered with the child’s clinical picture and any other tests. Depending on the result and the suspected disease, doctors may seek additional information, including tissue examination, or may use other tests to clarify what the image shows.

For that reason, the word “abnormal” on a report is not the same as a cancer diagnosis. The treating clinician can explain what the finding indicates in context and whether it changes the plan.
When doctors may need tissue, blood, or bone marrow examination
A biopsy is a procedure that provides cells or tissue for examination by a pathologist. When a tumor is suspected, examining tissue may be needed to identify what kind of cells are involved. Whether a biopsy is appropriate depends on the suspected cancer and the findings so far; it is not a universal step for every infant.
Blood cancers are evaluated differently from many solid tumors. For suspected leukemia, doctors may use blood and bone marrow testing. The NCI’s childhood acute lymphoblastic leukemia (ALL) summary lists diagnostic testing that can include a CBC, blood chemistry, bone marrow aspiration and biopsy, and immunophenotyping. Immunophenotyping identifies markers on cells that help classify them.

How doctors decide what comes next
The workup is guided by findings rather than by a universal order of tests. A clinician may begin with history and examination, then order laboratory tests or imaging based on what they show. If the results leave an important question unanswered, additional tests may be needed to identify the cancer type or determine its extent. Some suspected diagnoses require examination of tissue or marrow.
- Describe the concern and its course. Tell the clinician what has changed and how long the concern has been present. This helps establish the history that informs the examination and testing decisions.
- Ask what each proposed test is intended to answer. A blood test, scan, and biopsy provide different kinds of evidence. Understanding the question behind a test can make its role clearer.
- Review results as a set. Ask how a result fits with the infant’s history, examination, and other findings. One laboratory value or scan result is not the whole diagnosis.
- Clarify the next decision. If further testing is recommended, ask what uncertainty it is meant to resolve and how the result could affect the evaluation.
These are questions to support a conversation, not a substitute for clinical care. The infant’s clinician determines whether more evaluation is needed and, when appropriate, involves a pediatric oncology team.
What is specific to newborns and infants?
The testing decision is individualized for a child’s age, symptoms, medical history, and the type of cancer suspected. That supports a tailored approach, but it does not mean there is one special diagnostic sequence used for every newborn or infant. The NCI guidance does not establish a universal infant protocol.
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Fix the driver behind crashes, sound loss and screen glitches3Clear out junk files and repair common Windows errorsThere is also an important distinction between diagnostic and treatment considerations. The NCI’s childhood ALL summary notes that infants with ALL have different treatment considerations from older children. That treatment distinction should not be read as a diagnostic rule that applies to all infant cancers.
Best Value
Families should ask the care team how a proposed test is being adapted to their baby and what to expect in that specific case. The test details depend on the procedure and the infant’s circumstances; a general article cannot determine those details for an individual child.
How to understand an uncertain or abnormal result
It is possible for a test to raise a question without answering it. An abnormal blood result may need interpretation alongside other findings; an imaging result may require clarification; and a normal result may not exclude every possible cancer. This is why clinicians may order additional tests or seek specialist input rather than making a diagnosis from a single result.
- Ask which finding prompted the test and what the result does or does not show.
- Ask whether the result changes the suspected diagnosis or simply means another question needs answering.
- Ask what the proposed next step is and how it relates to the concern under investigation.
- Discuss persistent or severe concerns with the infant’s clinician rather than drawing a conclusion from a symptom or isolated test result.
There is no home test or piece of diagnostic equipment that can replace this clinician-led evaluation. Symptom notes or organized medical records may help a family communicate, but they are optional aids, not diagnostic tools or required purchases.
Sources and scope
This overview reflects the National Cancer Institute’s pages “Tests and Procedures Used to Diagnose Cancer,” its parent guide to childhood cancer, and its childhood ALL summary. The NCI’s 2025 estimates of 9,550 new cancer cases and about 1,050 deaths refer to U.S. children from birth through age 14, not specifically to newborns or infants; those broad figures do not estimate an individual baby’s risk and do not change how a particular child is evaluated.
For a real infant with concerning symptoms or test findings, the appropriate next steps depend on that child’s circumstances and belong with the treating clinician. When needed, a pediatric oncology team can help evaluate the suspected diagnosis.
