Its practical effect depends on the data, software, hardware, network access and authority surrounding a deployment. Human oversight matters when it is backed by monitoring, pre-set safety limits, qualified operators and a tested way to continue without AI.
What counts as critical infrastructure and operational technology?
Critical infrastructure is the interconnected set of sectors whose disruption could affect national security, economic stability, public health or safety. The Cybersecurity and Infrastructure Security Agency (CISA) identifies 16 U.S. critical-infrastructure sectors. Energy, water, transportation, communications and healthcare are familiar examples, but their equipment, regulations and failure modes differ substantially.

Operational technology (OT) is the part of this environment that interacts with the physical world. NIST’s initial public draft of SP 800-82 Revision 4, published September 21, 2026, describes OT as programmable systems or devices that interact with the physical environment or manage devices that do. Examples include industrial control systems, building-automation systems, transportation systems, physical-access controls and environmental monitoring or measurement equipment.
OT therefore has requirements beyond ordinary information-technology confidentiality. A delayed sensor reading, an unavailable controller or an unsafe command can affect people and machinery. Availability, reliability, timing and functional safety must be considered alongside confidentiality and integrity.

How can AI increase an attacker’s capability?
NIST describes a dual-use environment: AI can give defenders new tools while also enhancing the capabilities of people targeting organizations through IT and OT attacks. That statement describes potential capability, not proof that a particular outage was caused by AI. A useful way to think about AI is as an amplifier and an additional attack surface.
Processing information at scale
An attacker may use AI to sort large volumes of public or stolen information, identify relationships between systems, summarize technical documents or prioritize targets for human review. These activities could reduce the time needed to understand an organization, but they still depend on obtaining data and finding a path into the target. The available official guidance does not establish a universal set of AI attack procedures or show that these capabilities have produced a specific infrastructure incident.
Generating convincing material
Generative systems can help produce messages, code or other content that appears plausible. In a critical-infrastructure context, that could increase the scale of social-engineering attempts or make malicious material harder for a busy employee to recognize. It does not remove the need for delivery, execution and access, and defensive teams can use the same technologies for triage, detection and training.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsAdapting activity to a target
AI may help an adversary compare observed responses, adjust priorities or automate parts of a campaign. The risk rises when an organization exposes poorly controlled interfaces, credentials or data that allow a system to influence operational decisions. AI assistance should not be confused with independent control of a plant: authority, network segmentation and safety interlocks still determine what actions are possible.
Risks inside AI components and AI-enabled operations
AI introduces security questions at several layers. NIST highlights the familiar security dimensions of confidentiality, integrity and availability, along with risks to training and output data and to the software and hardware that support a model. Existing frameworks and guidance do not comprehensively address several machine-learning attack classes, including evasion, model extraction, membership inference and availability attacks.

Data and input risks
- Confidentiality: sensitive operational data can be exposed through training sets, prompts, logs, telemetry or model responses.
- Integrity: altered, poisoned or misleading data can influence a model’s recommendation or the conditions under which an automated action is considered safe.
- Availability: a model, its data pipeline or a required service can be overloaded or made unreachable at the moment an operator needs it.
Model and software risks
Evasion attacks seek inputs that cause incorrect classification or recommendations. Model extraction attempts to reproduce a model through repeated queries. Membership-inference techniques try to determine whether particular records appeared in training data. These categories matter even when a model is used only for advice, because an operator may act on an inaccurate result or stop receiving timely information.
Dependencies also matter. A model can rely on an application, update mechanism, identity service, cloud connection, sensor feed or specialized hardware. A weakness in any of those components can affect the AI-enabled function without a defect in the model itself. Inventorying only the model name leaves important exposure undiscovered.

Can AI hack a power grid or water system?
AI could assist an attack against a power, water or other OT environment, but the sources reviewed do not verify a comprehensive list of AI-caused infrastructure outages. They describe ways capability and risk could increase, not an inevitable outcome. Whether an AI-assisted attempt can produce physical impact depends on access controls, segmentation, engineering design, safety systems, operator actions and the reversibility of the targeted process.
CISA’s 2023–2024 AI Roadmap said the agency would assess whether AI adoption could make infrastructure more vulnerable to critical failures, physical attacks and cyberattacks, and consider mitigations. That roadmap records an announced priority at that time; it is not evidence that those attacks occurred or a report of current implementation.
What incidents have been verified?
Official sources used for this topic do not provide a named statistic for AI-enabled attacks against critical infrastructure, nor do they attribute a specific power, water, transport or healthcare outage to AI. It is important to keep three statements separate:

- [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by
- AI can expand the capabilities available to someone conducting an IT or OT attack.
- AI components and their data, software, hardware and availability can themselves be attacked.
- A verified incident requires an authoritative account linking AI to the observed action or impact.
Without that third element, describing a capability as a confirmed attack would overstate the evidence. Organizations should still plan for the possibility because prevention and recovery decisions must be made before attribution is certain.

