Fix RAG Failures on Real Documents: Find the Broken Stage

Real-document RAG failures often begin before the language model sees a query. Trace evidence from the source page through parsing, chunking, retrieval, and generation to find the earliest broken stage.

A RAG pipeline that works on clean text can fail on real documents because the failure may start before retrieval: parsing can lose reading order, structure, or meaning, and chunking can separate evidence from the context that explains it. Diagnose the earliest broken stage—source conversion, indexed chunks, retrieval, then answer generation—before changing models or prompts.

Why clean-text prototypes break on production documents

Retrieval-augmented generation (RAG) preprocesses and indexes source material, retrieves passages relevant to a query, then gives those passages to a language model. Each step depends on the previous one preserving usable evidence. If a PDF parser scrambles a table or separates a heading from its paragraph, retrieval may never see the relationship a person sees on the page.

Real files contain more than continuous prose: scanned pages, columns, tables, formulas, lists, footnotes, headers, forms, and unusual reading orders. A text extractor can return readable words while flattening or reordering those relationships. The resulting text may look plausible in a quick spot-check yet be incomplete or misleading for a question that depends on structure.

An Association for Computational Linguistics 2026 Industry Track paper examines 11 challenging document types, including complex or watermarked backgrounds, historical pages with non-standard reading order, decorated text, tables, and formulas. Its central warning is practical: high character- or word-level OCR accuracy does not guarantee useful evidence for RAG. Structural and semantic mistakes can still cause major retrieval failures.

Trace the failure from the source page to the answer

Use questions with known answers and supporting locations. For each failure, compare the original page, extracted representation, indexed chunks, retrieved passages, and final answer. The first point where the evidence becomes missing, disconnected, or incorrect is the stage to fix.

  1. Build a representative test set. Include the document types users actually query, such as clean digital PDFs, scans, tables, multi-column pages, footnotes, and forms. Pair each document with questions whose answers and evidence locations are known. Microsoft’s evaluation guidance likewise starts with collected test documents and queries.
  2. Compare extraction with the rendered page. Check whether text is missing, reading order is wrong, table labels are detached from values, or section headings disappear. For OCR, do not use character error rate as the sole acceptance test; inspect whether the extracted content preserves the information needed to answer questions.
  3. Inspect the indexed chunks. Confirm that passages retain their relevant heading, table context, and source location. Check whether a boundary splits a definition from its qualification or a value from its row and column labels. A chunk that looks coherent by itself should still be checked against the page it came from.
  4. Test retrieval without generation. For every test query, inspect the returned passages: do they contain the answer, all parts of it, and enough surrounding evidence to interpret it? If they do not, focus on parsing, chunking, metadata, embeddings, or retrieval settings—not on rewriting the answer prompt.
  5. Assess the generated answer against the retrieved evidence. Check whether it is grounded in the passages, complete, relevant, and correct, and whether it actually uses the returned context. If retrieval supplied the right evidence but the answer misstates or ignores it, investigate generation and prompt behavior.
  6. Change one stage at a time and re-index when necessary. Keep a before-and-after record against the same test set. The UK Government’s RAG overview notes that changing chunking or embeddings in its described workflow requires re-indexing the chunks.

What to inspect at each stage

Parsing: did the machine preserve what the page means?

Check extraction for missing text, altered reading order, lost hierarchy, broken table associations, and dropped footnotes. These are not cosmetic defects if they change which fact belongs to which heading, row, or qualification. A visually accurate OCR transcript can still be a poor retrieval source when its structure or meaning is wrong.

Choose preprocessing according to the actual file mix. PDFs and images may need conversion or OCR, while files with meaningful layout may benefit from parsing that preserves structural elements. Google Cloud’s documentation describes a layout-aware parser that detects text blocks, tables, lists, titles, headings, headers, and footnotes in supported file types. That is a documented capability, not evidence that it is the best parser for every corpus.

Chunking: did splitting preserve enough context?

Chunk size and splitting strategy affect both the context available to the model and the material retrieval can match. Very short pieces can lose the heading or qualification that makes a statement interpretable; large pieces can include substantial unrelated material. There is no universally best chunk size established by these sources.

Google Cloud documents an option to keep chunks within layout entities and append ancestor headings, which can help retain context during chunk retrieval and ranking. Its documented 100–500-token option and 500-token default describe that service configuration only; they are not general RAG recommendations. Test boundaries against your documents and questions rather than copying a number in isolation.

Retrieval: did the right evidence make it into the context?

Use known-answer questions to check whether retrieved passages contain the supporting facts and their qualifications. A relevant-looking passage may still be insufficient if it omits a table label, adjacent sentence, or another section needed to answer every part of the query. Also note when irrelevant passages are returned; excessive noise can make generation less reliable even when one useful chunk is present.

Generation: did the answer represent the evidence faithfully?

Only diagnose answer generation after verifying that retrieval returned adequate evidence. Compare each claim in the response with the passages supplied to the model. Look for unsupported additions, omitted parts of a multi-part question, and distortions of numbers or conditions. Microsoft’s evaluation guidance cautions that language-model responses are nondeterministic: the same prompt can produce different results, so interpret evaluation as a range across runs rather than a single definitive score.

A cluster of four MS-S1 MAX units successfully ran a DeepSeek-R1 671B Q4 large model. A reserved cluster power-on interface allows for unified start-up and shutdown
  • Groundedness: whether claims are supported by the supplied context.
  • Completeness: whether all parts of the question are answered.
  • Utilization: how much of the returned context is actually used.
  • Relevance: whether the response addresses the question.
  • Correctness: whether the answer is factually right.

These dimensions help localize a weak result. Low completeness can point to missing evidence or a missed sub-question; poor utilization can signal that retrieved context is irrelevant or that generation is not using it. Treat scores as diagnostic evidence alongside page, chunk, and retrieval inspection—not as a substitute for them.

How to interpret published benchmark numbers

A 2026 arXiv preprint evaluated PDF conversion on a manually curated corpus of 36 Portuguese administrative documents: 1,706 pages, approximately 492,000 words, and 50 questions. With the authors’ evaluation method and LLM-as-judge scores averaged over 10 runs, it reported 86.9% for a naive PDFLoader baseline, 97.1% for manually curated Markdown, and 94.1% for its best automated configuration. The authors report that metadata enrichment and hierarchy-aware chunking contributed more than conversion-framework choice alone.

Those percentages describe that paper’s corpus and evaluation, not expected results on another document collection and not a general parser leaderboard. The result supports a narrower engineering lesson: preprocessing choices, preserved hierarchy, and metadata can materially affect downstream RAG performance.

Choose an ingestion approach against your corpus

When comparing parsers, chunking methods, or retrieval configurations, use evidence tied to the files and questions that matter to your application. Check:

  • Document coverage: Can it handle your actual mix of scans, digital PDFs, tables, formulas, and layouts?
  • Structure preservation: Does it retain reading order, hierarchy, table relationships, and source locations?
  • Retrieval usefulness: Do returned chunks contain enough evidence for representative questions, without overwhelming them with irrelevant material?
  • Answer behavior: Are answers grounded, complete, relevant, and correct, and does the model use the context?
  • Operational cost: Will a change require rebuilding the index, and are the settings adjustable after the data store is created?

Vendor documentation can establish what a service says it supports; it does not establish comparative performance across your corpus. Base the choice on the same representative test set and failure checks you use to debug the pipeline.
