Read its sections together: gross pay is not the same as net pay, and current-period figures are different from year-to-date totals. A free template can help organize this information, but no single layout is a universal legal requirement.
What a check stub shows
A check stub connects the pay a worker earned with the deductions taken and the net amount paid. Many layouts include employee and employer details, a pay period, earnings, deductions, gross and net totals, and columns for the current period and year to date. The IRS describes this kind of layout and provides a sample payroll ledger with hours, rates, earnings, taxes, insurance and net pay: IRS guidance on pay statements and payroll records.
A template is a way to present payroll information, not a guarantee that the information is correct or that the document meets every jurisdiction’s rules. Federal recordkeeping rules and state pay-statement laws are distinct.
What each field and section means
Pay period and payment date
Pay period begin and end identify the dates of work covered by the payment. Payment date or check date is when the payment is issued; it may fall after the work period ends. The University of Oklahoma’s pay-stub guide explains this distinction: University of Oklahoma paycheck stub guide.

Employee and employer information
Names and identifying details associate the statement with the worker and employer. Templates may include addresses or an employee identifier, but exact fields vary. Avoid putting a full Social Security number on a sample or publicly shared document; mask sensitive information where it is not needed.
Tax status or withholding information
A statement may display filing status or other withholding details. Federal income-tax withholding depends on the employee’s current Form W-4 information and the employer’s calculation method. The IRS publishes annual withholding procedures in Publication 15-T: IRS Publication 15-T. An old “allowances” field should not be treated as a description of the current W-4 process, and a sample stub is not a withholding calculator.

Hours, rates and earnings
Hourly statements may list hours and rates by earnings type, such as regular time, overtime, bonuses or other pay. These details help explain how earnings were calculated. For covered, nonexempt workers, federal FLSA records include the employee’s pay basis and rate, hours, straight-time earnings and overtime earnings. State rules can require additional detail. For example, Maryland says statements under its law must report all rates and hours when multiple rates apply, including straight-time and overtime: Maryland pay stub requirements.
Gross pay
Gross pay is earnings before the deductions shown on the statement. Review the earnings categories as well as the total; different payments may not be treated identically for every tax purpose.

Deductions and withholdings
This section may include federal income-tax withholding, Social Security, Medicare, state withholding, insurance premiums or other deductions. Which labels appear depends on the worker’s situation and employer benefits. The IRS sample payroll record illustrates several of these categories, but it is an example rather than a complete list for every worker.
Net pay
Net pay is the payment amount remaining after the listed deductions. Compare the gross total, each deduction and the net total to understand how the payment was calculated. The figures alone do not establish that tax withholding is correct.

Current period and year to date
Current-period figures cover this pay period. Year-to-date (YTD) figures accumulate from the start of the year through the current statement. The IRS notes that paired columns help employees reconcile withholding over the year; a YTD total is not another amount paid on this check.
Leave balances and messages
Some templates include leave balances or a messages area. These can be useful additions, but they are not universal pay-stub fields. Maryland’s FAQ, for instance, says its pay-stub law does not require leave balances; a separate state sick-and-safe-leave rule may require a written balance statement.

How to use a free check stub template
- Choose a layout that fits the payroll. It should make the pay period, payment date, earnings categories, deductions, gross pay and net pay easy to distinguish.
- Include the detail needed to explain earnings. For hourly work, use separate lines or columns for hours and rates, including overtime or other pay where applicable.
- Separate current-period and YTD amounts. Label each column clearly so a reader does not mistake accumulated annual totals for this check’s figures.
- Protect personal information. Do not expose full Social Security numbers in shared examples or unsecured documents.
- Reconcile the totals. Check that listed earnings and deductions correspond to the gross and net amounts. Use current payroll and tax information rather than relying on a blank template to calculate withholding.
- Check the rules for the relevant jurisdiction. Employers should confirm applicable state and local pay-statement requirements with the relevant labor agency; a template’s downloadable or “free” status does not establish compliance.
A spreadsheet can be enough to track payroll categories when a payroll system does not provide the desired layout; the IRS notes that a spreadsheet may be designed for this purpose. Treat it as a sample layout, not a legally universal form.
Does federal law require a particular check stub form?
No single federal check-stub layout is prescribed by the FLSA. The U.S. Department of Labor states: “There is no required form for the records, but the records must include accurate information about the employee and data about the hours worked and the wages earned.” This applies in the context of FLSA-covered employers and covered nonexempt workers. DOL’s list includes identifying information, hours worked, wage basis and rate, straight-time and overtime earnings, additions or deductions, total pay per period, payment date and the period covered: U.S. Department of Labor recordkeeping guidance.
That is an employer recordkeeping rule, not a universal mandate to use a particular pay-statement form or to show every listed record on a worker’s stub. State pay-statement laws can impose separate disclosure duties. Maryland, for example, provides a voluntary template and says properly completing it satisfies that state’s stated pay-stub requirements. Requirements elsewhere may differ; this U.S.-focused overview is not a fifty-state survey.

How to assess whether a template fits
- Jurisdiction: Determine whether you need an employer payroll record, a worker-facing pay statement, or both, and identify the applicable state or local rules.
- Required versus optional fields: Do not assume fields in a sample are legally required—or that the sample includes everything your jurisdiction requires.
- Period labels: Check that current-period and YTD values are separate and clearly named.
- Earnings clarity: For hourly or multi-rate work, make sure the layout can show hours, rates, overtime and other earnings without merging them into an unexplained total.
- Privacy: Check how the template handles sensitive identifiers before distributing or storing completed statements.