Where human oversight makes a practical difference
Joint guidance from CISA, Australia’s Australian Signals Directorate’s Australian Cyber Security Centre (ASD’s ACSC) and partner agencies, issued in December 2025, says: “Ultimately, humans are responsible for functional safety.” The statement is meaningful only when people have the information, time, authority and alternatives needed to intervene.
1. Map every AI dependency
Create an inventory of models, data stores, sensors, applications, hardware, vendors, interfaces and downstream systems that rely on AI. Record whether each component is advisory, capable of sending a command or able to trigger an action without a second approval. Include development, testing, update and remote-maintenance paths. Review the inventory after architecture or model changes.
2. Log inputs, outputs and consequential actions
Retain the input data or a suitable reference to it, model version, output, confidence or quality indicators, operator response and any resulting command. Logging should cover failed requests and unavailable dependencies, not only successful recommendations. Protect logs from unauthorized alteration and make them usable during an incident review.
3. Set performance and safety thresholds before deployment
Define what counts as acceptable accuracy, latency, sensor quality and data freshness for each use. Establish limits for physical variables and operating states, and specify what happens when a limit is crossed. Thresholds should be tied to engineering and safety requirements rather than a generic model score. Monitor continuously for drift, unusual inputs and outputs outside the known-good range.

4. Match approval to consequence
For a passive system that recommends a maintenance priority, integrate the recommendation into the existing decision process so a qualified person can review it. For a system that can alter a set point, open a gate or change a protection setting, document which actions require explicit approval and which conditions block execution. The more consequential and difficult-to-reverse an action is, the stronger the approval and interlock requirements should be.
5. Provide a non-AI fallback
Identify the condition that causes the system to default to non-AI operation when performance, safety or availability falls below the agreed threshold. The fallback may be manual control, a deterministic rule set, a conventional controller or a reduced-service mode. Keep the necessary procedures, credentials, staffing and equipment available; a fallback that exists only on paper is not a recovery option.
6. Test restoration, not just detection
Exercise the transition to fallback and the return to normal operation under realistic load and timing constraints. Verify that operators can see why the transition occurred, maintain safe control while the AI service is unavailable and restore trusted data and software. Include model corruption, bad sensor data, lost connectivity and compromised update paths in exercises.
7. Put AI scenarios into incident response
Response plans should cover a manipulated input, an anomalous recommendation, an unavailable model, suspected model extraction, compromised credentials and an unauthorized automated action. Define who can disable the AI function, who can authorize reactivation and how safety and regulatory notifications are handled. Preserve evidence without delaying actions required to keep people and equipment safe.
Comparing AI deployments by risk and oversight need
The label “human in the loop” is not a sufficient safety control. Use the following questions to distinguish deployments:
Best Value
| Assessment axis | Lower-consequence pattern | Higher-consequence pattern |
|---|---|---|
| Authority | AI provides a recommendation; an operator executes any change. | AI can trigger or directly issue an operational command. |
| Consequence and reversibility | Outcome is limited and readily reversible. | Failure can affect safety or equipment and is difficult to reverse. |
| Visibility | Inputs, outputs, dependencies and model version are observable. | Data lineage or dependencies are opaque, delayed or incomplete. |
| Operator intervention | A qualified operator has enough time, context and authority to review. | Events move faster than review, or staffing and authority are unclear. |
| Limits | Performance and safety thresholds are defined and monitored. | No agreed threshold exists, or alerts do not block unsafe action. |
| Fallback and restoration | Non-AI control is available and has been tested. | Fallback is untested, unavailable or dependent on the same failed service. |
This comparison is a decision aid, not a numerical risk score. The official guidance does not quantify how much oversight reduces the probability or severity of an incident.
Current guidance and what its status means
NIST AI Risk Management Framework
NIST released the voluntary AI Risk Management Framework on January 26, 2023 and reports that it is being revised. “Voluntary” means organizations can use it to structure risk work; it is not, by itself, a mandatory sector regulation.
Critical-infrastructure AI RMF profile
NIST released a concept note for a critical-infrastructure profile on April 7, 2026. The planned profile is intended to help operators identify risk-management practices for AI-enabled capabilities. A concept note is not a final profile, so organizations should not present it as completed guidance.

The initial public draft of SP 800-82 Revision 4 was published September 21, 2026, with comments due November 30, 2026. It addresses OT security, including unique performance, reliability and safety requirements, and aligns recommended countermeasures with the NIST Cybersecurity Framework 2.0. It remains a draft rather than a final standard.
Joint principles for secure AI in OT
The December 2025 principles from CISA, ASD’s ACSC and partner agencies focus on secure integration of AI in OT. Their oversight and failsafe practices include inventory, input and output monitoring, known-good states, safe thresholds, operator or engineer participation and a defined return to non-AI operation.
A sensible operating rule
Treat AI as a component that can assist people and as a component that can fail or be attacked. Before granting it authority over an OT process, prove that its inputs and dependencies are visible, its limits are measurable, a qualified operator can intervene in time and a tested non-AI path can keep the operation safe. That approach addresses both sides of the risk: an adversary using AI to improve an attack and an AI-enabled system producing an unsafe result of its own.
