Colorlib’s Job Application Form V04 is a front-end template for collecting multiple previous jobs in a repeatable, vertical timeline. It gathers a title, employer, month-level dates, current-role status and optional responsibilities for each entry, then updates a visible role count and experience total. Bootstrap is not required, and the demo does not transmit applications.
What the V04 form collects
V04 focuses on work history for a fictional healthcare operations role. Each job card contains:

Applicants can add or remove cards as needed. As dates are entered, a badge recalculates the number of roles and years of experience. Colorlib lists the download at 281 KB; that is the publisher’s file-size figure, not a runtime or performance measurement. No. Colorlib describes the 2026 collection, including V04, as plain HTML, CSS and vanilla JavaScript, without a Bootstrap file, jQuery or a plugin. The collection says the forms can be used on their own or placed in a Bootstrap 4, 5 or 6 project; that is compatibility, not a dependency. Colorlib’s collection page provides that implementation context.

If you adapt the repeated fields, check that every control has a clear accessible name and that keyboard focus remains usable as entries are added or removed. Bootstrap’s v5.3.8 form guidance recommends appropriate names for controls and notes that visible labels are generally best for accessibility and usability. What happens when someone submits the demo?
The demo is front-end only. Colorlib says its templates show a confirmation but do not send an application. The visible form is therefore not a working hiring pipeline: submissions need to be connected to an applicant tracking system or another endpoint before it can collect real applications. Any uploaded files also need secure handling.
