How AI Models Predict How Cells Respond to Drugs

AI models learn from treated and control cells to estimate drug-related molecular changes. Their accuracy depends on the prediction target, metric, and whether tests use genuinely unseen datasets or cell types.

AI models predict cellular drug responses by learning patterns in measurements from untreated and treated cells, then estimating what might happen in a specified biological context. With single-cell RNA sequencing, that estimate might be a gene-expression profile, a list of genes expected to change, or a prediction about how a drug shifts the mix of cell states. These are different tasks, and success at one does not guarantee success at another—or show that a drug will benefit a patient.

What an AI drug-response prediction actually means

A drug-response model learns associations among three things: the starting state of a cell, the treatment applied, and the measured outcome after treatment. It uses examples from experiments to estimate an outcome for a condition it has not yet seen. Depending on the question, that outcome may be a predicted expression profile, a treatment-induced change, a response category, or a ranking of candidate compounds.

“Predicting the response” is therefore not one universal task. A model asked to estimate gene activity after treating a familiar cell type with a known compound faces a different problem from one asked to estimate the effect of a new compound in a cell type absent from its training data. Claims about a model should say which outcome and which kind of unseen condition are involved.

What the prediction is—and is not

  • Predicted expression: estimated expression values for genes in treated cells.
  • Predicted change, or delta: estimated differences between treated and control conditions.
  • Response class: a category assigned to an expected outcome; the exact categories depend on the study.
  • Drug ranking: an ordering of candidate compounds for a specified purpose, based on the model’s predictions.

These outputs are related but not interchangeable. A profile can resemble measured expression overall yet miss which genes changed because of the treatment. A ranking can be useful for prioritizing experiments without being a direct prediction of clinical effectiveness.

How the model learns from cell experiments

In a typical single-cell transcriptomic workflow, researchers measure gene expression in cells without treatment or under a control condition, and in cells exposed to a drug or another perturbation. They record context such as the cell type, treatment, and experimental collection. The model uses those inputs and observed outcomes to fit a statistical or machine-learning relationship, then makes predictions for a test condition.

  1. Measure a baseline or control. This establishes what gene-expression patterns look like without the treatment being evaluated, in the relevant experimental context.
  2. Measure treated cells. Cells exposed to a drug or other perturbation provide observed outcomes against which a prediction can be learned and tested.
  3. Represent the context and perturbation. The model receives information about the biological setting and the treatment. The exact inputs depend on the study; a model can only condition on information it has been given.
  4. Fit the model on examples. It learns associations between the recorded inputs and outcomes in the training data.
  5. Test on held-out examples. Researchers withhold a condition or collection from training, ask the model to predict it, and compare the prediction with observed measurements.

The last step determines what the reported score means. Holding out cells from familiar experiments tests something different from holding out an entire dataset, cell type, or drug. If related samples appear in both training and test data, the task may be easier than predicting in a genuinely separate biological context.

Why single-cell measurements matter

Single-cell RNA sequencing measures gene-expression profiles one cell at a time. That matters because a treatment may affect only a subset of cells, or may shift the proportions of different cell states. A bulk average can conceal such variation: two samples with similar average expression could contain different mixtures of responding and nonresponding cells.

Single-cell data also create modeling and evaluation challenges. The measurements are high-dimensional, sparse, and noisy, and technical differences between experiments can complicate comparisons. A treatment effect may be small, or concentrated in a minority of cells. In those cases, a model may achieve good agreement on broad expression patterns while failing to capture the biologically relevant change.

How to tell whether a prediction is accurate

There is no single accuracy score that answers every question. Evaluation should match the intended use and state what is being compared: expression levels, treatment-induced changes, gene directions, differentially expressed genes, or whole-cell distributions.

Evaluation approach Question it asks What a strong result does not establish by itself
Expression-level agreement Do predicted expression values resemble the measured values? That the model captured the treatment effect. Small-effect treatments can look like controls, making broad agreement easier to obtain.
Delta or change metrics Did the model predict the difference between treated cells and controls? That it will generalize to a different cell type, dataset, or drug.
Direction accuracy Did genes predicted to increase or decrease change in the correct direction? That the size of each change is correct.
Differential-expression recovery Did the model identify genes that change under treatment? That predicted expression values or cell-state distributions are accurate overall.
Distributional comparison Do the predicted and observed distributions of cells resemble each other, rather than only sharing an average? That every individual cell or every response mechanism was predicted correctly.

