Those can include login patterns, device and location context, payment details, and recent account changes. A single unfamiliar location or device does not prove fraud; the bank weighs the context, may ask for extra verification, and can block activity or investigate when risk is high. The exact controls vary by bank, service, customer, and jurisdiction.
What banks monitor
Fraud controls can assess activity before, during, and after a transaction. They may consider both what is happening in the account and how the account is being accessed.

- Login and account activity: changes in usage patterns, repeated failed logins, account lockouts, or activity that differs from the account’s established behavior.
- Device and access context: device identifiers or behavior, IP address, and approximate location. These signals can add context, but cookies, location, and IP matching are not reliable proof when used alone, according to US interagency guidance.
- Transaction details: amount, timing, payee, transaction speed or frequency, and whether the payment fits the account’s history.
- Recent account changes: changes that could affect access or payment risk, such as setting up a new payee or using a new device.
The US interagency guidance describes these as examples, not a checklist used identically by every bank. Its appendix is non-exhaustive and acknowledges that security practices evolve. Read the FFIEC authentication and access guidance.
How a bank decides whether activity is risky
A bank can compare current activity with behavior profiles, apply rules for known fraud patterns, and combine signals in risk scores or analytics. Systems may also group activity across channels and route suspicious cases to staff for review. Saudi Central Bank controls, for example, describe baselining, typology rules, customer segmentation, scoring, cross-channel aggregation, case management, and advanced analytics; they are requirements for the relevant rulebook, not universal specifications for banks elsewhere.

Risk is contextual. A login from an unfamiliar place may be less concerning when other details fit the account’s normal use, while a new device combined with an unusual payment or recent account change may warrant more scrutiny. In a November 18, 2025 article, Federal Reserve Financial Services said the payments industry is beginning to use broader digital risk signals to detect and mitigate fraud and scams. That describes a direction in the industry, not a claim that all banks use the same technology.

Why a bank asks for extra verification
If a login or transaction looks riskier than usual, a bank may require stronger authentication before allowing it to proceed. A challenge can be tied to a new device, an unusual pattern, a sensitive transaction, a new payee, or a recent account change. Risk-based checks can vary in strength according to the situation rather than asking for the same step every time.
Controls described in the sources include multi-factor authentication (MFA), one-time passwords, device-based public-key authentication, behavioral biometrics, approval through a trusted-device notification, and a callback. Which method is available depends on the bank and service. Under Saudi Central Bank controls, SMS one-time passwords alone do not count as MFA for identification and transaction verification under that rulebook; this requirement should not be assumed to apply in other jurisdictions.

Extra verification is a protective check, not by itself a finding that the customer committed fraud. If a request seems unexpected, use the bank’s official app, website, or published contact details to verify it rather than responding to an unsolicited message.
What happens after suspicious activity is detected
The response depends on the assessed risk and the bank’s procedures. The bank may ask the customer to confirm activity, send an alert, stop a transaction, restrict or freeze an account, or refer the case to fraud staff. Some controls also look for suspected mule accounts—accounts used to receive and move illicit funds—and aim to prevent further transfers.

Alerts and case-management systems connect detection to action: an alert can prompt a customer to report activity, while an escalated case can be investigated. A temporary block or verification request may therefore be a precaution, not a final determination about what happened.

How banks try to prevent fraud, not just detect it
Prevention layers access controls, payment monitoring, customer communication, and response procedures. Banks may use MFA and device checks, apply limits or additional controls to higher-risk payments, notify customers about events such as adding a payee, and update rules or models as fraud patterns change. Customer education and clear reporting channels also matter because phishing and impersonation can trick a person into authorizing access or sharing a verification code.
Best Value
The Saudi Central Bank rulebook provides examples of elevated-risk controls such as real-time monitoring, interventions, device checks, new-payee alerts, transaction limits, MFA triggers, and model updates. The measures a customer encounters will differ by institution and jurisdiction. See the Saudi Central Bank’s fraud-risk management controls.
What customers can do when something seems wrong
- Use the bank’s official app, website, or number on a bank card or statement to check an alert or report activity.
- Do not share passwords or one-time codes in response to an unsolicited call, message, or email. Pause and contact the bank through a trusted route.
- Enable available security features and account alerts, especially for logins, payments, or payee changes.
- If you do not recognize a transaction or account change, report it promptly through the bank’s official channel and follow its instructions.
These steps complement bank controls; they do not guarantee that fraud will be prevented or determine whether a particular activity is fraudulent.
How large is the account-takeover problem?
Federal Reserve Financial Services reported that an industry study estimated more than $15.6 billion in US account-takeover losses in 2024, up from $12.7 billion in 2023. These figures refer to reported account-takeover losses—not all online banking fraud—and are attributed to the industry study, not presented as a Federal Reserve-produced estimate.
