Claude Code Decisions: Where to Save the Choice and Its Rationale

Claude Code decisions made in a session are not automatically team knowledge. Capture the choice, reason, and source in a shared record teammates can access.

Your team agreed on an implementation choice while Claude Code was running on a cofounder’s laptop. A teammate later looks for that choice in a document and cannot find it. The decision may still exist in the session’s conversation, but that does not make it a shared, durable record.

Claude Code’s session context, its local auto memory, and a team-readable decision record serve different purposes. If a decision could change what someone else does next, capture the choice, why it was made, and where it came from in a place the relevant teammates can access.

Where Claude Code decisions can live

There are three distinct layers to keep in mind:

  • The session: The conversation is working context for a coding task. Anthropic says each Claude Code session starts with a fresh context window. A session can contain reasoning and choices, but it should not be treated as a shared team record.
  • Auto memory: Claude Code can retain learnings between sessions. Anthropic describes auto memory as machine-local: its files are not shared across machines or cloud environments, although worktrees in the same repository share the memory directory. It can help the person or environment using that memory; it is not, by itself, a team decision log.
  • Shared project instructions or decision records: Project-level CLAUDE.md or AGENTS.md files can be committed to source control so teammates can access shared conventions and architectural context. Anthropic describes these files as context, not enforced configuration. They can document a decision, but they are not automatically a log of every decision made in a session.

Anthropic’s official Claude Code memory documentation explains the distinction between persistent instructions and auto memory. A separate decision record—such as a project document, issue, or architecture decision record (ADR)—is a practical team convention, not a built-in guarantee of Claude Code.

Why a useful session can still leave a teammate guessing

A coding conversation is optimized for the work at hand. The person in the session can ask follow-up questions, test an approach, and revise a choice. Someone who was not there may not know what was decided, which alternatives were considered, or whether the apparent choice was tentative.

That gap matters even as AI becomes part of routine engineering work. Google Cloud’s 2025 DORA report said 90% of its nearly 5,000 worldwide technology-professional respondents used AI at work. That is a broad workplace-AI survey result, not a Claude Code adoption rate or a measure of how often decisions go undocumented.

Anthropic’s 2026 report, “How Claude Code is used in practice,” analyzed roughly 400,000 interactive sessions from roughly 235,000 people between October 2025 and April 2026. In its classifier’s estimates, people made about 70% of planning decisions while Claude made about 80% of execution decisions, averaged across sessions. Anthropic summarized the pattern this way: “People decide what to build, and the agent decides how to build it.” These are the report’s estimates for its analyzed sessions, not universal measurements of all coding work.

Neither figure establishes how often Claude Code decisions are lost, or what that costs a team. The practical point is narrower: people still make important choices during the work, and a teammate who needs one should not have to infer it from local context.

What to capture at the end of a session

Do not turn every exchange into documentation. First ask Claude Code to identify the decisions made and the reasons discussed. Then select only those choices that could change a teammate’s next action—for example, an API contract, a migration approach, a security boundary, or a deliberate choice not to support a behavior.

  1. Ask for a decision list. Request each choice, its rationale, alternatives considered if relevant, and any unresolved question. Treat the result as a draft to review, not an authoritative record.
  2. Select what has team impact. Leave out transient implementation details that are obvious from the code or unlikely to affect another person’s work.
  3. Write the outcome and reason together. Record what the team chose and why. A bare conclusion may be easy to misapply when the original constraint is no longer obvious.
  4. Include provenance. Point to the relevant pull request, issue, code, test, or session context where a teammate can inspect the supporting evidence. Do not assume that a local transcript is accessible to everyone.
  5. Save it in the team’s durable home. Use the existing project documentation, issue tracker, ADR process, or shared account knowledge system that the intended readers can actually access.

Tandem describes a related human-selection workflow for moving Claude Code context into shared account memory. Its August 2026 article says selection and sync are human-triggered; checkpoint-triggered automatic resync had not shipped when that article was published. That is Tandem’s account of its own product, not a general Claude Code capability or an independently validated standard. Check current product documentation if relying on that workflow.

Choose a record location that fits the decision

Use the team’s existing durable system where possible. The right location depends on who needs the decision and how it will be maintained.

Record location Best fit What to check
Project CLAUDE.md or AGENTS.md Shared conventions and architectural context that should inform future work in the repository. Commit it so teammates receive it, and distinguish guidance from enforced configuration. Link to a fuller decision record when the rationale or history needs more room.
ADR or project documentation A durable engineering choice with trade-offs, rationale, and a history that future maintainers should understand. Make it discoverable from the relevant project area and update the record when a later decision supersedes it rather than silently replacing the history.
Issue or pull request A decision tightly bound to a particular change, bug, or implementation discussion. Make sure the conclusion is explicit and accessible after the work is merged; link out if the decision will guide unrelated future work.
Shared account or team memory service Cross-functional context—such as customer implementation decisions—that does not belong only to one repository. Check who can access it, whether it preserves sources, how people select or sync information, and how superseded choices are handled. Tandem describes this use case for its own service; that is a vendor description, not a comparative assessment.

For each option, ask four questions: Can the intended readers access it? Does it preserve the reason as well as the outcome? Does it point to the session or evidence behind the choice? Can a later decision supersede it without erasing the earlier history? A tool or file that fails the access test is not a shared record, regardless of how well it summarizes the conversation.

Keep session management separate from decision capture

Claude Code guidance recommends clearing context between unrelated tasks and describes compaction as a way to retain a summary of important material, including decisions. Those practices help manage the agent’s working context. They do not make a decision discoverable to a teammate or move machine-local auto memory into a shared record.

That distinction answers a common question: saving a session transcript may preserve conversation details, but it does not automatically create an accessible, concise decision record. For the team, capture the selected decision and its rationale in the shared home; use a transcript or source link only when readers need to inspect the underlying discussion.

Turn a one-time choice into useful project context

When a decision should guide later Claude Code sessions, add a concise instruction or link in the project’s committed context file if that is appropriate. Keep durable guidance separate from a full decision history: a future coding session may need the current constraint, while a maintainer may need to understand when and why the team adopted it.

This avoids asking one file to do two jobs. Project instructions help orient future work; a decision record preserves the choice, its rationale, and its evolution. Neither should be assumed to replace the other.
