Designing Hindsight Retain and Recall for SRE Incidents

Use Hindsight Retain to preserve structured, time-anchored incident facts and Recall to find relevant precedents—then verify every lead against current evidence and its source record.

Use Hindsight to make past incidents easier to find—not to decide what caused the outage happening now. Retain structured, time-anchored incident knowledge with links back to its source record; during a new incident, recall relevant experience and patterns beside live operational evidence. Responders must verify every historical lead against current telemetry and records.

How can an incident response agent remember what fixed a previous outage?

Hindsight documents three core operations: Retain stores information in memory banks, Recall retrieves memories, and Reflect reasons over retrieved memories. Its memory hierarchy includes world facts, experience facts, synthesized observations, and curated mental models. For incident response, that offers a way to retain both case-specific history—what happened and what responders tried—and higher-level observations synthesized across cases.

A remembered mitigation is useful only with its conditions and provenance. “Restart the worker” without the affected service, observed failure, timing, evidence, and outcome is not a dependable operational instruction. Treat memory as a navigational aid to incident records and current evidence, not as an authority that can establish a live root cause.

What should we retain from an SRE incident?

Hindsight’s Retain API analyzes source content and decomposes it into structured facts; its documentation says the original content is not stored verbatim as the memory representation. Context and metadata can preserve source details, timestamps anchor temporal references, and a stable document ID supports updating an existing record. Keep the durable incident or postmortem record elsewhere if responders need to inspect the original timeline, logs, or discussion.

A practical incident record

The following is a design recommendation, not a Hindsight-mandated schema. Retain the fields that help a future responder judge whether the old case is relevant and verify what happened:

  • Identity and provenance: stable incident/document ID, service or component, incident-system reference, and source context.
  • When and what: event timestamps, symptoms, affected dependencies, and the scope of impact.
  • Reasoning and action: hypotheses considered, evidence supporting or contradicting them, actions attempted, and who or what initiated the action where recorded.
  • Outcome: mitigation, whether it was verified, what failed, and any conditions that limited the result.
  • Follow-through: follow-up work and links to relevant source evidence.

Record failed actions as well as successful ones. That is an operational design choice rather than a specifically documented Hindsight feature, but it helps prevent a later reader from mistaking an attempted or ineffective mitigation for a proven fix.

Preserve event time and update the same incident

Use the time the event occurred, not merely the time someone ingested the postmortem. An ingestion timestamp can say when information entered memory; it cannot reliably anchor the incident’s sequence or support time-based reasoning about when a symptom, change, or mitigation occurred. Keep timestamps attached to the facts they qualify.

Use a stable document ID when a timeline evolves into a postmortem, so later information can update the existing incident record rather than leave ambiguous copies. Hindsight documents support for repeatable document updates, but teams still need to decide whether their workflow replaces a record or appends incremental updates and how the durable source record tracks revisions.

How should Hindsight retain and recall incident postmortems?

Retain a postmortem or incident timeline with its event timestamps, source context, stable identifier, and metadata pointing back to the incident system. On recall, Hindsight returns structured memories rather than a substitute for opening the original record. Its Recall API documents retrieval using semantic similarity and spreading activation; the documented memory types include world, experience, and observation. Experience memories can preserve prior actions and interactions, while observations represent consolidated knowledge.

Those distinctions support two different questions: “What happened in this particular incident?” and “What pattern has appeared across incidents?” Keep case-specific experience available for audit, and treat a synthesized observation as a pattern to examine—not as a replacement for the cases that support it.

Choose the record strategy deliberately

Choice What it preserves Trade-off
Event timestamp rather than ingestion time When the incident event or action occurred Requires capturing reliable event times; ingestion time alone cannot stand in for incident chronology.
Stable-ID update rather than unrelated copies A continuing incident record as timeline and postmortem mature Updating keeps identity consistent; the team must define how it handles revisions and incremental additions.
Experience facts alongside observations Specific prior actions and outcomes, plus consolidated cross-incident patterns Experience is case-specific; observations are more general and should remain traceable to supporting cases.

How do we recall similar incidents without treating them as the current root cause?

Ask for precedents using concrete details available in the active incident: service or component, symptoms, error signatures, rollout or configuration identifiers, and relevant time context. A semantic match can surface a useful lead even when the wording differs, but similarity does not prove that two incidents share a cause.

Keep historical memory and live evidence in separate lanes

Evidence lane Examples How responders should use it
Current incident evidence Live telemetry, logs, traces, rollout or configuration changes, and incident updates Use it to test what is happening now and verify or reject a hypothesis.
Recalled incident history Prior symptoms, attempted actions, outcomes, and synthesized observations Use it to choose questions or checks; open the source record and confirm any lead against current evidence.

Show source references with recalled claims whenever the integration permits it. Google SRE’s description of its Incident Hypothesis approach emphasizes current operational data, patterns from similar incidents, facts linked to their source data, and verification by human responders. That is a useful safety principle for an Hindsight workflow: present a precedent as a reason to investigate, not as proof of today’s cause.

A responder workflow

  1. Describe the active signal: state the affected service, current symptoms, error signature, relevant change or rollout, and time window.
  2. Retrieve precedents: ask for relevant prior experience and synthesized observations; keep any recalled incident IDs or source references with the result.
  3. Inspect the source: open the cited incident record to check its chronology, evidence, conditions, and actual outcome.
  4. Test a lead against the present: compare the old explanation with current telemetry, logs, traces, changes, and updates. Record what supports or contradicts it.
  5. Choose and verify action: responders decide whether to run a check or mitigation, then confirm the effect using current evidence and update the incident record.

Google SRE describes reconstructing time-ordered “human trajectories” from chat, incident notes, and command-line entries to analyze response patterns and refine playbooks. That kind of source trail makes later memory more useful: a future responder can examine decisions and evidence in sequence instead of receiving an unqualified summary that a fix worked.

How do we evaluate whether AI incident assistance helps on-call engineers?

Evaluate the complete response workflow, not just whether a memory sounds plausible. Build a review set from closed incidents with source records responders can inspect. Check whether recall finds relevant precedents, whether claims point to evidence, whether suggested checks are actually verified, and whether the assistance helps responders reach mitigation without compromising safety or auditability.

Use reviewable levels of confidence

Google SRE describes evaluation data categories as heuristic bronze, calibrated silver, and human-verified gold. These are Google’s described categories, not Hindsight requirements or a benchmark. A team can use the distinction to avoid treating unreviewed examples as ground truth: label the level of review, preserve the source record, and make clear whether an outcome has been verified by a person.

Interpret published results within their scope

Google SRE’s article “AI Engineering for Reliable Operations” reports a 10% reduction in Mean Time to Mitigate (MTTM) for its Incident Hypothesis informational assistance. It separately reports roughly a 44% reduction in MTTM for supported incidents for Investigation Dashboards, and a 195% increase in overall findings from ML-based anomaly detection as one component of that dashboard approach. These are Google’s own reported results for its systems; they are not Hindsight benchmarks or evidence that another team will see the same effects. The article does not state a publication year in the material cited here.
