What TMS does—and why supervision matters
TMS uses magnetic fields to stimulate nerve cells in the brain. Repetitive TMS (rTMS) delivers repeated magnetic pulses. Mayo Clinic says rTMS is generally used for depression when other treatments have not been effective, and describes treatment sessions taking place in a health care provider’s office or clinic (Mayo Clinic’s TMS overview).

“Noninvasive” describes how treatment is delivered: it does not involve surgery. It does not mean that a person can safely choose a device, set its output, or determine their own eligibility. The FDA’s guidance for rTMS systems addresses device controls and labeling, operator training, patient screening, and safety monitoring. It calls for trained personnel who can respond to seizures, with emergency equipment available (FDA’s rTMS special-controls guidance).

Device settings and indications are not one-size-fits-all
FDA guidance says rTMS systems should be used within their labeled indications. Device labeling and indications can differ, so a generic online protocol or consumer magnetic gadget cannot establish that a device or setting is appropriate for a particular person. A clinician must assess the treatment plan and the device’s specific instructions.

Screening depends on the person and the device
Before treatment, clinicians need to assess relevant risks and device compatibility. Do not use a generic checklist to clear yourself: tell the clinician about implants or metal near your head, a history of seizures or epilepsy, medications, pregnancy, and relevant neurological or psychiatric conditions. The clinician can determine what matters for the specific device and treatment.
Monitoring and emergency readiness matter
Seizures are an uncommon but serious risk. FDA guidance recommends monitoring patients for seizures and having trained personnel and emergency equipment available. That is a clinical safety plan, not a precaution that can be replaced by stopping a session yourself.

Side effects and hearing protection
Common side effects reported by Mayo Clinic include scalp discomfort or pain, headache, tingling or facial-muscle twitching, and lightheadedness. Less common but serious risks include seizures and mania; hearing loss can occur if ears are not adequately protected. Mayo Clinic notes that more study is needed to establish long-term side effects (Mayo Clinic).

Hearing protection is part of safe rTMS sessions. The FDA guidance states: “Patients and those operating the device should always wear earplugs or similar hearing protection devices with a rating of 30dB of noise reduction during rTMS treatment.” The clinic should specify or provide suitable protection; wearing earplugs does not make at-home TMS safe.

What a supervised course can involve
rTMS is generally given as a course of repeated sessions rather than a single exposure. Mayo Clinic describes a typical depression schedule as sessions five days a week for four to six weeks. That is a general description, not a schedule for every patient; protocols vary. A clinician can explain the intended course, how it fits your circumstances, and what to expect at appointments.

- Share your relevant medical and psychiatric history, including seizures or epilepsy, and disclose medications, pregnancy, implants, and metal near the head.
- Ask whether the specific device is labeled for your situation and what screening or precautions apply to you.
- Ask how the clinic monitors for adverse events, who is present during treatment, and how it would respond to a seizure or other urgent problem.
- Confirm what hearing protection is required and whether the clinic supplies it.
- Discuss the proposed session schedule, practical access, and your insurance coverage or out-of-pocket cost directly with the provider and insurer.
Do not begin, adjust, or substitute a home magnetic device for prescribed treatment based on general information. If you are considering TMS, discuss the option with a qualified clinician who can assess your needs and explain the device-specific risks and alternatives.
