Combine an account-level failure counter with progressive delays, proportionate risk checks, a safe recovery route, and monitoring. A hard lockout triggered by anyone who knows a username can turn an anti-guessing control into a denial-of-service tool.
Build throttling around the account, not just the IP address
Apply controls on the server to the authentication mechanism, and track failures in a way that follows the account. A source-IP-only limit can be sidestepped by distributing attempts across addresses; an account-aware counter helps bring those attempts together. OWASP’s Authentication Cheat Sheet recommends associating the failed-attempt counter with the account rather than only the source IP.

Account awareness does not mean every failed password should immediately disable the account. If an unauthenticated person can trigger a durable lock by submitting a victim’s username, the control gives that person leverage over the victim. Set a threshold, an observation window, and a restriction duration for your system’s threat model; OWASP identifies these as related lockout-design choices, but the guidance does not establish one threshold or window that fits every service.
Choose a response that slows guessing without handing attackers a lockout button
| Control | What it helps with | Trade-off and use |
|---|---|---|
| Progressive delay | Slows repeated guesses while avoiding an immediate, fixed-duration account lock. | Increase waits as failures accumulate. OWASP describes exponential delay as an alternative to a fixed lockout duration; NIST suggests increasing wait as an account approaches its applicable limit. |
| Bot or CAPTCHA challenge | Adds friction to automated attempts. | Use as defense in depth, potentially after suspicious behavior or some failures, rather than as the only control or a challenge every user must always pass. OWASP cautions that CAPTCHA can be bypassed or outsourced. |
| Risk-based checks | Can help distinguish unusual attempts from a user’s more typical access pattern. | Signals such as IP address, geolocation, request timing, or browser context are imperfect. Do not treat any one signal as proof of identity or as a complete scoring recipe. |
| Temporary authenticator restriction | Can stop continued use of an authenticator after repeated failures where applicable requirements call for rate limiting. | Keep the scope precise: disabling an authenticator is not necessarily the same as locking an entire account, and users need an appropriate way to regain access. |
These controls can be combined. For example, a service can apply an account-aware delay and introduce a challenge when attempts look automated, while using contextual signals to decide whether an additional verification step is warranted. The objective is to make repeated guessing progressively less useful without making a stranger’s request sufficient to deny the account owner access.

Interpret NIST’s 100-failure limit in context
NIST SP 800-63B Revision 4, section 3.2.2, requires verifiers to rate-limit failed authentication attempts where the relevant authenticator requirements apply. Unless otherwise specified, it sets an upper bound of 100 consecutive failed attempts using a specific authenticator on one subscriber account before disabling that authenticator, and permits lower limits. This is a standards limit for specified authenticator cases—not a universal recommended setting for every website login or a target that products should automatically adopt.
NIST also describes measures that can reduce the chance that rate limiting blocks a legitimate claimant: a bot-detection challenge, increasing waits as the account approaches its limit, and risk-based signals such as IP address, geolocation, request timing, or browser metadata. After successful authentication, reset retry counts for the authenticators used in that successful authentication, as NIST specifies.

For organizations implementing NIST SP 800-53 Revision 5 control AC-7, the limit on consecutive invalid logons and the response are organization-defined. Its discussion notes that automatic lockouts are usually temporary because of denial-of-service risk; delaying the next login prompt or notifying an administrator are among the possible responses. AC-7 is organization-specific control guidance, not a consumer-site threshold.

Preserve recovery and communicate the restriction
Keep an alternative route available
Give legitimate users a usable path to regain access while a login restriction is active. OWASP identifies forgotten-password access during lockout as one possible mitigation. That route must be designed and tested as part of the authentication system: an easier recovery path can become the weak point an attacker targets. Consider the assurance required for the account and the recovery method, and do not assume that unlocking itself is harmless.

Explain what the user can do next
Tell the person attempting to sign in when a wait applies and what safe next step is available, without revealing whether an unauthenticated request has identified a real account. OWASP Top 10:2025 recommends consistent messages across registration, recovery, and API pathways to make account enumeration harder. Apply that consistency where account existence is sensitive, while still giving an affected user useful guidance after the service can safely establish which account they control.

Record failed authentication events and alert administrators to patterns that may indicate credential stuffing, brute force, or other attacks, as OWASP Top 10:2025 recommends. Monitoring should let responders distinguish repeated attempts against one account from broad or distributed activity, while externally visible responses avoid confirming account existence to an unauthenticated requester.

Define who reviews alerts and what response is appropriate for the service. A delay, challenge, investigation, or temporary authenticator restriction may be more suitable than a permanent account disablement. OWASP also recommends considering trusted, premade authentication, identity, and session-management systems; choosing to build or buy does not remove the need to verify that the resulting controls and recovery flows meet your requirements.
Test the lockout and recovery paths together
Test with accounts you control in a non-production environment or under an approved test plan. OWASP’s Web Security Testing Guide describes exercising failed logins and checking whether a correct login still works, as well as examining unlock behavior.
- Send repeated failures for one known test account, then check the response to correct credentials during and after the configured delay.
- Repeat attempts from changing IP addresses and verify that the server-side, account-aware control still applies.
- Check whether a third party can cause a victim account to become unavailable using only unauthenticated attempts.
- Exercise password recovery and any other alternate access route during a restriction; check both that legitimate recovery remains usable and that the route does not bypass the intended assurance.
- Run equivalent checks through login pages, APIs, registration, and recovery flows. Look for inconsistent account-enumeration responses or an unthrottled alternate endpoint.
- Verify that a successful authentication clears the retry state for the authenticators used, as intended by the applicable policy.
- Confirm that logs and alerts capture useful attack patterns without disclosing whether an account exists to the requester.
OWASP warns that unlock mechanisms can themselves create a denial-of-service path, and distinguishes time-based, self-service, and administrator-mediated approaches with different assurance implications. Test the complete path—not only the failed-password counter—before relying on it.
