Contact Form V15 is a free contact-form template from Colorlib. The publisher’s September 2026 update describes a plain HTML-and-CSS design with a little vanilla JavaScript and a PHP mail script. Colorlib lists the template under a CC BY 3.0 license and reports a 110 KB download. Those are publisher-stated details, not results from independent testing.
What Contact Form V15 includes
The design combines contact fields with space for company details, a telephone number, and an email address. Colorlib describes it as a minimalist form with a creative touch. The template is intended to provide a styled contact form rather than a complete website.

In its September 2026 update note, Colorlib says the template uses plain HTML and CSS, a few lines of vanilla JavaScript, and an included PHP mail script. The publisher says it does not require jQuery, Bootstrap files, or plugins. It also says the form can be used by itself or in Bootstrap 4, 5, or 6 projects; this is a publisher-stated compatibility description, not an independent test.

Colorlib says the form uses browser-built-in validation styled to fit the design. Its PHP script is described as sending the message without navigating away from the page. Neither description by itself establishes that the form is secure, correctly configured for every host, or reliable in a particular deployment.
What to configure before accepting messages
The included PHP script needs a recipient address. Colorlib says it can be set at the top of the file in the PHP folder. Before publishing, verify that this address is correct and that your hosting environment supports the PHP mail handling the script uses. Test the complete submission flow on the deployed site, including successful delivery and invalid or incomplete input.
Client-side validation improves the form experience, but it should not be treated as a substitute for server-side validation and appropriate protections against abuse. The publisher’s page does not provide independent security or deliverability testing, so assess those requirements for your own site and hosting setup.

Download size and page dates
Colorlib reports a download size of 110 KB, down from 1.2 MB in its stated September 2026 update. These figures are the publisher’s figures for the template; they are not independently measured here. The page itself is dated March 6, 2021, while it separately labels the template “Released: September 2026” and includes an “Updated September 2026” note. Treat the latter as the page’s stated update context, not as a replacement for the page’s original date.
