In the attack described by federal regulators, criminals first alter ATM controls and then use fraudulent cards to withdraw cash—so monitoring ATMs alone is not enough.
What happens in an ATM cash-out attack?
The Federal Financial Institutions Examination Council (FFIEC) described a form of ATM cash-out fraud called “Unlimited Operations” in a joint statement dated April 2, 2014. In that account, attackers gained access to web-based ATM control panels and changed settings so withdrawals could exceed account balances or normal controls.

The described intrusion could begin with phishing aimed at bank employees. Malware might then help attackers observe how employees accessed ATM panels and capture their credentials. After changing withdrawal controls, criminals used fraudulent debit, prepaid, or ATM cards made with stolen account information and PINs. The cash-out could involve large, simultaneous withdrawals at multiple ATMs over a short period. This is the sequence in the FFIEC’s 2014 warning, not a claim that every attack today follows the same path. Read the FFIEC joint statement.
The FFIEC noted that one historical incident involved more than $40 million in fraud using just 12 debit-card accounts. That figure illustrates the potential scale of the incident cited in the 2014 warning; it is not a current loss total or measure of how often these attacks occur.

Which bank controls help prevent cash-outs?
Regulators frame prevention as a review of connected control areas, rather than reliance on a single product or transaction threshold. The FFIEC identifies four domains, and the Office of the Comptroller of the Currency (OCC) says community banks should include the risk in enterprise risk assessments and review whether controls are adequate.

| Fraud detection and response | Whether suspicious activity is surfaced, investigated, escalated, and acted on in time. |
These are control domains named by regulators, not a prescribed vendor, authentication method, or universal threshold. A bank’s assessment needs to account for its own systems, operations, and risks. The OCC’s 2014 bulletin also advises community banks with ATMs to work closely with their service providers and ensure the providers are taking appropriate action to mitigate the risk.

How do banks spot suspicious activity?
Monitoring can look for activity that does not fit a customer’s normal pattern, including unusual ATM transactions. FinCEN’s account-takeover advisory gives this as one possible irregularity alongside geographically clustered ACH transactions, sudden wire transfers, and changes to customer or account profiles. These examples can help guide investigation and correlation; none, by itself, proves fraud or constitutes a complete detection rule set. FinCEN Advisory FIN-2011-A016.

That broader view matters because a rapid cash-out may appear as many transactions across multiple ATMs, while the underlying warning signs may also involve account or customer changes. Banks need processes that can bring relevant signals together and prompt a timely review. The regulator guidance identifies the need to review fraud detection and response processes but does not set one required monitoring model, alert threshold, or staffing arrangement.

Why provider coordination and response planning matter
ATM service providers may be involved in the systems and operations that banks rely on to manage ATMs. The OCC specifically tells community banks to work closely with providers and ensure they are taking appropriate action. Banks therefore need clear operational arrangements for raising concerns, coordinating investigation, and deciding who takes action; the public guidance does not prescribe one universal incident-response sequence.
How this differs from risk in independent ATM businesses
A separate issue arises when a bank serves an independent ATM owner or operator as a customer. The FFIEC BSA/AML Manual says risk depends on the circumstances of the relationship and highlights transaction volume, ATM locations, and the source of cash used to replenish machines. Risk-based due diligence may examine ownership and management, operating controls, cash-servicing arrangements, source of funds, locations, and expected activity compared with actual transactions. This is customer and illicit-finance risk management—not the same problem as an attacker compromising a bank’s ATM-control systems. What to look for when assessing a bank’s approach
- Does its risk assessment cover the IT network, card authorization, ATM parameter management, and fraud detection and response?
- Can monitoring identify and investigate unusual ATM activity in the context of a customer’s broader account activity?
- Are responsibilities for coordination and action with ATM service providers clear?
The cited regulator guidance describes these areas for review but does not compare technologies or establish the effectiveness of a particular product. It also does not establish a current aggregate rate of ATM cash-out attacks.