Metrics can disagree without any of them being calculated incorrectly: they measure different properties. A useful report names the target, metric, and test design instead of calling a model simply “accurate.” It should also account for effect size. A small treatment effect can make a prediction close to the control profile look good under expression-level agreement, even if the model has not learned the treatment-induced difference. For larger effects, delta and differential-expression measures can reveal information that a broad expression score obscures.

What recent comparisons have found

A broad comparison across datasets

A systematic comparison of single-cell perturbation response prediction models (2026) evaluated 13 representative models and baselines across 25 datasets. Its results showed that performance depended on perturbation effect size and on the evaluation perspective. Expression-level agreement was strongest for small-effect perturbations resembling controls, while delta and differential-expression metrics offered clearer signals for larger effects. No method consistently generalized across cell types.

The practical implication is that a leaderboard position, if reported without its test design and metric, can be misleading. A model may do well on one target or context and poorly on another. The comparison does not support treating any one approach as a universal predictor across cell types.

scDrugMap and the difference between pooled and external data

The 2026 scDrugMap study evaluated ten models: eight single-cell foundation models and two general-purpose language models. Its curated collection contained 495,237 cells. The primary collection had 326,751 cells from 36 single-cell datasets across 23 studies; an external validation collection contained 168,486 cells from 24 datasets. The primary collection covered 14 cancer types, three therapy types, five tissue types, and 21 treatment regimens. These figures describe scDrugMap’s collections, not the total data available in the field.

In that study, performance on pooled data was better than performance on cross-data evaluation. This distinction matters: combining data can expose a model to conditions similar to those it will later be tested on, while a separate dataset tests transfer across collections. Technical variation and batch effects can contribute to a transfer gap, so a score on pooled or familiar data should not automatically be read as evidence of performance on independent data.

Unseen cell–drug combinations and unprofiled compounds

A 2026 paper in Nature Machine Intelligence describes MAP, a mechanism-aware predictor evaluated on unseen cell-line–drug combinations and entirely unprofiled compounds. Those are distinct prediction settings. Estimating the response to a known drug in a new cell context is not the same as estimating the response to a compound with no prior response profile. MAP’s evaluation uses complementary gene-level and distributional metrics, reflecting the fact that no single view captures every aspect of a cellular response.

Together, these comparisons show why the test split is part of the scientific claim. “Held out” can refer to a cell, a treatment, a cell type, or a dataset. Without that detail, readers cannot tell whether a result demonstrates interpolation among familiar examples or transfer to a new biological setting.

How to evaluate a model claim

When reading a paper, tool description, or reported benchmark, check the following before interpreting a score:

  • What is predicted? Identify whether the output is an expression profile, a change from control, a direction, a differential-expression signature, a cell distribution, a response class, or a drug ranking.
  • What was held out? Look for whether the test withheld cells, perturbations, cell types, cell-line–drug pairs, studies, or entire datasets.
  • How independent is the test collection? A separate dataset is a stronger transfer test than a random split of related observations, although the biological contexts and experimental differences still matter.
  • Which metric supports the claim? Expression agreement, delta accuracy, direction, differential-expression recovery, and distributional similarity answer different questions.
  • What is the effect size? Ask whether a treatment causes a large, small, or heterogeneous change, since this can alter how informative a metric is.
  • What conditions match the intended use? Check cell type, drug, dose, treatment duration, and assay where reported. A prediction outside the model’s familiar conditions may be less reliable.
  • What resources and preparation are needed? Model choice can depend on data characteristics, fine-tuning, curation, runtime, and computational resources. These practical requirements should be considered alongside benchmark performance.

Why predictions do not establish patient benefit

A predicted molecular response is not the same as a clinical outcome. These models can help generate hypotheses, analyze potential biomarkers, or prioritize drug-discovery experiments. The benchmark studies discussed here do not establish that a model’s output alone can select an effective treatment for an individual patient.

Clinical use would require independent experimental and clinical validation. A predicted expression change does not by itself demonstrate that a patient will tolerate a drug, that a tumor will shrink, or that a treatment will improve an outcome. Those questions require evidence beyond the model’s molecular prediction.

What these models are useful for now

The most defensible use is as a way to focus follow-up work. A model can help identify candidate changes or conditions worth testing, provided its prediction target and limits are clear. Researchers can then compare those predictions with experiments in the specific cell contexts and treatments that matter.

The central question is not whether AI can “predict drug response” in the abstract. It is whether a particular model, trained on specified examples, predicts a specified cellular outcome under a test design that resembles the intended use. Current comparisons show genuine promise, but also substantial dependence on metrics, effect sizes, datasets, and cell types.
