Contact Form V12 is a downloadable HTML/CSS template for a general-inquiry page, not a hosted service that automatically stores or emails submissions. It gives a business a compact two-column layout with a contact form beside its contact details; to receive messages, you must connect it to a backend or form service.
What Contact Form V12 includes
Colorlib describes V12 as a minimalist, two-column contact page. The form column has labelled fields for a name, email address, and message. The adjacent column displays address and contact information. Colorlib’s collection says the columns stack on mobile, and lists the download size as 77 KB. These are the collection’s published specifications, not results from an independent test. View Colorlib’s Contact Form V12 page or its contact-form template collection.

The template is suited to a simple general-inquiry page when you want contact details visible next to the form. The listed fields cover basic correspondence; the available description does not establish built-in qualification fields, file uploads, or other specialized intake features.

What you need to make submissions reach you
The download provides front-end files, not message processing. Colorlib’s collection says to connect the HTML/CSS to a backend or form service. That component handles what happens after a visitor submits the form; the template alone does not establish that a message is stored or delivered.
#1 Best Overall
- Choose and configure a backend or form service to handle submissions.
- Connect the form markup to that processing endpoint and check that the expected fields are submitted.
- Test the complete submission path, including where messages arrive and how errors are shown.
Colorlib does not identify a particular service or document email delivery, spam protection, or privacy and security configuration for this template. Those depend on the processing setup you choose.
License and attribution
Colorlib labels the template CC BY 3.0. Its collection describes the templates as free for personal and commercial use provided the credit link is retained. Check the license and readme bundled with your downloaded copy for the exact terms that apply to that version. Colorlib’s template page lists the author as Colorlib and offers download and preview options.

How to interpret the template’s dates
The template page is dated March 6, 2021, while its details block says “Released: 2025.” A separate, similarly named Colorlib page uses a different URL and gives different dates. Colorlib’s September 28, 2026 collection lists Contact Form V12, but these labels do not establish one definitive release date. Identify the template by its Contact Form V12 name and the contact-form-12 page rather than treating the date labels as interchangeable.
