Choose a form that fits the job and review purpose, then adapt it to your organization’s process.

US federal performance forms and guidance
For US federal performance management, OPM and GSA provide resources built around federal processes. They can be useful examples, but should not be treated as universal private-sector forms.
OPM Performance Management Toolkit
The OPM Performance Management Toolkit separates resources for supervisors, employees, and HR. It includes guidance on performance plans, individual and group goals, performance standards, individual development plans, and an employee check-in template. OPM presents performance management as an ongoing process of setting expectations, tracking progress, and giving feedback—not simply assigning a rating.
OPM Employee Check-in Template
The Employee Check-in Template is intended to help employees document progress reviews during a performance-plan period. It is a fit for federal employees who need a record of check-ins, rather than a general appraisal form for any employer.

GSA Form GSA3440
GSA Form GSA3440 is titled “Performance Plan and Appraisal Record for Non-Supervisory Employees” and is listed under HRM 9430.2. The page gives a revision date of May 2026 and was last updated May 19, 2026. Its stated scope is federal non-supervisory employees.
How to choose the right form
Start with the task the document needs to support, then check its format and workplace context. A self-appraisal is for the employee’s perspective; a process questionnaire evaluates the appraisal scheme itself; neither substitutes for a manager’s assessment against job expectations.
Rank #3
- Objective review: Choose an objectives-based form when the review is anchored to goals communicated for the role.
- Role-specific review: Use the manual-worker form when that category better fits the work being reviewed.
- Promotion decision: Use a promotion assessment form when the question is suitability for a new role, not simply performance in the current one.
- Employee input: Add a self-appraisal when you want the employee to prepare their own account ahead of the discussion.
- Process evaluation: Use the scheme questionnaire to gather feedback about how the appraisal system works.
- Federal context: Use OPM or GSA material when the review is part of the relevant US federal performance-management system.
- File compatibility: Acas lists its templates in Word and OpenDocument formats; choose the version that works with the software your workplace uses.
What to include when adapting a performance report
A useful report connects an employee’s work to expectations that were clear for the role and review period. OPM’s guidance emphasizes clear standards, meaningful feedback, and development. Its handbook offers examples of performance elements and standards, including work results and observable behaviors; those examples are drafting references for comparable work, not ready-made standards for unrelated jobs.
- Identify the employee’s role and the period being reviewed.
- State the job-relevant objectives or standards used to assess performance.
- Provide space for specific evidence and comments rather than relying only on an overall rating.
- Give the employee an opportunity to record their perspective.
- Where relevant, record development goals, follow-up actions, and who is responsible for them.
These are practical design considerations drawn from the forms and OPM guidance, not a claim that every jurisdiction requires these exact fields. A template’s availability does not establish that it is legally sufficient for every location, role, agreement, or employer process; confirm applicable requirements before relying on it for compliance.

For examples of how performance elements and standards can be written, consult OPM’s A Handbook for Measuring Employee Performance. Use its examples to shape standards suited to the actual job rather than copying measures from an unrelated example role.
OPM also describes USA Performance as government-owned software supporting the federal appraisal lifecycle, including plans, goals, progress reviews, ratings, certification, and electronic records. OPM reports that more than 120 federal agencies use it for nearly 260,000 SES, SP, and non-SES/SP employees; those figures are OPM’s user data current as of March 2026, not an independently validated count.
