I shipped rules and statistical methods instead of an LLM because a health app should use the approach that fits its actual task, evidence, and consequences—not the newest model by default. That is the decision this title describes; it does not establish what the app does or prove that one architecture is safer or more accurate in every case.
Start with the job the app must do
The architecture choice only makes sense once the product’s input, output, and failure conditions are clear. A fixed calculation, a recommendation based on established guidance, and an open-ended question about someone’s health are different jobs. They call for different evidence and different ways to check whether the software behaves acceptably.

For this app, the available account does not specify its users, clinical area, claims, inputs, outputs, or test results. Those details matter: without them, no one can assess the particular decision or claim that the implementation was validated for a given use.

Rules for bounded tasks with established guidance
Rules encode behavior as explicit conditions and outcomes. The Office of the National Coordinator for Health Information Technology (ONC) describes rule-based clinical interventions as grounded in expert consensus and evidence such as clinical trials, observational studies, and other research. That can be a sensible fit when a task is bounded and accepted guidance clearly supports the behavior. It does not mean a rule is complete, current, or appropriate for every patient or health problem.

Statistics for patterns that guidance does not capture
Statistical and machine-learning predictive approaches can learn patterns from real-world data and may help address questions where formal guidelines are absent. Their usefulness depends on the data and the setting: poor-quality or unrepresentative data can undermine performance, and a model may behave differently when deployed in a context unlike the one in which it was developed. Some models are also difficult to interpret. ONC explains these distinctions in its overview of rules-based and predictive approaches in health care.
What an LLM changes—and what it does not solve
An LLM can generate fluent, plausible answers without guaranteeing that they are correct. The World Health Organization warned on 16 May 2023 that health-related LLM responses can be “completely incorrect or contain serious errors.” WHO also identifies risks involving bias, sensitive-data protection, consent, and transparency, and calls for supervision, rigorous evaluation, and clear evidence of benefit before routine widespread use. Those are reasons to scrutinize an LLM in a health product, not proof that every non-LLM system is safer.
Rules can encode incomplete or outdated guidance. Statistical models can be useful when fixed guidance does not answer the question. An LLM may be appropriate for some bounded support tasks, but fluent language is not a substitute for a reliable evidence base, careful evaluation, or a plan for oversight. The relevant question is whether the system’s behavior can be justified and checked for its intended use.

Compare approaches against the actual risks
| Question | Why it matters |
|---|---|
| Is there accepted guidance for the task? | Explicit guidance may support a rule; a question without formal guidelines may require another method or a narrower product claim. |
| How predictable and inspectable must the output be? | Rules make conditions explicit, while data-derived predictions and generated text require their own methods of interpretation and testing. |
| Are the data representative of the intended setting? | Data quality and differences between development and deployment contexts can affect predictive performance. |
| What happens if the output is wrong? | The potential harm, including whether a decision is time-critical, should shape the required safeguards and level of scrutiny. |
| What user data is processed, and on what basis? | Privacy, sensitive-data protection, and consent are material considerations, especially when introducing an AI service or sharing data beyond the app. |
| How will the team validate and monitor it? | A method needs evaluation in the intended setting and a feasible way to detect problems after deployment; no architecture earns trust by label alone. |
The sources do not establish that a rules-and-statistics stack is universally safer, cheaper, more accurate, or more effective than an LLM. A comparison of those outcomes would require app-specific measurements, defined test methods, and details about the relevant users and setting.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.U.S. FDA status depends on the software function
Calling a product a “health app,” “AI,” or “clinical decision support” does not determine its U.S. regulatory status. FDA’s analysis depends on intended use and the individual software function, including its inputs, outputs, user, claims, and the decision it supports. The agency’s Clinical Decision Support Software FAQ and CDS policy navigator explain the relevant distinctions.
For example, FDA says that software intended to perform simple medical calculations routinely used in clinical practice is not considered non-device CDS software; the FAQ then describes circumstances in which FDA intends to exercise enforcement discretion. That statement should not be read as a blanket classification of all calculation software. The cited non-device CDS criteria are also limited: patient-facing functions do not meet the criterion for recommendations to healthcare professionals, and time-critical decisions or specific preventive, diagnostic, or treatment outputs can affect whether the exclusion applies. A product’s classification cannot be determined from its architecture alone.

FDA’s page for its generative-AI medical-device discussion paper addresses risk assessment, premarket evaluation, and postmarket monitoring. FDA explicitly says the paper does not represent draft or final guidance and does not propose policy changes, so it is an active policy discussion rather than a binding requirement: Considerations for the Regulation of Generative AI-Enabled Medical Devices.

A first-person account of this choice is strongest when it connects the architecture to the product’s real constraints. It should explain what the app receives and returns, what evidence supports its behavior, what kinds of errors matter, how the approach was evaluated for its intended users, and how the team monitors it after release. Without those particulars, the rationale remains a general design principle rather than evidence that the shipped system works as intended.
